Position Overview

As an LPN Case Manager, you will conduct in-home assessments, monitor participant progress, update care plans, and educate caregivers. This role is ideal for LPNs who value autonomy, one-on-one care, and meaningful impact within the community.
Key Responsibilities
Conduct monthly in-home visits to assess medical status, home environment, and caregiver competency.
Provide caregiver education, coaching, and skills training as needed.
Complete semi-annual visits, including care plan updates, goal reviews, medication reconciliation, and communication tool audits.
Identify and escalate concerns related to safety, psychosocial needs, or changes in health status.
Complete required documentation in compliance with regulatory and organizational standards.
Communicate professionally with PCPs, caregivers, and interdisciplinary team members.
Demonstrate cultural humility, compassion, and inclusivity with all clients and families served.
Support team members through mentorship and collaboration.
Perform additional duties as assigned.
Qualifications
Active Massachusetts LPN license in good standing.
2+ years of nursing experience preferred (new grads encouraged to apply!).
Experience completing clinical assessments required.
Strong clinical judgment, communication, and care coordination skills.
Intermediate-advanced proficiency with Microsoft Office Suite.
Willingness to use telehealth tools (video, chat, etc.) when appropriate.
Experience with EMRs and care management systems.
Prior care management experience preferred.
Valid, unrestricted driver's license and ability to travel locally.
Working Conditions
Community-based role with in-home visits required.
Some remote work for documentation may be possible.
