Work as a member of a strong team to provide nursing care for the comfort and wellbeing of patients. Performs the functions and procedures as identified by the physician. Documents assessment, care delivery, patient condition, reaction and progress. Works as a liaison between providers and patients. Responsibilities include, but are not limited to, the following:
Clinical Support
Greets patient and escorts them from the waiting room to exam rooms; prepares patients for exam or treatment; takes vital signs.
Provides assistance to physician when requested.
Performs Spirometry and other breathing tests as ordered.
Prepares and maintains supplies and equipment for treatments, including sterilization.
Performs patient education when appropriate.
Completes appropriate documentation in patients medical record.
Facilitates prescriptions for patients as ordered by physician or nurse practitioner.
Performs allergy prick testing and/or intradermal testing as ordered by physician.
Administers allergy injections to shot patients as set forth on physician approved schedule.
Monitors patients medical status for possible adverse reaction following receipt of allergy injection.
Maintains emergency equipment, including oxygen, crash cart and supplies, suction, etc.
As necessary: appropriately assesses/evaluates patient problems over the phone, utilizing approved telephone triage protocol to determine the level of care the caller needs.
Demonstrates good clinical judgment, critical thinking skills, assessment skills, careful listening and good communication skills. Documents calls thoroughly and closes the loop between self, patient and provider in a timely manner.
Scribe for providers as requested/required by role within practice.
Follows standardized assisted documentation workflows as outlined in scribe policies.
Follows up to obtain referrals and insurance authorization as requested and needed.
Ability to organize requirements for biologics whether via Specialty Pharmacy or Buy and Bill.
This includes proper completion of all necessary paper work for ordering, billing, and reimbursement of biologics
Maintains clinical supply stock and medication samples.
Collaborates with patients, colleagues and interdisciplinary team members to achieve desired outcomes.
Other
Maintains patient confidentiality; complies with HIPAA and compliance guidelines established by the practice.
Maintains detailed knowledge of practice management and other computer software as it relates to job functions.
Attends CPR, OSHA, HIPAA, and Compliance training programs as required.
Attends all regular meetings.
Participates in professional development activities
Performs all other tasks and projects assigned by the Practice Manager.
Completes all assigned AP training (such as CPR, OSHA, HIPAA, Compliance, Information Security, others) within designated timeframes.
Complies with Allergy Partners and respective hub/department policies and reports incidents of policy violations to a Supervisor/Manager/Director, Department of Compliance & Privacy or via the AP EthicsPoint hotline.

Dispense medications without a prescription
Administer medications without a doctor's order
Provide any type of patient care unless provider is on site

Typical Physical Demands:
Position requires full range of body motion including manual and finger dexterity and eye-hand
coordination. Position involves standing and walking. Employee will occasionally be asked to lift and
carry items weighing up to 30 pounds. Normal visual acuity and hearing are required. Employee will
work under stressful conditions, and be exposed to bodily fluids on a regular basis.
Typical Working Condition:

Participates in rooming patients, vital signs, and patient treatment as directed by provider and within LVN scope of practice.
Performs, observes, and records symptoms and progress of the patient and reports findings to the provider. LVN does not make independent medical decisions.
Provides administration of medications and immunizations as directed by provider and within LVN scope of practice.
Coordinates with the Care Team (Primary Care Provider, Health Educator, Registered Dietitian, RN, Behavioral Health Provider, Director of Nursing, or Medical Director) to ensure appropriate services are arranged for the patient in order for the patient to meet his/her care goals.
Educates patients/families, creates a pre-visit checklist and participates or directs team huddles. Assists with patient/client follow up appointments within the recommended time frames.
Documents all care coordination activities in the medical record.
Tracks and compiles outstanding test and laboratory results required for examinations and visits for referrals and follow-up. Coordinates the notification process with support staff as recommended by health care providers.
Maintains and operates medical and laboratory equipment as necessary. May need to assist provider during treatment, examination, and/or testing of patients.
Collaborates with the Quality Improvement Department to comply with federal and state regulations and payer requirements. Assists with various report tracking in and all software programs as designated by the corporation.
Attends and participates in staff meetings and ongoing nursing review meetings.
Assists internal or external consumers with any problems they may have with excellent customer service.
Demonstrates professionalism when calling patients and provides quality customer service using AIDET Standards.
Ability to work with high volume of patients, internal/external customers, and deal with frequent changes, delay or unexpected events.

Document all new skin conditions on residents daily chart.
Follow Wellness Coordinator direction regarding residents skin care needs, i.e. prevention techniques such as continuous repositioning, use of heel protectors, etc.
Report and document any unusual incidents, such as falls, behaviors, etc. directly to the administrator on duty.
Call and inform the administrator on duty in case of emergency.
Prepare tables and set up for all meals of the day.
Escort residents to dining room.
Prepare and deliver tray services if resident is unable to leave room for meals. Assist residents that are not able to eat on their own.
Ensure daily laundry is maintained for each resident. Bed linens are done weekly.
Ensure that residents rooms are neat, clean, and orderly.

Ensure that residents are informed and may attend planned activities in the common activity areas.

Maintain all State mandated medication training records annually.
NOC Shift Specific:
Wellness check at beginning of shift - check in with exiting caregiver to receive update/current status on each resident.

Demonstrates appropriate clinical judgment in managing cardiac step-down /telemetry patients in the integration of clinical data with clinical assessment. Initiates a multidisciplinary, goal oriented care plan for each pt in collaboration with members of the health care team and documents education provided to patients and/or families using teaching methods appropriate to population served.
Delegates patient care tasks to ancillary personnel based on individual competency and scope of practice. Evaluates effectiveness of care performed. Completes legible, accurate and timely documentation of all nursing care provided, including evaluation of patient's response to medications, treatments & procedures.
Identifies and communicates opportunities to improve care, services, and unit operation. Tries new approaches to problem solving, is open to change and reaching new goals for the unit.

SUMMARY
Under the direct supervision of the Health Center Manager and in-direct clinical supervision of Nursing Administration, the Licensed Vocational Nurse (LVN) shall perform licensednursing duties under scope of practice, maintain and operate medical and laboratory equipment, assist in the treatment and care of patients, including performing physically demanding assignments.
It is the primary purpose of CHCCC to provide the highest quality of total care possible to the patient population it serves. Such a level of quality depends ultimately on the staff's desire and ability to work together, individually, and as a team. The employee is expected to be professional, punctual, maintain regular attendance, cooperative, motivated, and organized at all times.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Additional duties may be assigned with or without prior notice.
Participates in patient population management relating to clinical services while taking into account cultural diversities, sensitivities, and local recourses. Utilize Evidence Based Practice guidelines to assist with disease management.
Prioritizes the management of patient flow in the clinic by ensuring patients are roomed and cared for in a timely manner.
Participates in rooming patients, vital signs, and patient treatment as directed by provider and within LVN scope of practice.
Performs, observes, and records symptoms and progress of the patient and reports findings to the provider. LVN does not make independent medical decisions.
Provides administration of medications and immunizations as directed by provider and within LVN scope of practice.
Coordinates with the Care Team (Primary Care Provider, Health Educator, Registered Dietitian, RN, Behavioral Health Provider, Director of Nursing, or Medical Director) to ensure appropriate services are arranged for the patient in order for the patient to meet his/her care goals.
Educates patients/families, creates a pre-visit checklist and participates or directs team huddles. Assists with patient/client follow up appointments within the recommended time frames.
Documents all care coordination activities in the medical record.
Tracks and compiles outstanding test and laboratory results required for examinations and visits for referrals and follow-up. Coordinates the notification process with support staff as recommended by health care providers.
Maintains and operates medical and laboratory equipment as necessary. May need to assist provider during treatment, examination, and/or testing of patients.
Collaborates with the Quality Improvement Department to comply with federal and state regulations and payer requirements. Assists with various report tracking in and all software programs as designated by the corporation.
Attends and participates in staff meetings and ongoing nursing review meetings.
Assists internal or external consumers with any problems they may have with excellent customer service.
Demonstrates professionalism when calling patients and provides quality customer service using AIDET Standards.
Ability to work with high volume of patients, internal/external customers, and deal with frequent changes, delay or unexpected events.
Demonstrates adherence to and observes all safety policies and procedures, inclusive of infection control rules and regulations.
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served (infants, pediatrics, adolescents, adults or geriatrics).
Demonstrates knowledge of domestic violence, child and dependent abuse protocols.
Demonstrates cultural sensitivity and competence with patients.

SUPERVISORY RESPONSIBILITIES
