Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care. Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care.

Provide emotional support to patients and their families.

Educates patients and/or families in disease process, hospice care, and signs/symptoms of approaching death.
Implements and follows physician orders.
Notifies Charge Nurse or Clinical Director of significant changes in patient condition and any unrelieved symptoms.
Gives complete report to assigned case manager as needed.
Reports complaints, issues, and incidents to Clinical Director or IPU Administrator.
Delivers care in a cost-efficient manner.
Attends patient deaths.
Works to ensure continuity of care with other team members.
Assists the patient in the Activities of Daily Living (ADL) including personal care, hygiene, bathing, back rubs, shampoo, skin and nail care, shaving, eating, dressing, elimination, ambulation, transfers, changing and making the patient's bed. Very light housekeeping to provide a clean and safe patient environment and meal preparation as directed by care plan.

Responsibilities
In addition to performing visits and completing coordination of client care, the Registered Nurse RN Home Health is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. WORK ENVIRONMENT
