Administers treatments.
Reviews, transcribes, communicates and implements physician orders, obtaining RN signature as required by state regulations. Communicates with resident's physician. Assists with emergencies; administers cardiopulmonary resuscitation.
Documents care performed and observations of resident status in the clinical record promptly by utilizing standard clinical documentation guidelines as required by the company, and local, state and federal rules and regulations.
Participates in medication management to promote optimal safety and effectiveness for residents by
administering medications as ordered, including next scheduled dose following admission/readmission.
Notifying Charge Nurse of medications not available;
Preparing, administering and documenting medications as prescribed;
Observing and reporting resident responses to medication;
Identifying and promptly communicating adverse drug reactions;
Maintaining narcotic records accurately within scope of practice;
Completing medication interchange and insurance prior to authorization as assigned;
Ordering or arranging for ordering of pharmaceuticals;
Notifying physicians of automatic stop orders;
Completing recapitulated physician orders review, as assigned; and/or,
Cleaning, organizing and replenishing supplies on medication/treatment carts after each use and prior to shift change.
Completes clinical admission paperwork for new admissions.
Initiates and assists consultants while providing services. Reviews documentation from consultant visit and follows up on recommendation. Assist with follow up on results of qualitative and quantitative medical record audits. Communicates and implements pharmacy review/drug regime review recommendations, as assigned.

Responsibilities
Enhabit Hospice RNs work with patients, their families, and other health care professionals during the final stages of patients' lives. Our nurses work in private homes, residential care facilities, nursing centers, and other hospice care environments. They may also supervise licensed vocational/practical nurses and nursing aides. Our nurses must be capable of compassionate communication with patients and families, have keen observation skills, high ethical standards, and knowledge of when to alert doctors and others about changes in patients' conditions.
Qualifications
Must be a graduate of an approved school of nursing and licensed as a Registered Nurse in the state of employment
Must possess a valid state driver's license, automobile liability insurance, and have dependable transportation
Minimum of one year of clinical experience is strongly preferred
Previous home health or hospice experience is preferred

SCOPE OF RESPONSIBILITY:
The Licensed Practical Nurse, as an employee of Hospice Visions, LLC.. is responsible for meeting the established Standards of Nursing Practice while working under the supervision of the RN Case Manager. The LPN provides quality hospice care in a home setting using the nursing process to assist the RN Case Manager and other team members as indicated in assessing, planning, implementing, and evaluating the patient and families physical, emotional, psychosocial, and spiritual needs consistent with Hospice Visions policies and procedures.
EXPERIENCE/QUALIFICATIONS:
Graduated from an accredited school of nursing.
Holds a current license to practice as a licensed professional nurse in the State of Idaho.
Has valid Idaho Driver's License and availability of a car.
Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
Demonstrated ability to work well with a multidisciplinary team.
ESSENTIAL TASKS, DUTIES AND RESPONSIBILITIES:
Work on an as need basis for continuous care assignments.
Consult with the RN Case Manager and other team members and seeks guidance as necessary.
Promote and maintain good interpersonal and harmonious relationships with hospice team members, including volunteers and personnel of other departments.
Apply principles of asepsis and universal precautions.
Provide assigned patient care according to the Care Plan of the interdisciplinary team to patients and their families in need of twenty four (24) hour care, under the direct supervision of the RN Case Manager.
Provide a home environment conducive to safety and infection control.
Document care using the nursing process for team evaluation of the crisis situation.
Administer prescribed medications and treatments (except by intravenous push), based on knowledge of the drugs and treatments and their side effects for patient comfort and pain control.
Report pertinent information regarding assigned patients and their families to the RN Case Manager and to other team members as indicated.
Document information timely, at least every two hours and accurately on appropriate forms.
Provide appropriate support to families at time of patient death, and report to Bereavement Coordinator/Counselor any perceived problems.
Practice Universal Precautions according to OSHA regulations and Hospice Visions policy.
Attend twelve (12) hours of in-service annually, including OSHA requirements.
PHYSICAL REQUIREMENTS:
This position requires moving/lifting a bed-bound patient who is unable to assist in his/her own care in the privacy of his/her home. Weight may vary from 10 - 250 + pounds.
This position requires wearing appropriate attire and closed toe shoes in the clinical setting.
