The Region Intake Specialist performs the entire patient intake process, including communication with patients and families, data entry, insurance verification and MD verification. They build and maintain relationships with referral sources, address customer needs, and lead a daily intake meeting.
Key areas of responsibility:
Performs all daily patient referral and intake procedures including insurance verification and financial/clinical clearance and ensures patients are admitted in accordance with the intake policy. Provides recommendations for process and policy improvements. Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
Stays thoroughly informed about the details of contracts the organization has with insurance companies, healthcare networks, or other partners for Home Health and Hospice services. Ensures patient admissions comply with rules, requirements and terms in the contracts.
Establishes and maintains positive working relationships with current and potential referral sources.
Provides a smooth transition of patients to home care by overseeing patient education, care planning, and coordination with multiple providers, while also negotiating service pricing with insurance Case Managers and other payers within established financial guidelines.
Strengthens and monitors community and customer perceptions of Hartford Healthcare at Home as a high-quality service provider, while collecting and reporting referral data and key customer trends.

WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY:
Community outreach to identify Veterans at-risk
Educate Veteran families and networks on the risk of suicide in Veteran populations, including warning sign recognition, suicide prevention and education of protective factors
Participate in community events, such as education and training workshops
Assist the Veteran and their family with referrals, temporary financial support, case management and suicide prevention services
Help obtain any other services necessary for improving the mental health status and well-being
Ensure the goals of the grant are met according to the SPGP guidelines
WHAT YOU MUST HAVE:
Bachelors degree in Human Services or related field. Military experience can be substituted for this requirement.

For more than 30 years, A Positive Place has been a leader in integrating community health work that addresses social drivers of health and structural inequities with clinical care to prevent and treat HIV, Hepatitis C, and sexually transmitted infections. We provide healthcare, housing, counseling, advocacy, peer support and other supportive services for people living with HIV; testing, counseling and treatment for infectious diseases; gender-affirming health and social services navigation; harm reduction, education, and safe supplies distribution for people who use drugs.
Become a member of our interdisciplinary CHW team and clinical providers with a focus on access to health insurance and housing. Opportunities for growth.
Help clients access and secure health insurance coverage, HDAP drug reimbursement, PReP, and PRePDAP. As housing equals health, this position also partners with tenants to obtain and maintain permanent housing by applying for homelessness prevention/utilities assistance and rental start-up, accompaniment to appointments, leasing up, annual renewals of housing subsidy, landlord payments.

Attending housing appointments with participants for housing interviewing, touring and others

Valid driver's license, clean driving record, and a reliable clean, insured vehicle that has had manufacturer's recommended scheduled services and transports client as appropriate.

Facilitates referral process for patients using Healthify, E-referral, Community Help, and other tools as applicable.
Advocates for patients regarding transportation, interpretation, health insurance, and FHCW services.
The CHW will attend patient appointments with providers and community resources as deemed necessary and will participate in home visits, telephone contacts, office/hospital visits and other means of connecting with clients to reach identified goals.

What You'll Do

Qualifications
We seek the following principal qualifications:
