ROLE of the POSITION
The Early Outreach Coordinator, reporting directly to the Director for Early Outreach, maintains a high level of internal and external public contact serving as a liaison between the Office of Admissions, various academic units and organizations within the University, as well as between prospective students and their parents, school counselors, and the Las Vegas community.
The Early Outreach Coordinator will assist with the development and implementation of effective early outreach strategies and activities to provide access to and potential for success in navigating postsecondary educational information and processes for 25,000+ students and families annually in elementary, middle, and early high school. Outreach efforts should include the development and coordination of outreach to schools, community organizations, students and families and establishing relationships with both on and off campus entities to promote college awareness and readiness efforts in the region. An additional aspect of importance will be the implementation of family participation strategies and activities as well as presentations to parents, students, agencies and educational personnel. These presentations should include but are not limited to: college readiness, career development, financial literacy, study skills, academic programs, linkage between education and careers, etc.
The Early Outreach Coordinator will be responsible for program development, coordination, and implementation for 15+ Early Outreach events each school year. Additionally, the Early Outreach Coordinator will contribute to the implementation and expansion of the university wide summer camp initiative that includes Young Rebels programming as well as 40+ summer camps, weekend camps, and any camps or programming including young students held at or by a university related office, program, department, faculty, or staff. Creating a plan to increase access for students from various socioeconomic backgrounds to the unique academic and social enrichment opportunities available through Young Rebels programming is also a priority of the Early Outreach Coordinator.
The Early Outreach Coordinator will actively participate in grant writing individually and collaboratively to both ensure continued funding from current grants as well as establish new veins of funding to assist with program development and implementation as well as future expansion of staffing and programming. The Early Outreach Coordinator will also assist with monitoring the Early Outreach budget and any subsequent grant budgets. Assisting the Director for Early Outreach in creating a plan for developing programming to increase the number of prospective students in the pipeline and awareness of UNLV across the valley is also a priority of the Early Outreach Coordinator.
The Early Outreach Coordinator will demonstrate the ability to work independently within established goals and general directives under the supervision of the Director for Early Outreach. This position is also responsible for providing leadership in carrying out special initiatives as assigned by the Director for Early Outreach. The Early Outreach Coordinator will be expected to travel both in and out of state and work irregular hours, including evening and weekend programs.

Essential Duties and Responsibilities:
Fosters and maintains an effective working relationship between peers, professional staff, and other personnel within the community
Has a solid understanding of basic hospice regulations, levels of care and admission criteria
Remains up to date on both Federal and State regulations as they change and promptly educates referral sources to changes in the regulations when necessary
Uses the CRM software and community resources to research and identify referral sources, potential medical director relationships and missed marketing opportunities in the community
Has the ability to create new relationships by constantly seeking out new referral relationships and opportunities in the communities served
Maintains an accurate book of business within the company CRM database including updates on daily, weekly, and monthly visits as well as ensuring accurate referral source information within the database
Ensures that all marketing materials are current and up to date with ProCare policy and current regulations-notifies management if any marketing material is out-of-date or incorrect
Plans for and achieves monthly goals for referral contacts and admissions
Is proactive, organized, timely and appropriately persistent in pursuing leads and follow-up
Gathers data on patient referrals to assist with the admission process when requested
Demonstrates ability to problem-solve and make decisions by assessing situations and considering alternatives, seeking guidance and direction as necessary, utilizing proper chain of command
Demonstrates effective time management skills by coordinating activities to achieve maximum productivity and efficiency, willingness to adjust or revise work schedules as requested

(This could include attending the admission, obtaining consents, obtaining attending physician certification of terminal illness and other admission duties to be assigned, when necessary)
Coordinates effective community education programs to increase community awareness on the hospice benefit and to help generate qualified referrals
Directs or assists with the coordination of community awareness events including health fairs, health care mixers and ProCare sponsored community events
Maintains a positive attitude, energy, enthusiasm, initiative, fortitude, perseverance and follow-through with leads and referral sources within the community and always projects a positive, compassionate image as a representative of ProCare Hospice of Nevada
Project a professional, credible, compassionate image by maintaining above the line standards by only representing the positives of ProCare Hospice of Nevada and never indulging in or focusing on negative banter about other agencies, their employees or referral sources, even when it might be true

Qualifications and Experience:

JOB PURPOSE
The SNAP Outreach Specialist supports The Just One Project's mission by connecting eligible individuals and families to the Supplemental Nutrition Assistance Program (SNAP). RESPONSIBILITIES

Knowledge of Clark County community resources and public assistance systems.

The Community Relations Liaison, in conjunction with the other members of the Admission Team, is responsible for activities and actions related to promoting the services and expertise of the organization to current and potential referral sources, patients and their family members, and the community at large. Education: Bachelors' degree in health care related field is preferred.
Credentials: None
Experience: 2+ years of hospice, palliative, home health, or similar experience is preferred.
Core Competencies: Working knowledge of applicable Medicare, Medicaid, and Private Insurance regulations and guidelines. Excellent verbal and written communication skills; must be able to respond to the needs of ordering providers, discharge planners, facility staff, patients, and patient family members. Must be proficient in making meaningful observations regarding changes in patient status and provide accurate, comprehensive, and timely reports to the patient's care team, facility staff, and the patient's family, as appropriate. Demonstrated experience in establishing and implementing effective Marketing strategies strongly preferred.
Other: Valid Driver's License and current automobile insurance.
FUNCTIONS & RESPONSIBLITIES:
1. Work in conjunction with the admission team.
2. Meet with facility staff and other community members to establish an organizational presence within assigned territory.
3. Assist the Admission Team with obtaining any outstanding admission documents and patient financial information, as needed.
4. Goes into the field and establishes presence of our organization to our service members.
5. Responds to referrals for evaluation and/or admission to the organization's program.
6. Verifies and completes all admission and financial information.
7. Explains the program to patients, family members, and other referral sources.
8. Works with admission team for referrals, admissions, and care coordination of patient and family needs.
9. Identifies key referral sources and assists the admission counselors to develop a marketing plan for the program.
10. Attends and participates in in-services, staff meetings and case conferences.
11. Completes all documentation in an accurate and legible manner.
12. Complies with company policies and procedures, laws and regulations and standards of practice.
13. All other tasks and duties deemed necessary and appropriate.
