Summary:
A Registered Nurse (RN) in a long-term care (LTC) facility provides skilled nursing care to residents, ensuring their health, comfort, and well-being. RNs oversee patient care plans, administer treatments, monitor for changes in condition, and collaborate with healthcare teams to provide high-quality, resident-centered care. They also play a key role in supervising nursing staff, educating residents and families, and ensuring compliance with healthcare regulations and facility policies.
Essential Work Functions:
Assess patient or resident health, monitor for changes, and take appropriate action to ensure their well-being
Develop, update, and implement personalized care plans in collaboration with residents, families, and the patient care team
Administer medications and treatments safely and accurately
Observe residents for any medication reactions or changes in condition and report concerns to the healthcare provider
Provide treatments including wound care, IV therapy, catheter management, and other skilled procedures within scope of practice
Supervise and support LPNs and CNAs, delegating appropriately and ensuring residents receive quality care
Educate residents and families on medications, treatments, and managing chronic conditions
Document care provided, resident progress, and any health changes in a timely manner
Respond quickly to emergencies, delivering critical care and coordinating with medical teams when needed
Work closely with physicians, therapists, and other healthcare professionals to ensure coordinated, comprehensive care
Perform other duties as assigned within the scope of practice
Adhere to facility safety protocols, infection control guidelines, and regulatory standards
Required Essential Skills:
Critical thinking, service excellence, and good interpersonal communication skills
The ability to read, write, and communicate in the English language
Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills
Physical Abilities - Must be able to remain in a stationary position, move about, move equipment (50-100lbs), push, pull, and bend
Interpersonal Skills - Must be able to work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalism
Technical/Motor Skills - Must have the ability to grasp, perform fine manipulation, push/pull, and move about when assisting with procedures and/or using department equipment
Mental Requirements - Must be able to cope with frequent contact with the general public and customers while meeting deadlines under pressure. Must be able to work under close supervision occasionally, as well as working without assistance from other personnel. Must be able to contend with irregular activity schedules occasionally and continuous concentration to detail

Job Duties
Participate in a multidisciplinary team to develop, execute, monitor, and adjust comprehensive and coordinated care plans that include shared goals with measurable outcomes.
Serve as primary point of contact and coordinator of care for assigned patients.
Assist patients with Social Service needs as necessary.
Participate in Peer to Peer or Care Management reviews with patient relations liaison.
Ensure the Unitized Transdisciplinary Care program and units are scheduled and prepped for presentation.
Present patients to provider teams during care reviews.
Complete patient Care reviews as needed.
Provide nursing services including assist providers with examinations and procedures; administer medications and injections to patients; check patient's blood glucose, process referrals to external providers and community health resources; and relay provider instructions to patients.
Document medical and behavioral encounters in the health record system.
Maintain and stock the clinic STAT Kits, AED and O2 tanks on a regular basis.
Complete follow-ups on behalf of patients and providers including outreach phone calls.
Engage patients and provide care in individual and group settings that focus on achieving care plan goals and education.
Care for a medically complex and emotionally challenging patient population that requires compassion, empathy, and the ability to deescalate and appropriately react to unacceptable behavior in individual and group settings.
Encourage patients to take control and responsibility of their health through self-management and use of community resources.
Participate to be On-Call and be available to take patient calls after hours via phone.
Qualifications
Current Vocational Nurselicense in the state of California.
General computer skills including typing, Microsoft Word, Microsoft Excel, Microsoft Outlook, and medical record data review and entry.
Bilingual (English/Spanish) preferred.

Key Responsibilities:
Conduct comprehensive nursing assessments, including ASAM forms.
Provide routine care at various levels of care.
