LPN Responsibilities and Duties
Administration of medications
Documentation of medication administration
Providing resident care in accordance with federal, state and local standards
Facilitate Care Plans as dictated by residents' medical professionals

Admit and orient new residents; follow complete admission procedures.
Ensure that only trained and authorized personnel operate your unit/shifts equipment.
Maintain resident charts, performs charting duties, keeps narcotic counts and medication records, and counts controlled substances with on-coming shift nurse.
Complete forms for transfer and discharge.
Perform or delegate and supervise all care for assigned residents.
Make rounds, records vital statistics data.
Administer medications; perform other professional nursing duties as required. These may include administering enemas, tube feeding and G-tubes, glucometer readings, catheterization, suctioning, administering of intravenous fluids, treatments involving sterile technique dressings, catheter irrigations, isolation technique, setting up oxygen, performing CPR, caring for terminal and deceased residents.
Administer emergency care, notify physician and family or residents representative, of condition change or emergency and render follow-up care according to physicians' orders.
Respond immediately to resident's emergency call system, including for independent living residents) and take appropriate action; document all such occurrences.
Accompany and assist physician with examinations, follow Care Plan, administer medications, stop orders, orders needed, order medications from pharmacy, notify physician of any unusual or abnormal observations.
Perform restorative and rehabilitative nursing for residents by teaching self-help and independence in activities of daily living according to the residents care plan. May help prepare residents for discharge.

The Licensed Practical Nurse provides care to residents. Responsible for maintaining a safe abuse free environment for all residents in the Orchards of Alliance facilities.

The Crisis Recovery Center will transform crisis care by providing 24/7 walk-in behavioral health services meant to immediately identify, triage, address, stabilize and treat persons in mental health crisis and assist those who have substance abuse withdrawal needs.
This position will operate on 12-hour shifts. Nurses will provide companionate and competent behavioral health services in our newly built facility located in Lorain. Nurses will work with a team to provide 23-Hour Observation and / or 3.7 Withdrawal Management Services.
Pay Range: $22.85 - $27.85 per hour
Placement within the pay scale is commensurate with experience and qualifications
POSITION PURPOSE AND OBJECTIVES
Provides a wide range of direct and indirect nursing services to adults/adolescents and children with mental illness, including conducting physical health assessments, mental status examinations, provides education about medication and medication side effects, purpose of medication as well as benefits of medication. Monitors clients placed on medication initially and medication changes. Conducts assessment of strengths, abilities, needs, desires, preferences, referrals and coordination of services. Services are provided primarily in the office. Nursing services are provided under the supervision of the physician.
ESSENTIAL JOB FUNCTIONS
Manage the clients who are only in pharmacological service and complete pharmacological ISP's
Meet with client to obtain vitals, assess compliance, update psychiatrist, and set client up with tele-psychiatrist
Works closely with the physicians, tele-psychiatry physicians, and/or nursepractitioner(s) regarding the care and treatment of clients receiving psychotropic medication. Meets with client to obtain vitals, assess compliance, update psychiatrist, and set client up with tele-psychiatrist
Takes verbal orders from physicians and assist with implementation of written orders. Assists with the obtaining of psychotropic medication via samples or through central pharmacy or calling prescriptions into local pharmacies.
Monitors clients on newly prescribed medication or those undergoing medication changes through scheduled appointments.
Conducts physical/mental health assessments.
Obtains vital signs, blood sugar testing, weights, BMI, waist circumferences, toxicology screens and other parameters relevant to medication monitoring and health maintenance.
Provides education on physical/mental wellness in individual and group settings. Provides education on the effects of alcohol and other drugs on physical/mental health in individual and group setting.
Assists with the administration and/or monitoring of prescribed psychotropic medication including injections. Provides education on the purposes, benefits, risks, side effects of psychotropic medication.
Identifies current and potential physical/mental health care needs and provides needed care and or assists the individual with securing needed care in the community.
Collaborates and coordinates linkages with agencies, treatment providers in the community for continued physical/mental health care.
Responds to routine and emergency requests for medical assistance.
Assists and teaches clients and staff about client safe-care of medical conditions.
Communicates with case manager, counselors, clinical supervisor, and other members of the treatment team relevant information concerning the care of clients especially those dealing with multiple physical/ mental health care problems.
Maintains required productivity expectation.
Attends required staff meetings.
Reacts to change and stress productively.
Maintains regular and predictable attendance and punctuality.

WORKING CONDITIONS
The work is provided at the Crisis Recovery Center.
This individual may periodically be required to lift and/or carry up to 10 pounds. Must be able to physically interact with client and others in emergency situations to prevent harm to client and others.
The individual in this position must be able to communicate clearly both verbally and in writing.
Due to the extensive computer work this individual must be able to sit for extended periods of time and operate required equipment.
REQUIREMENTS/QUALIFICATIONS
Candidates must possess a valid Ohio state license as an LPN. Experience working in a fast-paced environment and with clients who have severe and persistent mental illness preferred.
Phlebotomy experience preferred.
Computer skills including familiarity with Windows and Word applications required.
Favorable references and/or evaluations are required.
