Essential Work Functions:

JOB POSITION/SUMMARY
EmPATH will provide non-hospital facility-based services that renders short-term treatment to facilitate access to services and stabilization to prevent ED and acute psychiatric hospitalization admissions.
EmPATH will provide these services to individuals who present to the program or are transferred from area EDs.
Initial admission will be on a voluntary status to adults (18 years and older) who are presenting with a mental illness and/or co-occurring disorder and experiencing a behavioral health crisis.
This position hires, trains, and supervises nursing staff for EmPATH.
Oversees scheduling, monitors training needs, and functions in a leadership role in the multi-disciplinary EmPATH treatment team.
This position will be responsible for ordering supplies for the unit and monitoring all nursing duties.
Completes chart reviews to ensure agency compliance.
Provides direct care to clients as needed and assists with crisis interventions and medical emergencies, Serves as a liaison with other agencies as indicated.
This position requires certification in and adequate implementation of verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, ability to go down on knees, running, and walking.
Regular attendance is an essential job function.
Required to provide nursing duties and coverage despite any weather conditions or holiday that arise.

COMPENSATION:
Starting salary for this position is approximately $34.34 /hr based on relevant experience and education.
QUALIFICATIONS - EmPATH RN Team Leader
Experience:
Associate's/Bachelor's Degree from an accredited College/University.
Experience in psychiatric nursing and acute care preferred.
Must have the ability to present professionally, work within a team format, and have the mental ability to exercise sound, clinical judgment under pressure.
Must have the ability to communicate effectively (both oral and written) and possess good time management, organizational, and supervisory skills.
Basic computer skills are required.
Experience with electronic medical record (EMR) is preferred.
Experience with interdisciplinary collaboration and care coordination preferred.

Essential Job Functions

Minimum Data Set:
Oversee and coordinate the development and completion of the resident assessment (MDS) in accordance with current Federal and State rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPs and Triggers.
Assemble information from the Initial Nursing Assessment, resident interview, and clinical record review to complete the nursing portion of the Minimum Data Set within 10-14 days of admission or annual review, and when there is a significant change in a resident's condition.
Notify all members of the interdisciplinary team at least one week in advance of the MDS due date for all new admissions, annual reviews, and significant changes in resident condition.
Monitor and follow-up with team members as needed to verify that all assigned sections of the MDS are completed, dated, and signed within designated time frames.
If a member of the interdisciplinary team is absent during the time frame for completion of a MDS, conduct necessary research and referral to confirm that all MDS sections and triggered RAPs are completed.
Review each MDS for accuracy, consistency, completeness, and signatures prior to submitting to the designated RN for final review and signature.
Verify that MDS documentation is placed in resident's medical record and that documentation is complete, including dates, signatures, and sections completed by all members of the interdisciplinary team. Complete, date, and sign MDS quarterly review sheets.
Verify the face validity of all Minimum Data Sets before electronic submission.
Participate in and oversee the timely electronic submission of all MDS.
Review the validation report and verify that appropriate action is taken.
Resident Assessment Protocols (RAP):
Review the Resident Assessment Protocols correlated with nursing issues and answer the questions as identified in the computer documentation system. Once all the questions have been answered, complete narrative summaries of the information, indicating the decision whether or not to include the identified problem on the Plan of Care.
Consult the RAP summary sheet and verify that all triggered RAPs and corresponding narrative summaries have been completed, dated, and signed by the appropriate disciplines.
For triggered RAPs included in the Care Plan, verify that any additional supportive documentation related to RAP issues is completed.
If a triggered RAP is not included in the Care Plan, verify that documentation in the RAP summary clearly indicates reasons for not proceeding.
Care Plans:
Schedule all interdisciplinary care plan meetings, and notify staff in advance which residents will be evaluated.
For Care Plan reviews, notify the resident's family in writing 30 days in advance of care plan meeting (except for care plans requiring immediate revision due to significant change or unforeseen circumstances.)
Identify and document nursing problems, goals, and approaches, and coordinate the development of an individual Plan of Care for each resident in cooperation with the physician, Medical Director, nursing staff, interdisciplinary team, and outside consultants (nursing, dietary, pharmacy, therapists, etc.) in accordance with corporate, state, and federal guidelines.
Correlate the information to update resident care plans quarterly and after each significant change. Verify that all updates are completed. Generate final copy, and verify that signatures from the physician, interdisciplinary team, and contributing resident or family members are obtained.
Make a copy of each resident's care plan accessible to CNAs.
Other Responsibilities:
Disseminate any new or updated materials involving the RAI process.
Create an opportunity for family participation in the care planning process.
Communicate with the Business Office Manager and Administrator on a regular basis regarding the case mix scores and how they impact reimbursement.
Coordinate the interdisciplinary assessment process for all residents of the facility. Verify that the Resident Assessment Instrument is individualized, complete, accurate, and timely for each resident.
Conduct and facilitate the Interdisciplinary Care Plan meetings. Educate peers on MDS, RAPs, and Care Plans.
Attend in-service education programs in order to meet facility educational requirements.
Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill & Evacuation Procedures and know how to use the information.
Maintain confidentiality of resident and facility records/information.
Protect residents from neglect, mistreatment, and abuse.
Protect the personal property of the residents of the facility.
Others as directed by the supervisor or administrator.

Base pay in range will be determined by applicant's skills and experience. Role is also eligible for team based bonus opportunities. Temporary associates are not benefits eligible but may participate in the company's 401(k) program.

RN What You ll Do:
Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team

Conducts orientation for new team members of the department to ensure that all employees are knowledgeable of the goals, policies, and procedures of the department.
Conduct interviews/observations on a continual basis of residents to alert Executive Director, family, and other appropriate individuals of any changes in the physical or emotional health of the resident.
Supports administration in referral development and maintaining a high census, act as care manager and resource person for the residents and their families.
Measures outcomes, set goals and leads the care team to achieve those outcome goals
Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough handwashing, use of disposable gloves where indicated and proper disposal of soiled materials. Also providing care that maintains each resident's skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents, turning, repositioning immobile residents and by applying moisturizers to fragile skin and other areas.
Performs various tasks assigned by the charge nurse, including checking vital signs, weighing residents, applying creams/ointments, and collecting specimens.
Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.
Report any occupational exposures to blood, body fluids, infectious materials, and or hazardous chemicals in accordance with facility policy.
Report work related injuries and illnesses immediately to your supervisor.
As a condition of employment, complete all assigned training and skills competency.
Participate in all life safety and emergency drills and trainings.
Fulfill responsibilities as assigned during implementation or activation of the facility's emergency plan.
Assists with evacuation of residents in an emergency situation
