Works as part of the interdisciplinary team to provide a holistic approach to end of life care and symptom management. Creates a specialized plan of care, in collaboration with the patient, family, physician and interdisciplinary group (IDG), to ensure quality care is provided and patient care needs are met.
Essential Duties and Responsibilities The following statements describe the principal activities of this position identified and shall not be construed as a detailed exposition of all the duties that may be inherent in this position. • Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family/caregiver to determine hospice needs. • Provides a complete physical assessment and history of current and previous illness(es); • Provides professional nursing care, • Initiates the plan of care and makes necessary revisions as patient status and needs change; • Uses health assessments to determine nursing needs and any follow up needed; • Develops a plan of care that identifies interventions which will allow patient and family/caregivers to meet their goals; • Administers medications and treatments as prescribed by the physician in the physician's plan of care; • Provides education to the patient and family/caregiver as appropriate per assessment and plan of care; • Assumes responsibility to coordinating patient care for assigned caseload; • Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given; • Records pain/symptom management changes/outcomes as appropriate; • Documentation will be completed and returned to the office within 24 hours of assessment; • Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required; • Assists the patient and family/caregiver and other team members in providing continuity of care; • Works in cooperation with the family/caregiver and IDG to meet the emotional and spiritual needs of the patient and family/caregiver; • Attends IDG meetings; • Ensures that equipment, supplies, medications and other necessary items and ancillary services are available and delivered/provided when needed; • Actively participates in quality assessment performance improvement teams and activities; • Participates in on-call duties as needed; • Knowledgeable of admission process and paperwork; • May complete admission visits as needed; • Represent the Company in a courteous and professional manner at all times; • Complies with Compliance Plan; • Complies with HIPAA Policies. Maintains confidentiality of information relating to the patient and family. Will discuss only those aspects necessary to the care and treatment of patient and family with those directly involved in the patient's care; • As business needs may change, may perform additional responsibilities assigned by supervisor that are reasonably related to the position. Education and Experience Requirements Education: • Graduate of an approved School of Professional Nursing; • Current state RN licensure in the state of practice; • Possess and maintain current CPR certification. Experience: • Hospice experience preferred but not required. EOE #CE
