Continuously assess resident's conditions and needs; report observations to DON. Contribute toward development of resident care plans, utilizing all knowledge of restorative and rehabilitative nursing principles.
Admit and orient new residents; follow complete admission procedures.
Ensure that only trained and authorized personnel operate your unit/shifts equipment.
Maintain resident charts, performs charting duties, keeps narcotic counts and medication records, and counts controlled substances with on-coming shift nurse.
Complete forms for transfer and discharge.
Perform or delegate and supervise all care for assigned residents.
Make rounds, records vital statistics data.
Administer medications; perform other professional nursing duties as required. These may include administering enemas, tube feeding and G-tubes, glucometer readings, catheterization, suctioning, administering of intravenous fluids, treatments involving sterile technique dressings, catheter irrigations, isolation technique, setting up oxygen, performing CPR, caring for terminal and deceased residents.
Administer emergency care, notify physician and family or residents representative, of condition change or emergency and render follow-up care according to physicians' orders.
Respond immediately to resident's emergency call system, including for independent living residents) and take appropriate action; document all such occurrences.
Accompany and assist physician with examinations, follow Care Plan, administer medications, stop orders, orders needed, order medications from pharmacy, notify physician of any unusual or abnormal observations.
Perform restorative and rehabilitative nursing for residents by teaching self-help and independence in activities of daily living according to the residents care plan. May help prepare residents for discharge.

* The Site Assessment is done during the initial contact and whenever there is a change in address. It includes obtaining personal information, condition of the home, and other safety features.
* During the initial assessment, the nurse gathers more information about the client's current and past medical history, mental and behavior conditions, daily routines, habits, and preferences. Compiling all that information to come up with a care plan to meet the client's needs.
* Makes necessary revisions to the plan of care as patient status and needs change.
* Maintains open lines of communication between nursing shifts, schedulers, supervisors, families of clients, and case managers.
* Conducting an assessment every 60 days in combination with a Satisfaction Survey.
* During the scheduled visits, record their vital signs and see if there are any significant changes or modifications to the care plan. Notify your supervisor. Client's case manager will be notified accordingly.
* Conducting an assessment for a change in condition. Notify supervisor with changes.
* Being available to do unplanned assessments in case of emergencies or as requested by the client or the case manager.
* During the assessments, monitor client's condition & status. Try to detect any changes that indicate improvement or deterioration in their condition. Notify supervisor.
* Observation and supervision of caregivers to assure they are following the care plan.
* Advocate and act on client's behalf to assure individual rights are met.
* Maintain client confidentiality. Respect their choices and beliefs.
* Ensure that arrangements for equipment and other necessary items and services are available.
* Other job tasks as needed.
Communication:
* Communicating relevant information to the proper people to assist in increasing the effectiveness of the care plan.
* Effective communication: verbal and written.

In collaboration with the interdisciplinary care team (Care Management in coordinating care [e.g. Children's Protective Services], transfer centers, and pharmacies), conducts initial and ongoing patient assessment, analyzes assessment data, creates a place of care, educates patient and/or family, implements nursing care/interventions, evaluates the effectiveness of the nursing care/intervention, and advocates for the patient/family.
Provides episodic (outpatient) and inpatient (at designated units) Obstetrical/Women's Health Nursing care were applicable.
Administers medications in a safe manner consistent with the State of Practice and Bon Secours Mercy Health; coordinates care within hospital departments.
Acts as a patient safety advocate by participating in ongoing quality improvement in the department.
Applies knowledge of contemporary obstetrical/women's health nursing practice and future innovations.
Applies the nursing process in the care of women during pregnancy, birth, postpartum, gynecological and other women's health issues and the neonate from birth until discharge with differing gestational ages, diagnoses, and complications.
Provides care to women of all ages requiring triage, prioritization, assessment, education, stabilization, resuscitation and emergency management.
Models core cultural and safety behaviors to eliminate preventable harm, ensure worker safety and safe high quality experience of care.
