Nurse - Essential Duties and Responsibilities:
1. Assumes the professional responsibility and accountability for a group of residents/patients including:
a. Medication/Treatment Administration
b. Teaching
c. Supervision of all nursing staff to assure implementation
of the interdisciplinary care plan.
2. Performs ongoing and systematic assessments utilizing the nursing process to establish a comprehensive goal directed interdisciplinary plan.
3. Communicates and collaborates effectively with residents/patients, families, colleagues and other members of the interdisciplinary team to assure that treatment is appropriate to meet the age, needs and abilities of resident/patient and unit needs.
Documents accurate, pertinent and comprehensive information in a timely and concise manner as required by regulation & facility policy & procedure.
5. Supervises the care provided by CNAs & LPNs assuring adherence to the plan of care, resident profile, policies and procedures.
6. Participates in educational programs, inservices, meetings and workshops and utilizes current literature to enhance professional skills and knowledge. Attends all annual mandatory programs.
7. Participates in quality improvement activities as directed
Follows standards of the Diligent Minimal Lift Program
9. Performs other duties as required.
Minimum Qualifications:
Education: Associates Degree/Diploma Program, Graduate of accredited school of nursing
Experience: 1-2 years nursing experience in clinical setting preferred. New Grads may be considered. Previous LPN or CNA experience is a plus.

The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Medication Administration
Provide the highest degree of quality care and services by administering medication and treatments in a safe organized manner.
Review, read, notate, and initial the electronic health record to document and learn about pertinent information about residents.
Receive medication updates from Resident Care Director (RCD) or Wellness Nurse.
Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
Ensure that medications are passed according to times utilizing a mobile medication cart.
Document and initial as medications are given and ensure that appropriate documentation is completed for refusal or missed doses.
Maintain confidentiality of all resident information including resident medication among other residents.
Report all resident concerns and unavailable medication while administering the medication to the RCD or Wellness Nurse.
Restock medication cart after all medication passes.
Assist in checking medication regardless of packaging system.
Assess the residents to determine need for "as needed medication" and appropriately document and report to supervisor.
Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift and report discrepancies to the RCD or Wellness Nurse.
Maintain and clean the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
Follow re-fill process for medications.
Help residents maintain independence and promote dignity and physical safety of each resident adhering to the Sunrise Principles of Service.
Strive to understand and respond to each resident with empathy, always remaining mindful of the resident's unique communication patterns, history, and basic human needs.
Practice routinely good standard care precautions of cleanliness, hygiene, and health.
Audit medication carts.
Resident Care
Notify RCD of any resident and/or family concerns.
Attend and actively participates in daily Cross Over meetings facilitated by the LCM.
Conduct Service and Health Updates as directed by RCD.
Participate in the development of the Individualized Service Plans (ISP).
Transcribe orders.
Risk Management and General Safety
Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations.
Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
Report all accidents/incidents immediately.
Report all unsafe and hazardous conditions/equipment immediately.
Ensure any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits, and in compliance with fire codes.
Comply with all infection control techniques, placement of bio-hazard containers, removal techniques, procedures, and policies.
Understand and practice the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.
Report occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.
Ensure oxygen tanks are stored safely, exchange guest/resident's tanks when empty, and monitor to make sure liters of oxygen are at prescribed levels.
Training and Contributing to Team Success
Participate as a member of a team and commit to working toward team goals.
Demonstrate in daily interactions with others, our Team Member Credo.
Commit to serving our residents and guests through our Principles of Service.
Contribute to the overall engagement programs and processes (customer and team member engagement) including participating in the team member engagement survey and engagement improvement planning workshops.
Attend regular meetings; Town Hall, Department Team, Cross Over, Medication Technician and others as directed by the Supervisor/Department Coordinator.
Attend regular training by RCD and neighborhood coordinators.
May be designated as shift supervisor.
May supervise other medication care managers.
Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.
Perform other duties as assigned.

General Purpose:
The LicensedPracticalNurse will provide quality nursing care in the department, as directed by the departmental lead nurse.
Description of Duties:
Perform screening procedures and documentation in accordance with Cornerstone policies and procedures.
Identify potential patient problems, alert the RN or Provider and anticipate orders for intervention.
Perform screening procedures according to Cornerstone policies.
Assist providers with in-office procedures and exams as needed.
Participate in Cornerstone's Care Team Model including but not limited to:
§ Participate in Pre-Visit Planning (see Pre-Visit Planning Policy).
§ Lead care team huddles/communication.
§ Direct population management activities for assigned patient panel.
§ Direct and participate in quality improvement activities within the care team and department.
Administer medication, treatments and vaccines according to provider orders and scope of practice.
Perform hearing and vision screenings on children and adults.
Responsible for carrying out Medical Providers orders for the following actions: EKG's, Emergency response, medication administration (Oral Medications, vaccinations, prescribed treatments) and other procedures according to Provider orders.
Strictly adhere to safe nursingpractice, including but not limited to, Infection Control, National Patient Safety Goals, Medication Management and Waived Testing set forth by Cornerstone policy and incompliance with Joint Commission and OSHA.
Refer phone calls and patient triage to Lead Nurse or Physician.
Float to other departments and the laboratory as needed.
Make recommendations to supervisors for the improvement of health care to all patients.
Ensure a clean and safe patient environment.
Be familiar with, and adhere to, all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Policy and Procedure Manual and Employee Handbook.
Attend and participate in monthly department, All Staff and other required meetings.
Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.

Job purpose

WORK ENVIRONMENT
Nurse Corps Officers may serve at any one of more than 250 Navy and medical facilities around the globe, from Hawaii to Japan, Germany to Guam and Washington, D.C., to Washington state. As a Navy Nurse, you could work at one of the highly acclaimed National Naval Medical Centers in Bethesda, MD, Portsmouth, VA or San Diego, CA. Or you could provide medical support aboard one of two dedicated hospital shipsthe USNS Comfort and USNS Mercy. Additional opportunities are available on surface ships, with aircraft squadrons or even with the Fleet Marine Force.
QUALIFICATIONS AND REQUIREMENTS
To become a Commissioned Officer in the Nurse Corps, qualifications include:
U.S. Citizen between the ages of 18 and 41
Currently licensed and practicingnursing in the U.S. (new graduates must obtain a license within one year of beginning Active Duty service)
In good standing (as a student or graduate) with a CCNE-accredited U.S. education program granting a
Bachelor of Science degree
General qualifications may vary depending upon whether youre currently serving, whether youve served before or whether youve never served before.
PART-TIME OPPORTUNITIES
There are part-time opportunities as a Reserve Navy Nurse. Serving part-time as a Navy Reserve Nurse, your duties will be carried out during your scheduled drilling and training periods. During monthly drilling, Nurse Corps Officers in the Navy Reserve typically work at a location close to their homes.
