Job Summary
The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. Demonstrates knowledge of nursing principles for the resident in the relevant clinical setting. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards related to the physical and psychosocial needs of the resident as per care plan. Possess the ability to perform testing to evaluate data reflective of the resident's status and to provide the care needed. Demonstrates ability to recognize problems, recommend solutions and respond effectively to assist in an emergent situation.
Ensures the health, comfort and safety of residents. Contributes to the assessment of residents and administers medications or treatments as ordered. Provides technical support to healthcare professionals as needed. Documents resident interactions as needed and the outcomes related to resident care, medication, behavior, activity, and dietary. Skilled in establishing and maintaining effective working relationships with resident, health care providers, and the public. Ability to work effectively with professional and supportive personnel as a member of an interdisciplinary care team. Ability to communicate clearly and maintain confidentiality of sensitive information. Counsels residents, family members and/or resident advocate, under the direction of a registered nurse, advanced practice provider or physician on prevention and treatment plans. Knowledge of resident education principles to develop a self-care program. Performs other related duties as assigned.
Qualifications
Graduate from an accredited practical nursing program as a Licensed Practical Nurse. Associate degree in practical nursing is preferred.
If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.
Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum or have had a minimum of 400 hours of experience as LPN at another facility. Specific services/positions may have additional education, training or experience requirements.
Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges. Obtains and subsequently maintains required department specific competencies and certifications. Completes required annual continuing education units (CEU) and competency training requirements of the department and facility.

All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations.
· Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses.

· Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned.
· Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.
· Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.
· Ensures patient awareness related to transplant and treatment modality options.
· Required to complete CAP requirements to advance.

Responsible for serving on the patient care team for an assigned area including direct patient care as an unlicensed professional and under the direct supervision of a Registered Nurse (RN). Incorporates the primary job functions of a certified nursing assistant (CNA) and other duties related to the care of patients, dependent upon the work area and/or patient populations to which they are assigned. Reports to Nurse Manager, Charge Nurse, House Supervisor or Staff RN.
Incumbents holding nursing licensure will be held accountable to Nebraska nursing licensure requirements and must maintain licensure or an active Nebraska CNA.
PRINCIPAL JOB FUNCTIONS:
1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values. .
2. *Recognizes and reacts to emergency procedures per role and policies/procedures.
3. *Answers telephones and assigned communication devices as appropriate and relays information as necessary. Refers all physician telephone orders to a licensed professional.
4. *Assists with patient care that is consistent with standards of care and clinical priorities determined by the RN with the nature of the area/unit, patient care division policies and procedures, demonstrated competencies, and with consideration given to risk management and infection control practices.
5. *Performs and documents routine direct nursing care tasks according to identified age-specific needs.
6. Obtains and records vitals, height, weight, and bedside glucose results; reports results to appropriate personnel.
7. *Performs Activities of Daily Living (ADLs) for assigned patients and documents in the EMR.
8. *Responsible for labeling and securing patient care belongings and ensuring that the patient room is safe and uncluttered.
9. *Distributes patient's meals/water and assists with feeding of patients as needed.
10. Assists with ambulation, lifting and positioning according to procedure, including the appropriate use of patient lift devices and use of good body mechanics.
11. Assists with transporting patients, including at time of transfer and/or dismissal.
12. *Assists patients with elimination needs and measures/records bodily elimination in medical record.
13. Bathes patients as directed by the RN and documents in EHR.
14. Participates as a member of the team in Hourly Patient Rounds to proactively address patient needs.
15. Provides close observation or one-to-one care, as directed by the RN, to prevent patients from wandering or potential injury.
16. Collects, accurately labels and transports specimens to lab as required.
17. Assists with dressing changes, applying splints, setting up suturing and procedures, and wound care on closed wounds as directed by RN.
18. *Cleans, stocks and cares for supplies and equipment in assigned work area.
19. Collaborates with other departments to coordinate exams and treatments, minimizing fragmentation and enhancing patient satisfaction.
20. Assists in collecting specific information from patients and contributes to admission, transfer and dismissal processes when delegated by a RN, and with responsibility for patient assessment being performed by a RN; assures timely reporting and recording of patient's Electronic Medical Record (EMR).
21. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
22. Participates in meetings, committees and department projects as assigned.
23. Performs other related projects and duties as assigned.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
1. Maintains clinical competency as required for the unit including but not limited to age-specific competencies relative to patient's growth and developmental needs, annual skill competency verification and mandatory education and competencies.
2. Knowledge of computer hardware equipment and software applications relevant to work functions.
3. Knowledge of general hospital processes and procedures.
4. Ability to work in a fast-paced environment related to changing patient needs, including work with patients with acute, chronic and complex disease processes, and those who are dying.
5. Ability to practice proper body mechanics to ensure personal and customer/patient safety.
6. Ability to recognize and react to emergency procedures per role and policies/procedures.
7. Ability to communicate effectively both verbally and in writing.
8. Ability to perform crucial conversations with desired outcomes.
9. Ability to establish and maintain effective working relationships with all levels of personnel and medical staff.
10. Ability to problem solve and engage independent critical thinking skills.
11. Ability to maintain confidentiality relevant to sensitive information.
12. Ability to prioritize work demands and work with minimal supervision.
13. Ability to maintain regular and punctual attendance.
EDUCATION AND EXPERIENCE:
Current enrollment and in good standing in a Nursing Program such as an accredited Baccalaureate Nursing Program or RN Associate Degree program required. Active Nebraska Certified Nursing Assistant (CNA) credential required. Six (6) months CNA experience preferred.
Patient Care Technicians working primarily in the Emergency Department may have additional competencies specific to their population.
OTHER CREDENTIALS / CERTIFICATIONS:
Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
