Our prevention, intervention and treatment programs spread the message that there is hope and that substance misuse and addiction are preventable and treatable.
In alignment with our strategic objective to provide support and services to our community members within and outside our buildings, we are excited to hire Case Managers serving Lexington County. These SCORF (South Carolina Opioid Recovery Fund) grant-funded positions provide partnership with Lexington County behavioral health agencies and other community partners to create access points in the community, assist patients navigate and overcome traditional barriers to treatment, and support patients during and following treatment through intensive case management services.
POSITION LOGISTICS
Full-time 37.5 hours a week
Schedule may be dictated by assigned community partner, and may include weekend and evening hours. Flex schedule available as approved by supervisor.
Primary office location is the LRADAC Lexington county office, with alternative locations to include the assigned community partner office.
Must be comfortable engaging in non-traditional work environments which may include ride alongs with law enforcement/EMS or medical provider facilities.
Grant-funded position with a June 30, 2026 end date. Position continuance contingent upon grant renewal.
EDUCATIONAL AND EXPERIENCE REQUIREMENTS
Bachelor's degree in a human service field is required
One (1) year in the field performing the essential duties of the position.
Must have reliable transportation and valid SC driver's license.
Familiarity with Lexington County and experience with designated community partners preferred.
Addiction Counselor (ADC), or ability to gain ADC certification within three (3) years, is required.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provide 24/7 access to intensive case management services by responding to calls for assistance from patients in a timely manner.
Attend community events in and around Lexington County to raise awareness and build relationships with prospective patients and community partners.
Partner with emergency services to engage patients identified as frequent users of emergency services.
Assist patients through referrals to resources to overcome barriers that prevent successful engagement in treatment (transportation, vocational/educational, housing, legal, etc.).
Utilizes crisis intervention/de-escalation techniques as necessary.
Develops, maintains, monitors and updates, as necessary, case management plans for assigned patients.
Collaborates with Treatment Team to monitor patient progress, identify patient needs, and maintain clinical documentation in compliance with all applicable regulatory standards.
Participates in regularly scheduled staff meetings and case management team meetings.
Assists with Point of Entry (POE) duties to ensure timely access to assessments.
Schedule and attend appointments with patients as needed to ensure warm handoffs to community providers.

Position Overview
Perform routine and preventive maintenance tasks, such as HVAC filters, inspecting fire extinguishers, testing smoke detectors, and addressing other safety and compliance-related concerns.
Repair and maintain building system and equipment, including HVAC systems, plumbing, fixtures, lighting, and electrical systems.
Conduct floor, painting, drywall, furniture/fixture, and equipment repairs as needed to maintain the building's interior and exterior aesthetics. Assist with building renovation, remodeling projects, and other capital improvement initiatives.
Manage and maintain inventory of maintenance and building supplies, tools, and equipment, and procure necessary materials as needed.
Coordinate with external contractors for specialized repairs and maintenance tasks beyond the scope of in-house capabilities.
Ensure compliance with building codes, safety regulations, and environmental standards.
Keep accurate records of maintenance activities, work orders, and equipment history using computerized maintenance management systems or other tracking tools.

The Withdrawal Management (WM) Nurse Manager provides direct nursing care to patients and supervises the day-to-day nursing activities on the WM unit. Ensures nursing personnel deliver nursing care within scope of practices. Coordinates care delivery, which will ensure that patient's needs are met in accordance with professional standards of practice through provider orders, facility policies and procedures, and federal, state, and local guidelines.

This dual-modality approach is designed to promote early intervention, streamline referrals, and support long-term recovery by meeting individuals where they are - physically and situationally. Under regular supervision of the Program Manager, staff will spend their time in the community to identify at-risk individuals, conduct brief assessments, provide referrals, and reduce barriers to care. Flexible scheduling may include weekends or variable hours, depending on law enforcement operations.

Participates in regular clinical supervision.
Provides assessments as needed.

The Clinical Counselor's primary duty is the performance of work requiring advanced knowledge, discretion and judgment in the application of therapeutic interventions designed to improve the overall function and wellbeing of patients diagnosed with substance use disorders, mental illness, and/or co-occurring disorders.
Position Logistics
Full-time, 37.5-hour work week (5 days a week)
Routinely 8:00 am - 5:00 pm shifts Monday - Thursday and Friday 8:00 - 12:00 pm Daytime and evening hours are required, with a flex schedule as approved by the supervisor.
Hiring salary range: $48,000 - $62,000 (Level and compensation will be commensurate with experience and licensure.)
We are open to hiring as a Clinical Counselor II, III or IV. Level and compensation will be commensurate with experience and education.
Essential Duties and Responsibilities
Provides evidence-based, best practice therapeutic interventions to individuals, groups, and families with substance use disorders, mental illness, and/or co-occurring disorders.
Completes appropriate screening and/or biopsychosocial assessments for all patients upon admission
Utilizes crisis intervention counseling as necessary
Develops, maintains, monitors and updates, as necessary, individualized plans of care for assigned patients
Manages and monitors patient progress, identifies patient needs, and maintains clinical documentation in compliance with all applicable regulatory standards
Participates in regularly scheduled clinical supervision with assigned Program Manager
Collaborates with other members of the treatment team to provide case consultation and case management services to all patients
Marginal Duties
Assists with POE duties to ensure timely access to assessments
Attends all required meetings and training sessions
Other duties as assigned
Education, Experience & Licensure Requirements
A Bachelor's degree in human service field is required.
A Master's degree in human service field and experience in performing the essential duties of the position for substance use/misuse populations are preferred.
Licensure as an LPHA (Licensed Physician; Licensed Psychiatrist; Licensed Psychologist; Licensed Psycho-educational Specialist; Licensed APRN; LISW-CP; Licensed PN; LPC; LMFT; LAC) or certification as ADC/AADC are highly desired.
Certificates, Licenses, Registrations
Licensed in South Carolina as an LPHA or Certification as an Alcohol and Drug Counselor or Advanced Alcohol and Drug Counselor through Addiction Professionals of South Carolina are highly desired. If unlicensed/uncertified, ability to obtain licensure or certification within three years of hire.
