Summary
Responsible for assisting the Director of Clinical Operations and Administrator in planning, organizing and directing of patient care services in accordance with current standards and regulations. This position is responsible for the overall supervision and coordination of an interdisciplinary team of staff to ensure the quality of care is delivered safely to home health patients. The Clinical Manager directs patient assignments, staff scheduling, and coordination of referrals, patient visit volume distributions, and assists in meeting financial targets by effectively managing utilization, productivity, personnel and supplies. This position ensures adherence to clinical best practices to promote optimal patient outcomes and patient experience and ensures that patient needs are continually assessed through development, implementation and updates to the individualized plan of care in coordination/collaboration with the Case Manager, physician and patient.
Requirements
Education:Bachelor's degree required, masters preferred.
Experience:Minimum of 2 years clinical experience and 3 years Home Health experience required. Demonstrated leadership skills or experience required.
Certification:BLS (American Heart Association / Basic Life Support) required.
License:State of Florida Registered Nurse License or Physical Therapist License or Occupational Therapist License or Speech-Language Pathologist License required.
Other:Must establish and maintain positive and effective working relationships with staff, physicians, patients and other departments. Proficient computer skills.

Summary
The Case Manager has clinical expertise with knowledge of medically appropriate criteria, and applies principles of utilization and quality management, discharge planning and the management of clinical/financial resources as a facilitator and active participant of the multidisciplinary patient care team. The Case Manager predicts, facilitates and evaluates the transition of patients across the care continuum and links people to systems that provide them with resources, services and opportunities to achieve and maintain maximum functioning.
Requirements
Education:
Associates Degree in Nursing with ongoing efforts to obtain Bachelors Degree.
Experience:
Minimum of 3 years clinical experience required. Previous experience in discharge planning, utilization management, knowledge of community resources, assessment skills and clinical and social factors that affect patients functional status at discharge preferred.
License:
Currently licensed as an RN in the State of Florida.
Certifications:
Case Management certification in: ACM, CCM, NACCM, or CMCN preferred.
Other:
Communication and interpersonal skills, basic computer, analytical, grammar and spelling skills. Ability to effectively multi-task and prioritize responsibilities.

Nurse Navigator- Advanced Heart Failure Ambulatory
Nurse Navigator- Heart Care Transition Clinic
What Youll Do:
Educate patients and families on heart failure, medications, lifestyle, and self-management.
Triage and prioritize patients, scheduling follow-ups based on urgency and need.
Identify gaps in care and collaborate with interdisciplinary teams to optimize outcomes
Build relationships across inpatient, outpatient, and community care settings
Stay current on guideline-driven heart failure care and best practices.
Requirements:
RN license in Florida; BSN preferred.
cardiac nursing experience is a must; heart failure, palliative care, or transplant experience preferred.
BLS required; cardiac specialty certifications preferred.
Strong patient education, communication, and care coordination skills.
Able to work autonomously, prioritize effectively, and advocate for patient needs.
