c. Provides services within the scope of practice as defined by state law and in accordance with the POC;
d. Coordination with other IDT members and participate in IDT conferences;

f. Participate in discharge planning, and in-service programs related to the needs of the patient;
g. Act as a consultant to other members of the IDT; and
h. When medical social services are discontinued, submit a written summary of services provided, including an assessment of the patient's current status, to be retained in the clinical record.
IV. MINIMUM QUALIFICATIONS
QUALIFICATIONS:
The social worker shall be an individual who holds a current, valid license as a social worker (LMSW) issued by the Louisiana State Board of Social Work Examiners (LSBSWE), has master's degree from a school of social work accredited by the Council on Social Work Education, and who meets the following:
a. Has at least one year of health care experience
b. Has documented clinical experience appropriate to the counseling and casework needs of the terminally ill;
c. Shall be an employee of the hospice; and
d. When the social worker is employed by one or more agencies, he/she shall inform all employers and cooperate and coordinate duties to assure the highest performance of quality when providing services to the patient.

TASK DESCRIPTION:
Tools and Parts Attendant (21210): Receives, stores, and issues hand tools, machine tools, dies, replacement parts, shop supplies and equipment, such as measuring devices, in an industrial establishment. The Tools and Parts Attendant does the following keeps records of tools issued to and returned by workers, searches for lost or misplaced tools, prepares periodic inventory or keeps perpetual inventory and requisitions stock as needed, unpacks and stores new equipment; visually inspects tools or measures with micrometer for wear or defects and reports damaged or worn-out equipment to superiors; coats tools with grease or other preservative, using a brush or spray gun, and attaches identification tags or engrave identifying information on tools and equipment using electric marking tool.
TASK REQUIREMENTS:
Receives, stores, and issues hand tools, machine tools, dies, replacement parts, shop supplies and equipment, such as measuring devices.
Keeps records of tools issued to and returned by workers, searches for lost or misplaced tools.
Prepares periodic inventory or keeps perpetual inventory and requisitions stock as needed .
Unpacks, inspects and stores new equipment.
Visually inspects tools or measures devices with micrometer for wear or defects.
Reports damaged or worn-out equipment to superiors .
May coat tools with grease or other preservative.
May attach identification tags or engrave identifying information on tools and equipment
Requisition materials, and consumable supplies through military supply system.
Perform other tasks as directed.
PREFERRED QUALIFICATIONS:
Experience with aviation special tools, equipment, and sets, kits and outfits (SKO).
Familiarity and ability to use Microsoft Office.
BASIC QUALIFICATIONS:
One year experience with basic common hand tools.
Employee must provide own steel toe safety shoes.
Must be able to work 1st, 2nd or 3rd shift schedule as required.
Must be able to work flexible work-week schedules i.e. M-F, Tues-Sat, Sun-Th or as required.
Must be able to pass a background check.
Must be able to obtain a favorable National Agency Check.
Compensation
$22.30/hour
Other Compensation
Employees will receive a minimum health and welfare fringe benefit of $2.65 for every hour paid up to 40 hours per week.

**Minimum Position Knowledge, Skills, and Abilities Required:**
+ Must be able to obtain and maintain facility credentials/authorization. Note: US Citizenship is required for facility credentials/authorization at this work location.
+ Must be able to speak, read, write and understand English.
+ High school diploma or equivalent. Vocational/technical training beyond high school.
+ Minimum of seven (7) years of job-experience or equivalent.
+ Operating knowledge of precision sheet metal welding, high vacuum tungsten inert gas welding, shielded metal arc welding, soldering and operating knowledge of CNC equipment is required.
+ Must have or be able to obtain licenses/certifications as needed to perform duties.
+ Must have a valid and current state driver's license.
+ Must be able to operate a government general-purpose vehicle.
+ May be required to obtain and maintain a forklift license.
