The Unaccompanied Children Long Term Foster Care Program provides a safe and secure placement to Unaccompanied Children (UC) who following an initial placement in an ORR/DUCO care provider facility, and after efforts for the safe and timely release indicated that no sponsor is willing or able to care for the UC, and the UC is expected to have a protracted length of time in ORR care. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in licensed foster homes in the community. We provide therapeutic, medical and educational services, as well as other supports that youth need to achieve success through an array of strength-based and targeted interventions as we link them with family members and/or other sponsors in the United States. We want all children we support to move forward with their lives and thrive.
About the Role:
The Behavioral Support Crisis Intervention Specialist acts as a mentor for the minor, assists program staff in providing stabilization of the minor, teaches social skills, and works with the minor on existing behavioral issues with the clinician. Oversees the minor's progress while in care and shares updates and recommendations with the rest of the team to ensure holistic support for the minor's treatment plan as the minor continues in the existing placement. This role is also responsible for the implementation, instruction and coordination of Life Skills Training for all youth placed in the LTFC program.
Responsibilities
Provide direct support to the children in the LTFC program.
Conduct routine calls to the foster parent to assist with coordination of services
Visit youth placed in the LTFC program on a weekly basis to follow up and assist in their development of independent living skills.
Assist in completing applications and referral forms;
Accompany foster parent/youth (or provide telephonic support) to facilitate school enrollment (backup for Case Manager), appointments and/or access to community resources, as needed;
Complete Life Skill Assessment for each youth within 15 days of admission.
Facilitate individual and group Life Skills training to all youth in the LTFC program.
Create and update, as needed, a comprehensive Independent Living/Life Skill program curriculum. Ensure that the program curriculum includes community involvement activities.
Find and utilize community resources, partnerships, and local out-of-network placements to further support the needs of the youth
Conduct outreach with local community agencies to identify appropriate community resources for youth, ensuring they are culturally and linguistically appropriate;
Tutor and mentor youth as needed.
Attend Treatment Team Meetings as requested.
Document all services provided on a monthly basis.
Qualifications
Bachelors in education, psychology, sociology, or other relevant behavioral science
Two years' experience in a related program for adolescent population
Knowledge of child welfare and immigration related legislation
Excellent verbal, written communication and organizational skills are essential
Proficient in Microsoft Office suite, knowledge of Connections
Bilingual (English/Spanish) a must. Other languages preferred as well.
Must complete Therapeutic Crises Intervention Training (TCI)
Capability to establish and maintain working relationships with staff, community groups, and community-based organizations.
Understanding of practice models such as FTC, PBIS, Trauma-Informed care and evidence-based models a plus.
Ability to transport youth and travel.
Possession of a valid vehicle operator's license and clean driving record
Supervisory Responsibility
This position does not have direct supervisory responsibilities. The LTFC Behavioral Support Crisis Intervention Specialist will work independently with guidance and support from their supervisor and other team members, collaborating closely on case management and service delivery.
Work Environment:
This role requires four in-office days and one remote day per week with flexibility for after-hours, weekend, and holiday support as needed. The position involves collaboration with multiple teams, fieldwork, and travel for foster home visits to assess adjustment and safety of the child in their home environment.
Position Type/Expected Hours of Work:
This is a full-time position with an annual salary of $54,600. The position operates for 5 days from 12:00 PM to 8:00 PM, with a flexible weekly schedule and availability for after-hours, weekend, and holiday support as needed.

The Case Planner Supervisor is responsible for the supervision of Case Planners in an Enhanced family foster care program. S/he will play an integral role with assisting Case Planners with the integration of trauma principles into case planning activities. Supervisors must instruct Case Planners on how to apply the trauma response models into the daily casework activities conducted with the child, parent/caregivers and foster parents. Additionally, supervisors must utilize Solutions Based Casework strategy during their interactions with case planners, youth, birth parents, and foster parents. Enhanced Family Foster Care incorporates evidenced-based and evidenced-informed models with case planning activities into one model of family foster care to achieve desired safety and permanency goals in a timely manner.
Salary Range:

Essential Functions:
Oversee a unit of 4 Case Planners and a Socio-therapist The unit will be responsible for approximately 40 active cases
Facilitate the attainment of EFFC goals by supporting and advising appropriate case planning activities
Coordinate the child welfare and administrative work of Case Planners, stressing the importance of each case adhering to permanency policy and ASFA timelines
Work closely with TRIPP trainers to ensure staff learns and are able to implement the model within the agency
Provide group and individual supervision to Case Planners to address case-related issues/themes and review case progress and compliance with EFFC expectations
Complete necessary case-related and supervisory documentation
Review and approve all FASPs, progress notes, Court Reports, correspondence and other documentation for quality and timely submission
Participate in program development and quality improvement activities
Present at weekly case conferences
Complete other duties as assigned.
Minimum Requirements
Masters of Social Work or equivalent human services graduate degree
At least 2 years of documented supervisory and relevant experience
An understanding and respect for community needs and cultures

Job Summary
This position represents the rare opportunity to combine a trauma-focused therapeutic intervention with a holistic approach to casework, helping young people through the stages of healing from trauma while helping the family create a trauma-sensitive environment. Trauma Systems Therapy (TST) is a comprehensive, phase-based treatment program for children and adolescents who have experienced trauma. Major Responsibilities / Activities:
Partner with families to provide assessment, treatment planning, interventions and case management to prevent foster care and promote wellbeing.

EFFC Program Description
Catholic Guardian Services' Enhanced Family Foster Care (EFFC) program provides family permanency services to children and families served within the program. Our program teams are interdisciplinary and are composed of Family Specialists/Case Planners, Social Work Supervisors, Directors of Socio Services, Parent Advocates, Case Aides and Socio-Therapists.
Our work includes the implementation of evidence-based models to address trauma, mental health and other challenges faced by children and families. We successfully provide wrap-around services and support during and after foster care placement by having direct access to Catholic Guardian Services' portfolio of social services and behavioral health programs. These programs include children's health home care management services, evidence-based preventive services programs, outpatient mental health services, career development and job readiness services for adolescent. Our Enhanced Family Foster Care Teams closely collaborate with these programs to ensure ongoing support to our children and families.
Major Duties & Responsibilities:
Participates in training and consultation sessions to develop skills in evidence-based and evidence-informed models and ensures that direct reports effectively utilize these models and do so with fidelity.
Attends internal and external meetings as necessary. Including interdisciplinary team meetings.
Maintains a professional and collaborative approach while interacting with the Socio-Therapist Team.
Participates in family team meetings and utilize learned skills that demonstrates an ability to understand, assess and engage families, children and foster parents in the service plan process and create workable and realistic service plans.
Engage families and effectively delivery services for purposes of reunification, addressing issues which necessitated placement.
Participate and complete of all required and mandated trainings.
Perform other duties as needed.
COMPLIANCE OVERSIGHT
Case Planners/Family Specialists are instrumental in making sure that our casework practice and service delivery are aligned with the agency's and ACS' performance expectations. As such the agency's expectation is that all foster care programs Case Planners/Family Specialists adhere to the following:
Comply with all due dates set by your supervisor for the completion and submission of all FASPs, PHRs, PYA and CANS.
Comply with all National Youth Transitional Database (NYTD) entries in Connections. This includes updating education tabs, making sure that ILS service goals are listed in all FASPs (or Plan Amendment) for all eligible youths, and documenting tribal affiliation progress notes.
Ensuring that Family Team Conferences (SPRs) are scheduled and held by identified due dates. If a conference does not take place on the first scheduled date a rescheduled date must be immediately requested.
To prevent higher rates of missed and rescheduled conference Case Planners are expected to reach out to birth families, foster families and children to identify good/bad times to hold the conference.
Monthly submission of your Case Planners' Self-Reporting Contact Compliance Report. Due to your supervisor the last workday of the month. The report tracks all required contacts and the timely documentation of these contacts.
Timely documentation of all progress notes. As per agency policy progress notes are to be documented within five (5) days of the event/activity.
Documenting child placement status change in Connections via a Notice/Approval Progress Note and making sure that a Child Tracking Form is submitted to your supervisor within 24 hours of such status change (i.e. AWOL, trial discharge, final discharge, hospitalization, detention facilities, etc.).
Monthly submission of your FEDERAL CONTACTS (Case Planner-Foster Child face-to-face contact) calendars to your supervisor by the 4th Monday of the current month for the upcoming month.
Making a successful face-to-face contact with EVERY child on your caseload by the 15th of each month and bringing missed contacts to the attention of your supervisor if not achieved by required date.

Sex trafficking screening in the Child Trafficking Database (CTDB)
MHCU hospital admission and discharge notifications to ACS
Making sure that you are prepared for and attend ALL court hearings on time. This includes preparing the Court Update Report

CPST services are goal-directed supports and solution-focused interventions intended to address challenges associated with a behavioral health need and to achieve identified goals or objectives as set forth in the child's treatment plan. CPST services must be part of the treatment plan, which includes the activities necessary to prevent, correct or ameliorate conditions discovered during the initial assessment visits. CPST is an intervention with the child/youth, family/caregiver or other collateral supports. This is a multi-component service that consists of therapeutic interventions such as counseling, as well as functional supports. Activities provided under CPST are intended to assist the child/youth and family/caregivers to achieve stability and functional improvement in daily living, personal recovery and/or resilience, family and interpersonal relationships in school and community integration. The family/caregiver, therefore, is expected to have an integral role in the support and treatment of the child/youth's behavioral health need. CPST is designed to provide community-based services to children and families who may have difficulty engaging in formal office settings but can benefit from home and/or community based rehabilitative services. Provide weekly individual counseling sessions with pregnant youth and bi-weekly individual counseling sessions with parenting youth; fulfill the required contacts with the biological families of these youth; make home visits as required
Co-facilitate weekly therapeutic group with pregnant and parenting youth in the program

Job Responsibilities:
Case management responsibility for 14-18 young adults who are living in New York Foundling's ESSHI Harlem location. May be required to report to Foundling's Manhattan, Bronx, or Brooklyn offices.
Adhere to Fair Futures Coaching model and provide Core Case Management services for each case, with the goal of supporting participants independent living skills and overall wellness.
Coordinate youth's service needs. Make appropriate referrals and follow-up with participant and service provider to ensure young adult is accessing services. Where youth is not accessing services, coach young adult and advocate on their behalf to help remove barriers.
Conduct required monthly in-office and home visits with assigned cases. Complete additional meetings as needed to ensure optimal outcomes for the case.
Provide flexible scheduling to complete case meetings and prioritize participants schedule.
Collaborate with program vocational specialist to ensure that they are receiving appropriate vocational and educational referrals and skill building coaching.
Work with young adult to facilitate their integration into the community, support their ability to live independently, support the develop of relational skills to secure a broad support network.
Supervise or facilitate workshops, field trips, community days and enrichment activities for young adults in the program.
Attend trainings and conferences for professional development.
Maintain clear, concise and accurate documentation of progress that promote peer and supervisory review and feedback, and that demonstrates compliance with evidenced based clinical model and meeting our agency's expectation. Assure documentation (data entry) for contacts, incidents, and reports are submitted timely (as indicated by program).
Timely data entry of all services practices of the program in agency record and for reporting, and other system as provided.
Receive supervision from NY Foundling leadership. Attend mandatory weekly supervision and consultation on-site at identified location by the supervisor.

The Trauma Recovery Center (TRC) of Rising Ground understands the importance of removing barriers to care and providing trauma-informed services. Through this dedication, the TRC aims to transform services for survivors of violent crime. Rising Ground's TRC is designed to help victims of violent crimes overcome barriers to accessing mental health treatment, health care, and legal resources in the acute aftermath of trauma. The TRC supports recent victims of violent crime, including survivors of physical assault, sexual assault, gun shot wounds, stabbings, domestic violence and human trafficking. The Trauma Recovery Center facilitates healing for survivors of trauma, violence, and loss through an innovative model that blends comprehensive care, advocacy and outreach.
The Clinical Social Worker/Therapist of the Trauma Recovery Center (TRC) will provide clinical intakes,assessments, planning and implementing individual and group psychotherapy, assertive outreach, and clinical case management services, using evidencebased practices that are effective for the service population. The ideal candidate will bring a passion for social justice and anti-oppressive practice, ample experience providing clinical services to survivors of trauma, and a desire to work collaboratively with a team of creative and committed advocates. All candidates must have a strong understanding of trauma-based work and have experience providing crisis intervention and emotional support to trauma survivors. The ideal candidate recognizes that “the most harmed are the least helped,” understands the importance of removing barriers to care, and has a passion to increase accessibility of services to survivors of violent crimes.
This position is located in our downtown Brooklyn location with a hybrid schedule of 3 office days, and 2 days remote. To meet program needs, all staff in the TRC will conduct home visits and accompaniment/advocacy in the community, as needed.
Responsibilities
Clinical Intervention, Consultation, Psychotherapy, Liaison, Outreach and Case Management :
Conducts clinical intakes of new clients using a trauma-informed approach and identifies service needs when appropriate. Assessment includes identifying presenting problems, psychosocial history, mental status exam, and DSM-5 diagnosis.
Clinical interventions include: crisis intervention and emotional support to clients.
Clinical services also include short-term, trauma-informed individual and group psychotherapy as well as comprehensive clinical case management, liaison with other health care providers, and referral and accompaniment to community services when appropriate.
May visit patient at home and/or in the community for the purpose of outreach and follow-up if the patient is unable to keep regularly scheduled appointments.
Caseload consists of complex clients, all of whom have histories of trauma, including acute and chronic physical and sexual abuse, and are victims of interpersonal violence such as sexual assault, physical assault, domestic violence, gunshot wounds, political torture, and immigration trauma. Many clients also have concurrent medical problems, psychosocial problems, and may have substance abuse problems as well.
Utilizes knowledge of the mental health, medical, and community resources in New York City and maintains familiarity with new programs.
Develops a comprehensive treatment and service plan for each client, integrating knowledge from a wide range of human service resources.
Facilitates coordination of services among various agencies; advocates for and links client to needed services. Helps to eligibilize clients for social security disability, Victim of Crime, and other financial entitlements. Provides assistance with U-Visas and psychological asylum evaluations as needed. Serves as a client advocate and liaison with the District Attorney's Office and Police Department for victims of crime wishing to file police reports, restraining orders, etc.
