Licensed Practical Nurse (LPN) JOB SUMMARY Under the direction and supervision of the Clinic Practice Manager, administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone/telehealth. In Coordination with the Interdisciplinary Team and the RN Care Manager participates in planning and executing interventions in accordance with the participants plan of care. Performs the administration of prescribed medications, vaccinations, and treatments in accordance with nursing standards and documents appropriately. As designated by the Clinic Practice Manager and meeting the requirements within the scope of practice shall withdraw blood from participants as instructed. Maintains participants' medical record and fulfills PACE ORGANIZATION charting and reporting requirements. Observes, records, and reports participant's condition and reaction to drugs and treatments to physicians. Triages participants within scope of practice to determine priority of care. Educates participants and family/caregivers regarding medications and treatment instructions Prepares equipment and aids physician during treatment, examination, and testing of participants. Assists other staff in clinic in daily appointments and emergencies. Maintains timely flow of participants. Responds to and refers incoming telephone calls. Assists in the maintenance of the exam rooms for necessary supplies and materials. Ensures cleanliness. Ensure clinic supplies are on the reorder supply list as appropriate. Responsible for monitoring expiration dates of medications and clinic supplies monthly as assigned. Effectively communicates in interdisciplinary team meetings, clinic meetings or any other meetings as assigned. May Assist in the scheduling of specialist appointments and assisting in hospital admissions and calling report to unit, as assigned or requested Follows all BoldAge Policies and Procedures and OSHA safety guidelines. Maintains the confidentiality of all company procedures, results, and information about participants, clients or families. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions. Maintains safe working environment. Follows BoldAge Safety Policies and Procedures. Participates in and supports Quality Improvement initiatives. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications. Performs other duties as required or requested. EXPERIENCE AND EDUCATION: Graduate of an accredited school of nursing. One year working as an LPN strongly preferred. Current LPN/LVN license in good standing to practice in the required state. (NJ: New Jersey State Board of Nursing). BLS required (must have within 90 days of employment) IV Certification preferred. 1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring. PRE-EMPLOYMENT REQUIREMENTS: Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
