Responsibilities

Responsibilities
The RN Manager, Care Management provides leadership, direction, and resource management for case managers and social workers in the inpatient and ambulatory settings. The RN Manager effectively resolves utilization review and denial management issues, provides leadership in resolving psychosocial issues, and removes barriers and complexities for case managers and social workers to effectively facilitate safe discharge planning. This individual has a strong working knowledge of Medicare/Medicaid, regulatory compliance mandates, auditing agencies, national coverage determinations (NCDs) and local coverage determinations (LCDs). Possesses knowledge of various payer sources, benefits, and limitations and understands and applies utilization management standards from the patient's point of entry through discharge. The RN Manager assists in developing and creates ownership for the department's vision and planning, fostering teamwork and an open creative environment for staff. The RN Manager coordinates, implements, and evaluates staff development and manages staff performance proactively and decisively. The RN Manager supports staff leadership activities, including community involvement and participation in committees and research. This individual must demonstrate knowledge of the principles of growth and development of the life span and appropriately interpret information relative to the patient's specific needs.
The RN Manager, Care Management supports leadership amongst the staff through support of activities that include staff development, community involvement, and participation in committees and research. The American Nurses Association (ANA) Nursing Scope and Standards of Practice, the ANA Code of Ethics for Nurses, and the Commission for Case Manager Guideline provide the basis for practice.

Responsibilities
The Registered Nurse (RN) Case Manager works within an interdisciplinary team to facilitate the patient plan of care throughout the continuum of care by ensuring appropriate utilization management, care coordination, resource utilization, and clinical documentation. Qualifications
Rochester: Graduate of an accredited, or those in the candidacy process, baccalaureate nursing program, as recognized by the Accreditation Commission for Education in Nursing (ACEN), Commission on Collegiate Nursing Education (CCNE), National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Starting rate is $90,000-$110,000/ annually, depending upon experience
Responsibilities
Participate in the admission of new residents to assigned homes.
Develop plans to guarantee that the medical and healthcare needs of the residents are met and ensures the appropriate medical follow-up.
Review lab work and coordinates annual physical exams and care with clinics, outside physicians, and hospitals.
Provide special instructions to resident and home staff related to medical diagnosis, allergies, diet, etc., and is available for ongoing consultation to staff regarding medical problems and healthcare.
Clarify medication orders and contact physician to review consumer's status and medication plans.
Participate in quarterly and Annual Individual Plan (IP) meetings for assigned participants.
Create and distribute Individual Plan Medical/Health Reports detailing participants' general health, health care goals, and physician recommendations for each participant.
Provide documentation regarding individual's health status for various support people.
Make referrals for needed medical services and ensure that follow-up care is pursued.
Provide quarterly in-service training for support staff.
Advises supervisor of all concerns and/or issues related to the provision of nursing services.
Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation
Follows strict OSHA procedures regarding blood borne pathogens.
Minimum Requirements
Current Nursing License required.
OPWDD experience preferred.
Minimum 2 years' experience as an RN.
Bachelor's Degree preferred.
Diabetes Education Certification preferred.
Ability to calculate figures related to medical dosages.
Ability to establish a comfortable and supportive relationship with a person receiving supports.

Position Overview:
We are seeking a dedicated Registered Nurse to join our multidisciplinary team in delivering compassionate, client-centered care in a fast-paced, high-impact environment.
At The Recovery Center we provide critical alternative to inpatient hospitalization. We offer walk-in outpatient services 24/7, a 23-hour observation unit, residential psychiatric stabilization, and medically monitored detox services.
Location/Schedule:
3575 Fulton Mill Road, Macon, GA - 36 hours 3-12 hours shift Sunday-Saturday and Every Other weekend 7am7pm or 7pm7am.
Key Responsibilities:
* Assess client health problems and develop individualized care plans.
* Deliver direct nursing care to clients in crisis, including those with mental health and substance use challenges.
* Administer medications (oral, sublingual, IV) and monitor their effects.
Qualifications:
* Current and valid Registered Nurse (RN) license in the state of Georgia.
* Minimum of one (1) year of professional nursing experience.
* CPR and First Aid certification required.

Purpose of this Role
Belle is seeking a full-time, remote Telephonic Nurse Case Manager to help deliver high-quality, member-centered care. In this role, you will play a key part in improving clinical outcomes, supporting care coordination, and enhancing satisfaction for both members and their caregivers.
As a Nurse Case Manager, you will:
Review documentation from in-home visits
Provide telephonic case management for members
Facilitate and track referrals to appropriate health and community resources
Assess Social Determinants of Health (SDOH) and support the resolution of identified barriers to care
Develop, update, and manage individualized care plans in collaboration with members and their care teams
This role is ideal for a compassionate nurse who is experienced in care coordination and chronic disease management, and passionate about improving the health and well-being of members through personalized, holistic support. Responsibilities
Manage, validate, and track all member cases, ensuring timely communication with appropriate healthcare and community resources
Leverage strong people skills to relate to members, show empathy for their situational/clinical circumstances, and seek creative ways to advocate for the member's best interest
Provide feedback on BT documentation
Communicate areas for improvement clearly and constructively to support continuous enhancement of care quality and the member experience
Experience, Skills, & Personality
Active Registered Nurse (RN) license with Multi-State Compact Licensure
2 years of experience in clinical case management
Experience working with older adults
Strong organizational and time management skills
Excellent verbal and written communication skills
Naturally empathetic with strong active listening abilities
Quick-thinking and resourceful problem solver
Passionate about population health and improving outcomes for aging populations
Skilled in coordinating and facilitating timely referrals to appropriate care and community resources
Preferred:
Case management certification
Skilled in motivational interviewing
Bilingual Spanish-speaking
Experience with G Suite, Slack, Tebra, CCIQ, eClinicalWorks
Experience working remotely
