High-Risk Cardiology
High acuity telemetry units
Patients are admitted from ED, CCU, admitting, procedure areas and other hospital units, and from outside hospitals. Most of our patient population is those diagnosed with myocardial infarction requiring cardiac catheterization, congestive heart failure, heart arrhythmias, and pulmonary hypertension.
Cardiac PCU
Most patients are diagnosed with congestive heart failure, heart arrhythmias, pulmonary hypertension, and other life-threatening conditions. With this known diagnosis many may require cardiac catheterization and stent placement. As a leader in advanced therapies for heart failure, we offer care to pre- and post- heart transplant and Left Ventricular Assist Device (LVAD) patients. This unit specifically cares for readmitted LVADs and manage complex issues like pump clots, stroke, and manage medications.
Vascular Surgery PCU
Our nursing staff provides pre/post-operative care to the wide variety of interventions needed for peripheral vascular disease patients. Surgical services include endoluminal and open repair of thoracic and abdominal aortic aneurysms, thoracic outlet decompression, carotid artery angioplasty/stenting, carotid endarterectomies, dialysis access (fistulas/grafts), catheter-directed thrombolysis, and various bypasses and stents for aorto-iliac and upper/lower extremity occlusive disease.
Cardiothoracic PCU
Post Thoracic Surgery patients ranging from lung transplants, lung surgeries, and esophagectomy. In addition, we care for the Cardiothoracic population after they stabilize from the ICU. We will take care of Pre/Post CABG, Valve Repairs, Heart transplants, and other surgical procedures.
Cardiac Surgery PCU
We provide post-operative care to cardiac surgery patients. Our most frequent procedures are CABG (coronary artery bypass graft), Cardiac Valve Repair/Replacement, LVAD (left ventricular device implantation), Aortic repairs, and MAZE.
Cardiac ICU
Patients with MIs, cardiomyopathies and a variety of irregular rhythms, including life-threatening v-fib and v-tach, device designated floor LVAD, IMPELLA, intra-aortic ballon pump, Non post operative side, pre surgical heart intervention patients.
Cardiothoracic ICU
Patients are CTICU, had surgery from their esophagus to diaphragm, open heart surgery, cabbages, valve replacements, active heart failure program (heart is failing and only thing to do for is to add devices) ventricular assist device. Surgical Post Operative area, patients under cardiac umbrella, heart transplants. Under thoracic procedures, lung transplants and lung surgeries. Vascular patient population, specific to vascular procedures with the aorta. CT patient population and Cardiac, Thoracic and Vascular.

Division 2100 is a fast paced high risk cardiology division at Barnes-Jewish Hospital. We care for a variety of cardiology patients including congestive heart failure, left ventricular assist devices, post cardiac device implantation, post EP and Stent procedures. The Staff Nurse (RN) is an integral part of the patient care team using critical thinking skills and clinical expertise to continually assess the patients' status. The Staff Nurse (RN) must be certified in BLS and obtain ACLS certification within 1 year of working on the division.

Job Profile Summary
The Intensive Community Care Manager (ICCM) is a Registered Nurse (RN) who works with our highest complexity patients, their primary care physicians, and other members of the care team that provides hyperfocus case management and field nursing interventions to prevent unnecessary hospital arrivals, keep patients engaged in our intensive primary care model and maximize their healthy time at home.
The Intensive Community Managers (ICCMs) will serve as a clinical lead for the Complex Care Team. They will assess, evaluate, and coordinate the team's efforts to stabilize our highest risk patients, with special areas of focus including safe transitions of care from facilities back to our primary care teams, stabilization of our highest risk ambulatory patients and outreach to patients who are assigned to us but are not engaged in care. This person will perform assessments and design comprehensive plans of care, and drive the actions needed to keep the most complex patients safely at home. This professional will also provide clinical supervision to other team members in delivering the plan of care and in other tasks necessary to meet their needs and engage them in care. As a clinical leader for the team, this person will also be deeply involved in prioritizing team efforts and may also become the direct supervisor for some team members. The Intensive Community Manager works in partnership with the PCPs to draft personalized care plans that address patient's immediate needs that cause a risk for unnecessary hospital arrivals.
This position adheres to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance and policies and procedures.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
Provides in home and telephonic visits to patients at high-risk for hospital admission and readmission (as identified by CM Plan). Main goal to prevent and admission or readmission to the ER/hospital .
Provides home visits to perform initial assessment of patient and the development of care plan as they perform the follow up patient visits, once patient has completed their episode of care management the register nurse (RN) will review patient chart for discharge and conduct final discharge with patient.
Conducts supervisory visits with License Practical Nurse (LPN) and patient to provide any additional education patient may need and to oversee appropriate patient discharge from case management.
Performs clinical and Social determination of Heath screening (SdoH) assessments to include disease-oriented assessment and monitoring, medication monitoring, health education and self-care instructions in the outpatient in home setting.
Coordinate the Plan of Care:
Provides oversight for the License Practical Nurse (LPN) with clear plan of care and education which is mandatory during all LPN visits.
Conducts/coordinates initial case management assessment of patients to determine outpatient needs.
Ensures individual plan of care reflects patient needs and services available in the community or review of their benefits.
Completes individual plan of cares with patients, family/care giver and care team members.
Communicates instructions and methodologies as appropriate to ensure that the plan is implemented correctly.
Assesses the environment of care, e.g., safety and security.
Assesses the caregiver capacity and willingness to provide care.
Assesses patient and caregiver educational needs.
Coordinates, reports, documents and follows-up on multidisciplinary team meetings.
Helps patients navigate health care systems, connecting them with community resources; orchestrates multiple facets of health care delivery and assists with administrative and logistical tasks.
Coordinates the delivery of services to effectively address patient needs.
Facilitates and coaches' patients in using natural supports and mainstream community resources to address supportive needs.
Maintains ongoing communication with families, community providers and others as needed to promote the health and well-being of patients.
Establishes a supportive and motivational relationship with patients that support patient self-management
Monitors the quality, frequency, and appropriateness of HHA visits and other outpatient services.

The ideal applicant will have a strong interest and experience in product liability and transportation related litigation with exceptional analytical and writing skills, the ability to take and defend depositions as well as experience in drafting and arguing dispositive motions in state and federal courts. Responsibilities:
Development of strategies and initiatives to maximize the financial & operational performance.
Oversight of all operational functions related to the safe, efficient, and effective operation of in-park revenue departments; as well as, overseeing the operation of the purchasing department.
Budgeting and maintenance of department operating expenses, labor, revenue and cost of sales to include accurate monthly financial forecasting.
Lead the development, rollout, and execution of new product, pricing, promotional strategies & sponsorship opportunities.
Participate in long range planning for continued net profit growth of in-park revenue departments.
Maintain guest satisfaction levels as relates to park cleanliness, team member guest service, and efficient operations of every revenue location.
Ensure Staffing & Training plans provide the support necessary to consistently execute the Service Mantra - Friendly, Clean, Fast, & Safe; while maximizing revenue opportunities.
Provide guidance, counseling, and development for the in-park revenue management team.
Monitor guest feedback and provide recommendations to the Park Management team to improve product offering and responsiveness to service objectives.
Monitor department team member voice survey scores and assist in the development of initiatives to address areas of needed improvement.
Build effective relationships throughout the Park & Corporate management team to ensure the effective execution of the Park's business plan and ensure company goals are met.
Maintain and adhere to Six Flags standards and actively enforce all park policies & procedures.
All other duties as deemed necessary by Senior Management.
Reporting Structure
Reports directly to the Park Manager.
Dual reporting to the Corporate Directors of Food, Beverage, Retail, Games & Attractions, and Purchasing.
Qualifications:
Minimum of six (6) years of progressive experience in theme park management with a strong background in high capacity restaurant and/or multi-unit concessions management.
Bachelor's degree in Business Administration or related field preferred.
Demonstrated leadership in driving change relative to the guest and team member experience.
Strong analytical skills to monitor and analyze market trends and internal performance trends.
Must be able to demonstrate excellent written and oral communication, mathematics, and interpersonal skills.
Must possess strong leadership and motivational skills.
Accounting/financial skills to effectively prepare and monitor financial plans.
Must possess a valid State Driver's License.

Salary - and Duty Location Recruitment Incentives - and Benefits
Annual Base Salary for newly appointed CBPOs varies as follows:
GS-9: $61,111 - $124,443 per year
Note: A fully trained CBPO is eligible for up to $45,000 in overtime pay in addition to the starting salary.
Officers are eligible to select from an array of federal employment benefits that include health, dental and other insurance plans, generous annual and sick leave, and participation in the Thrift Savings Plan.
LOCATION INCENTIVES: If you accept one of the duty locations listed below, you may be eligible to receive an additional 15%-25% of your salary each year for the first three (3) years of employment in addition to any overtime pay. Some locations may offer 25% for four (4) years.
Duty locations offering 25% recruitment incentives: Otay Mesa, San Francisco, San Ysidro, and Tecate, CA; Key West, FL; etc.
Duty locations offering 15% recruitment incentives: Honolulu, HI; Portal, ND; etc.
Duty location offering 10% recruitment incentives: Calexico, CA
Qualifications:
You qualify for the GS-9 grade level in one of the following ways: experience, education, or a combination of both.
Experience: A minimum of one (1) year of specialized experience equivalent to at least the next lower grade level that includes:
Detaining suspected violators of federal, state, or local laws and/or arresting violators using the proper law enforcement methods.
Enforcing laws and regulations relating to importing, exporting, and/or international shipping.
Utilizing intelligence techniques and behavior analysis to identify potential threats.
Conducting interviews in a law enforcement capacity.
Education Substitution: A master's (or higher) degree or an LL.B. or J.D. from an accredited college or university; OR
Combination of Experience and Education: A combination of specialized experience AND successfully completed graduate-level education.
Other Requirements:
Citizenship: Must be a U.S. Citizen.
Residency: Primary U.S. residency for at least three of the last five years.
Age Restriction: Must be referred before your 40th birthday (some exceptions apply).
Veterans' Preference: Eligible veterans may qualify for excepted service appointment.
Formal Training: Includes two-week orientation and 101-day academy at FLETC in Glynco, GA. Spanish training may be required for certain locations.
