DSPs perform a wide variety of tasks and activities based on the valued outcomes of each person they work with, and reflecting the specific skills and abilities of each individual. DSPs encourage, guide and offer training to develop daily living skills, ensuring personal needs are met, promoting active engagement in the community life and ensuring health and safety. DSPs create a positive and enriching program environment for all persons supported. Direct Support Professionals are a vital member of each person's support team. DSP's are expected to know, understand and live the Code of Ethics.

Responsibilities
For this position, we are looking for a great candidate to work with individuals in fostering an environment of personal growth for everyone by enriching the lives of people with intellectual/developmental/mental health disabilities. These responsibilities include:
Working directly with an individual in their home or out in the community
Enhance social skills and opportunities for socialization.
Planning and implementing activities that help foster independence and promote community involvement.
Helping individuals develop social and relationship-building skills.
Maintaining and completing all daily data and monthly documentation.
Modeling productive and appropriate behavior for the individuals you work with planning activity within the community.
Qualifications

Position Details:
Rising Ground has an immediate opening(s) for a Per Diem Direct Support Professional to work directly with one of the people we support in our Community Habilitation Programs in the Bronx/Yonkers. Join our team in teaching daily and independent living and work readiness skills, providing individual counseling, supporting Personal Outcome Measures, promoting community integration, and fostering a person-centered culture. The ideal candidate will be dedicated to enriching the lives of people with intellectual/developmental/mental health disabilities including comfort in supporting ADLs (including assisting with showering, toileting, grooming, etc.) as necessary.
Salary: $17.00/hr. ($17.23/hr. Associate's Degree; $17.73/hr. Bachelor's Degree)

DSP positions are non-exempt and eligible for overtime pay for hours worked in excess of 40 during a single calendar week (Monday - Sunday).
Under the supervision of the residence manager and the clinical team, the Direct Support Professional (DSP) is responsible for the direct care and personal development of each person assigned who lives in a Catholic Guardian Services (CGS) residence. Interactions and supports are to be carried out in a manner that creates a homelike atmosphere and fosters learning, dignity and respect.
Essential Job Functions
?Implements all person-centered supports and services in a respectful and positive manner to assist with the achievement personal outcomes and life goals
Assists each person to overcome barriers and challenges to establish and maintain a network of community-based relationships and valued social roles
Protects health, safety and well-being of each person served to the fullest extent possible, in accordance with the plan of care.
Supports each person with dignity and respect and teaches skills in adaptive behavior, activities of daily living, development of self-help and social skills and which foster greater independence and self-direction.
?Carries out all interventions, procedures and notifications, including but not limited to medical, behavioral and nursing, in accordance with the service plan, agency requirements and OPWDD regulations.
?Fully participates in all meetings and trainings as required
?Attends to personal hygiene needs of each person (i.e. toileting, bathing and changing as per the service plan and as the situation warrants).
?Ensures that all measures for health, safety and well-being are maintained at the highest level by maintaining a clean, safe, personalized, and homelike atmosphere.
?Prepares complete, concise written reports including but not limited to incident reports, progress notes and completes all required documentation in an accurate and itimely manner.
?Ensures actions and services are conducted in a manner that uphold the right to privacy, support informed decision-making and prevent abuse and neglect.
?Transports individuals in agency vehicles or using public transportation in a safe and secure manner.
Carries out meal purchasing, planning, preparation and implementation in accordance with each person's dietary needs and doctor's order's.
?Performs housekeeping, cooking, laundry, light maintenance and light preventative maintenance duties as needed to maintain a clean, personalized, safe and homelike environment.
?Administers prescribed medications/treatments to individuals upon successful completion and maintenance of all AMAP certification requirements and in accordance with agency protocol and OPWDD requirements.
Responds immediately to crisis situations and emergencies per the individual service plan, level of professional proficiency, agency policy and OPWDD regulations to protect the health and safety of each person served.
Prevents, recognizes and immediately reports all incidents, including allegations of abuse and neglect, serious reportable incidents and exceptional occurrences in accordance with agency policy and OPWDD (Office for People with Developmental Disabilities) and Justice Center regulations.
Activates the agency emergency notification protocol and summons police, fire, ambulance or other emergency personnel when warranted.
Acts immediately to prevent or redirect behavior that is threatening or dangerous to theperson served or others and will exercise appropriate interventions in accordance with OPWDD regulations, and/or SCIP-R guidelines, the person-centered plan of care and level of training.
Communicates in a respectful manner with all people including the people served, families and advocates, agency staff and members of the broader community.
Adheres to the DSP Code of Ethics and all confidentiality guidelines in accordance with HIPAA requirements and agency policy.
Minimum Job Qualifications
18 years of age or older
High School Diploma or GED
Six months of related experience is preferred
Valid New York State Driver's License that is maintained in good standing

Salary range: $17.72 - $18.52
Responsibilities
Day habilitation Specialist displays a congenial, warm acceptance for all individuals.
They interact constantly with the individuals to develop a sense of security and belonging.
Is responsible to support and supervise the individuals visiting the particular program assigned.
Interprets the Agency policies and program to the individual, other Day Habilitation Specialist and the community.
Is alert to the expressions of attitudes, feelings and relationships of each individual. Develops an awareness of the individual's needs, not only through the observation and interaction of everyday living but, also through a study of the individual's case history and by appropriate consultation with other members of the staff.
Must keep a record of pertinent observations and impressions about the group and the individual's behavior.
Attempts to form a meaningful relationship with each individual in the group. S/he helps each individual make the most of his/her day at program.
Is responsible for matters of everyday living concerning personal cleanliness of the individual, and cleanliness of the program area, furniture, supplies and habilitation activities.
Provide a solid program of constructive and interesting activities for the individuals.
It is essential that Day Habilitation Specialist know and protect individual's rights.
Maintain control of the individuals within the program in a discreet and non-threatening manner.
Must keep accurate records, pertaining to the day-to-day operation of the Day Habilitation program. Be sure to record and report any unusual or bizarre behaviors or incidents.
Qualifications
High School Diploma or equivalent and 5 years experience in the I/DD field or a Bachelor's Degree in Psychology or related field and 1 year experience in the I/DD field.
Familiarity of IRA regulations relative to residential habilitation.
Proficient in computer applications
Must have a clean and valid NY, NJ CT, or PA driver's license with 6 points or less
Maintain all certifications. Including but not limited to the following: First aid, CPR and SCIP.
