About the Role
The per diem psychiatrist will be responsible for providing psychiatric services, assessments and consultation in our Passage of Hope Program to support Unaccompanied Migrant Children in our Basic Shelter, Transitional Foster Home, and Long-Term Foster Care programs.
Responsibilities
* Provide high quality psychiatric services to individuals supported within the programs listed above, following current best practice guidelines for care.
* See individuals at least monthly for medication management. Ensure that all medications are reviewed and renewed as necessary, considering both the efficacy of the medication and the well-being and risks of polypharmacy to the individual.
* Provide comprehensive psychiatric evaluations with recommendations and complete written reports within 2 business days of evaluation.
* Ensure proper consents are in place for all prescribed medications.
* Participate in interdisciplinary clinical team meetings as well as staffing with our governmental oversight to ensure comprehensive patient care plan.
* Provide in crisis intervention as necessary
* Participate in the intake process, which includes reviewing referrals packets within 24 hours and participating in Skype interviews as needed.
* Must have flexible work hours and be available outside of regular business hours and on-call as needed.
Qualifications
* A Medical degree with a license to practice in the State of New York.
* A current DEA registration for the prescribing of controlled substance
* New York Ordering/Prescribing/Referring/Attending status to prescribe to Medicaid population.
* An active Health Commerce Account to execute New York State I-STOP law.
* Board Certification or plan for certification with specialization in Child/Adolescent population.
* Bilingual Spanish required.
* Knowledge of Microsoft Office (Word, Excel) and experience with operating an Electronic Health Record and E-prescribing.
* Able to encourage positive attitude toward individuals' unique capabilities and limitations

About the Role
The Passage of Hope Program provides transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the Federal Office of Refugee Resettlement (ORR), our program provides a safe and nurturing environment for youth in our Basic Shelter and Transitional and Long Term Foster Care programs located at our licensed residential facilities and foster homes throughout New York City and Westchester County. While in our care we search, assess, and provide connections with family members and/or other potential sponsors in the United States, so they can move forward with their lives and thrive. In all programs, we provide therapeutic and educational services and other supports that youth need to achieve success through an array of strength-based and targeted interventions. The Teaching Assistant will assist the Teacher in creating, reviewing, and implementing the academic curriculum in accordance with the NYS State Education guidelines for
ages 0 to 17.
Responsibilities
Will assist the Teacher in the development of all lesson plans and curriculum for all assigned youth in the program.
Will ensure that the lesson plans meet the developmental needs of the youth in the program, are in their native language, and provide opportunities for the youth
to learn English, Reading, Writing, Math, Science, Physical Education, Social Studies, and Acculturation.
Will work with the Office of Refugee Resettlement and other governing bodies to ensure that the program meets ORR standards as they relate to the educational needs of the youth supported by the program.

This position is exempt and not eligible for overtime pay for hours worked more than 40 during a single calendar week (Monday - Sunday).
Under the direct supervision of the Senior Residence Manager, and in coordination with the clinical team the Assistant Residential Manager is responsible for supervising all aspects of supports and services provided to each person living in an assigned Catholic Guardian Services (CGS) residence or cluster of residences. The Assistant Residence Manager is responsible for the health and safety of each person, while maintaining a homelike, supportive atmosphere that treats each person with dignity and respect and fosters independence and self-direction to the maximum extent possible.
Essential Job Functions:
Supervises the implementation of all person-centered supports and services to ensure the achievement of personal outcomes and life goals in conjunction with the Senior Residence Manager.
Ensures each person is provided maximum opportunities to establish and maintain a network of community-based relationships and valued social activities

The Program's services are provided by bilingual (Spanish/English) staff who receives specialized training in the special needs of refugee children and in Evidence-Based Trauma Focused-Cognitive Behavioral Therapy (TF-CBT). The program provides a safe, therapeutic environment in a family-like setting that addresses the requirements of each child/youth in a manner that is sensitive to his or her age; preserves cultural, ethnic and religious heritage; and meets dietary needs, native language, sexual orientation, gender identity, and other critical individual needs.
Responsibilities
The Assistant Program Director Provides support to the Program Director and is a secondary liaison with the Office of Refugee Resettlement (ORR).Responsibilities include supervising youth care supervisors, Educational Coordinator and working with clinical and Case Management staff to develop and implement treatment plans for each youth in the home. Oversees the facilities to ensure a safe environment in each of the homes that is sensitive and respectful to the individual's needs and culture, and ensuring that the home functions in accordance with agency policies, as well as New York City, New York State, and federal regulations.
RESPONSIBILITIES
Responsible to supervise the case managers assigned to the shelters and to the foster boarding home program as well as the Educational Coordinator
Oversees that the highest standard of service is provided to each resident; including comfortable living conditions, nutritious meals, adequate clothing, supervised recreational opportunities, and the implementation of each residents' educational plan.
Oversees the adequate maintenance of each residence including the proper functioning of all utility systems. Collaborates with maintenance staff to ensure that work assignments are completed.
Ensures adequate staff coverage 24 hours a day, 7 days a week, for each residence. Provides coverage of the residence in the event of staff shortages.
Oversees the house budgets, including funds for food, clothing, recreation, transportation, and petty cash.

Responsible for ensuring the overall functionality of every dwelling (Home, Apartment, Dorm Room and alike) that encompasses The New York Foundling, DD “Developmental Disabilities” divisional footprint. This responsibility is not limited in any way and includes the entire Structure, Grounds Landscape, Hardscape, Driveway, Parking, Mechanical (Boiler, Hot Water, Generator, Pump and Motor) any and all association to maintenance of dwelling or commercial property are included in this statement.
The AM will be the liaison to all trades, utilities, service providers, and when a dwelling is leased/rented the Landlord and or their assignee, to ensure the dwelling meets the standard of living defined by OPWDD/NYCRR.
The (AM) will assist in the coordination and will serve as one of the liaisons between New York Foundling Hospital, The Developmental Disabilities Division, Area Manager, Utility providers, Fire Department, Building Department, State Health Department, OPWDD and others. The (AM) will remain knowledgeable of all codes, standards and practices generated by regulatory agencies having jurisdiction on dwelling especially OPWDD.
Should the dwelling require maintenance, part replacement or maintenance that exceeds the abilities of New York Foundling maintenance team and/or exceeds $1,500.00 value, the AM must contact the appropriate DD area manager responsible for dwelling to obtain approval to proceed with work. The AM must obtain approval in writing from DD. Moreover, before proceeding with work (repair/replacement) expense, must be approved by the Facilities Senior Manager or Facilities Director. All warranty, other relevant information supporting the expense will be forwarded to the Facilities Coordinator or the Facilities Administrator for pre/post activity or warranty reference.
Proactively inspect each dwelling without limits, he/she will be required to complete comprehensive inspections regularly, for each of the dwellings that make up the New York Foundling portfolio. During each inspection the AM will make certain the dwelling is mechanically, structurally sound that all assets, fixtures and mechanical equipment within the dwelling remain fully operational, maintain compliance with all regulatory agencies. At the time of inspection, AM will document any and all deficiencies. Each deficiency will be recorded electronically, using New York Foundling Hospital approved work order program (Faciltiy4you) so that each deficiency has a unique individual record.

The Plus Mentoring program is designed to help juvenile clients make safe decisions and avoid further involvement in the juvenile justice system. The Plus Mentoring team works along with the Department of Probation to provide an alternative to detention group program for high risk youth, ages 13-18 in Brooklyn, and in the Bronx.
The Plus Mentoring team is a group-based mentoring program that helps young adults transform the attitudes and behaviors that led to their involvement with the criminal justice system by developing positive and appropriate relationships and supporting them in making safe decisions in the community. The team is made up of a full-time Program Coordinator and 3 part-time Credible Messenger Mentors. Youth will come together twice weekly as a cohort of up to 16 youth, ages 13-18, with the Plus Mentoring team supporting them. The mentors will utilize the Powersource Curriculum and ROCA Rewire CBT to support the youth to examine their feelings, evaluate their own behaviors and develop strategies for change. Group will also be used for group builders, educational activities, anger reduction strategies, recreational outings, group-based community service activities, and family events to name a few. Youth will also meet with mentors and Program Coordinator on an ad hoc basis throughout the week for one-on-one mentoring as well as for support with case management services.
Responsibilities
Participate in two therapeutic groups per week in Brooklyn on Tuesdays and Thursdays from 4:00 PM to 6:30 PM. During this time, the mentors will greet the group participants, prepare for dinner, facilitate group, clean up and process the group/individual progress. Mentors will be available during these hours, and will be available outside of group hours to provide crisis intervention or additional mentoring support to youth, by phone and in person, as needed, for up to 8 hours total per week.
Participate in as many group team-building and family engagement activities as possible, such as recreational outings, community-service activities, etc. The mentors will be included on a rotating on-call list to provide 24 hours/7days a week support for youth.
Participate in all Juvenile Justice and Credible Messenger Mentor-related training.
Participate in individual supervision and treatment team meetings to review youth's progress/challenges and service plan goals.
Collaborate and communicate with the Plus team members regarding case planning services and safety/risk concerns.
Work in partnership with Probation Officers, schools and other community-based organizations to ensure the safety of the youth and to follow through with their participation in court-mandated services.
Model reliable, supportive and positive relationships to help youth navigate challenges and achieve their goals. Build and maintain supportive relationships with the youth that focus on improving problem solving and decision-making skills.
Model professionalism and respectful behavior in both the community and in group.
Work Environment: Groups will be facilitated at the Rising Ground office located at 151 Lawrence Street 5

Floor, Brooklyn NY. On occasion, the mentors will be required to travel, to navigate NYC roads/public transportation system and complete home and school visits, and visits to other service providers' offices (i.e. Court, Probation, resource center, pro-social activity, etc.).
Position Type/Expected Hours of Work: This is a per-diem position involving 6-8 hours per week. The mentors that are expected to participate in the Brooklyn groups are to be present every Tuesday and Thursday from 4:00 PM to 6:30 PM.
Additional hours will fluctuate to meet the needs of the team and clients. As a team, we are available to clients 24/7/365 and can respond to crises that may emerge. The per diem mentors might be called upon to respond to a crisis/need of a youth and need to be flexible/available to meet the needs of the group members.
Travel: The Plus team provides services to youth and families who reside in Brooklyn and the Bronx. Typically, the mentors will be positioned at the group site. If field work, home visits or school visits need to be conducted, the mentors will provide their own transportation and work-related travel expenses will be reimbursed.
Qualifications
High School/equivalent diploma is required.
Experience with group facilitation and experience working with adolescents.
History of criminal/juvenile justice involvement and familiarity with the Brooklyn community is preferred.
Bilingual Spanish/English speaking is a plus.
Familiarity with Brooklyn and knowledgeable about the major culture groups within the neighborhoods served.

The Unaccompanied Children Long Term Foster Care Program provides a safe and secure placement to Unaccompanied Children (UC) who following an initial placement in an ORR/DUCO care provider facility, and after efforts for the safe and timely release indicated that no sponsor is willing or able to care for the UC, and the UC is expected to have a protracted length of time in ORR care. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in licensed foster homes in the community. We provide therapeutic, medical and educational services, as well as other supports that youth need to achieve success through an array of strength-based and targeted interventions as we link them with family members and/or other sponsors in the United States. We want all children we support to move forward with their lives and thrive.
About the Role:
The Behavioral Support Crisis Intervention Specialist acts as a mentor for the minor, assists program staff in providing stabilization of the minor, teaches social skills, and works with the minor on existing behavioral issues with the clinician. Oversees the minor's progress while in care and shares updates and recommendations with the rest of the team to ensure holistic support for the minor's treatment plan as the minor continues in the existing placement. This role is also responsible for the implementation, instruction and coordination of Life Skills Training for all youth placed in the LTFC program.
Responsibilities
* Provide direct support to the children in the LTFC program.
* Conduct routine calls to the foster parent to assist with coordination of services
* Visit youth placed in the LTFC program on a weekly basis to follow up and assist in their development of independent living skills.
* Assist in completing applications and referral forms;
* Accompany foster parent/youth (or provide telephonic support) to facilitate school enrollment (backup for Case Manager), appointments and/or access to community resources, as needed;
* Complete Life Skill Assessment for each youth within 15 days of admission.
* Facilitate individual and group Life Skills training to all youth in the LTFC program.
* Create and update, as needed, a comprehensive Independent Living/Life Skill program curriculum. Ensure that the program curriculum includes community involvement activities.
* Find and utilize community resources, partnerships, and local out-of-network placements to further support the needs of the youth
* Conduct outreach with local community agencies to identify appropriate community resources for youth, ensuring they are culturally and linguistically appropriate;
