Responsibilities
The Staff Nurse - Residential renders professional nursing care at the residential site pursuant to the objectives and policies of the facility. Must possess excellent assessment skills, self-confidence and be assertive in advocating for good patient care in working independently as a nurse. Must be highly organized and prioritize all nursing tasks and procedures in a shift. Education of patients and staff is a vital component of this position. Some on-call work is required.
This position is part-time and is performed at the residential site Saturdays and Sundays from 1pm-10pm. Your key responsibilities will include:
Performs nursing techniques for the comfort and well-being of the patient.
Supervises, administers, assists with, and delegates the distribution of medications.
Takes vital signs and monitors the medical progress of patients. Completes documentation and reporting.
Follows physician's orders and individual nursing care plans, including assisting patients with daily routines, catheter irrigating and assisting with respiratory treatments.
Provides emergency first-aid for patients and staff if needed. Administers TB tests and Hepatitis B vaccines for staff.
Teaches, monitors, administers and documents individual patient programs including ADL's (as outlined on the Needs and Services Plans) and therapeutic home programs in English.
Performs toilet and chair transfers with assistance for patients up to 150 pounds utilizing approved transfer techniques.
Provides for the safety of patients, visitors, staff and self when interacting with patients who may have behavioral outbursts.
May transport patients in company-owned or personal vehicle when company transportation vehicles are unavailable requiring transferring and driving abilities.
Qualifications
Successful candidates will have:
Passion for Care: A deep understanding of the requirements for providing the type of care and supervision needed, including the ability to communicate effectively with them.
Leadership Skills: The ability to recruit, employ, train, and evaluate qualified staff, and to make tough decisions when necessary.
Experience and Education
Educational Background: Current/clear California LVN licensure required. Blood Draw and IV certifications preferred. Valid driver's license required.

The Wellness Nurse is a licensed nurse responsible for coordinating resident care and medication management under the medical direction and supervision of the residents attending physicians in order to ensure the resident remains as independent as possible for as long as possible. The Wellness Nurse works on the scheduled days off of the Health Services Director to provide the community with consistent coverage from a licensed medical professional seven (7) days a week.
Responsibilities:
Communicate with community healthcare professionals, including physicians, discharge planners, nurses, and state agencies on the care needs of all residents in the community.
Coordinate incidental medical, dental, vision, hearing, and podiatry care for residents.
Perform nursing duties as directed and in accordance with the appropriate scope of the Nurse Practice Act, including administering medications and performing treatments as ordered by a physician.
Provide support to Medication Technicians to ensure competent delivery of medications to residents.
Assist with resident assessment tasks as directed by the Health Services Director.
Conduct staff interviews and observe residents on a continual basis to ensure community staff is informed of changes in residents physical and emotional health. Communicate all relevant resident information to physicians and other healthcare professionals. Alert Executive Director and responsible party to concerns as needed.
Complete thorough documentation in the resident chart on an ongoing basis according to company policy and state regulations.
Review and monitor staff use of the Shift Report daily and participate in the change of shift meeting to communicate pertinent information regarding resident care needs as needed.
Coordinate prescription orders with doctors offices and pharmacy delivery of medication. Oversee the central storage and delivery of medications.
Communicate with residents and responsible parties regarding the community s preferred pharmacy and incontinence supply programs.
Qualifications:
Must be a Licensed Vocational or Registered Nurse holding a valid California nursing license in good standing
Prefer one (1) year of experience working in a Residential Care Facility for the Elderly (RCFE)
Able to operate a personal computer and be proficient in MS Office programs (Word, Excel, etc.)
Able to operate standard office equipment, including fax machine, copier, telephone, etc.
Knowledge of and/or ability to learn the theory and practice of assisted living and dementia care

Our facility at 890 Hayes Street (890) is a male-identifying only program with approximately 105 beds that provides clients with residential treatment for Substance Use Disorder (SUD) for up to 90-days. Clients are supported by a team of SUD and behavioral health professionals who provide counseling and education for substance abuse disorder, life skills, as well as individual and group therapy. The Health & Wellness Registered/Licensed Vocational Nurse is an integral part of this interdisciplinary team that supports our clients in navigating evidence-based treatment plans and the healthcare system. Our goal is that our clients transition into the community with with the skills and resources to sustain their recovery goals and optimal well-being independently.
Leads the Health and Wellness team in collaboration with the Addiction Medicine team in the provision of treatment with Medication Assisted Therapy (MAT) for clients with Opiate Use Disorder (OUD) and other Substance Use Disorders (SUD). Provides support of treatment adherence via client education that assists them in developing strategies for taking their medication regularly and effectively. Coordinates care for the client with the residential behavioral health team, the addiction medicine team, primary care, and other internal and external healthcare resources for the client. Utilizes the Nursing Process, Harm Reduction, and Trauma Informed Care modalities in all actions taken to advocate for and support the client in their planning and goals.
Key Responsibilities
Works collaboratively with all Health and Wellness staff.
Monitors patient medication use and helps to mitigate risk to patient by flagging high dose medications and potential medication interactions to the Director of Addiction Medicine.
Supports Medication Assisted Treatment through counseling, education, and administration of injectable medications under
the supervision of the addiction provider and standing orders.
Provides appropriate client care education and counseling.
Provides complex care and care coordination for all clients accessing healthcare services internal and external to HealthRIGHT 360.
Works closely with stat collecting and reporting contractual data as necessary.
Functions as a nurse within the scope of California BRN licensure and credentialing and privileging of the agency.
Education and Knowledge, Skills and Abilities
Male-identifying, individuals with lived experience, and/or fluency in Spanish, Cantonese, & Tagalog are encouraged to apply.
Active California RN/LVN license.
BSN or equivalent degree.
BLS Certification or must obtain within 30 days of hire.
1-2 years or equivalent experience, interest in working with homeless populations and in treating Opiate Use Disorder and other Substance Use Disorders.
Culturally competent and able to work with a diverse population.
Strong proficiency with Microsoft Office applications -- Word, Outlook, Excel, Teams, and other internet applications.
Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency.
