The Housing Case Management Specialist is responsible for the care coordination of those living with HIV along with the Matthew 25 team to ensure that patients that test positive for HIV/AIDS are connected to housing resources, with vulnerable populations being the most urgent. The Housing Case Management Specialist will accomplish this by utilizing Matthew 25's housing programs, HOPWA and TBRA. These programs assist clients who are at-risk for homelessness and those who are homeless by providing case management services.

Position Overview
* Carry a maximum caseload of 8-10 young adults
* Meet with the young adult a minimum of one time per week, increasing and decreasing as directed by the Clinical Supervisor on the basis of clinical need.
* Provide on call crisis support to the client 24 hours a day 7 days a week (alternating on-call duties).
* Provide accurate and complete information to Clinical Supervisor in a timely manner in the event of a crisis situation.
* Implement suggestions of interventions provided by supervisor and/or consultant in a timely manner to be reflected in the next treatment plan cycle.
* Participate weekly in all supervision exercises to ensure the appropriate conceptualization and implementation of treatment.
* Provide all therapeutic services in accordance with Youth Villages' Mission and Values and LifeSet Model Principles.
* Conduct on-going assessment of the young adult's needs from a strength focused, solution-based perspective.

The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Essential Duties & Responsibilities:
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community, and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Make referrals to services and resources on behalf of the families and children.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports, and include their input in the person-centered planning process.
Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Monitor and document the status of the consumer's progress.
Participate in Quality Assurance/Quality Improvement activities to include audits, corrections and designated improvement plans and projects.
Qualifications
Education & Experience:
Minimum of a Bachelor's Degree in human services or related field is required.
Two years' experience working with youth or families.
Current Family Centered Treatment Certification is preferred.

LSW or eligibility for provisional licensure preferred.

QMHP or ability to register as a QMHP.
Mississippi

KEY RESPONSIBILITIES
Assessment of Health Status
Performs and documents in the medical record a complete history and physical examination for acute and complex chronically ill patients
Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
The APC may serve as the first responder for both nursing and staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions.
Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, fluid, and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures.
Coordinates daily clinic schedule with the attending physician team, presents patient report, collaborates in plan of care, and follows assigned patient population.

Identifies expected outcomes from diagnosis and formulates and documents a plan of care to address complex acute and chronic health care needs.

Prescribes diagnostic strategies and therapeutic interventions both pharmacologic, non-pharmacologic, and surgical, needed to achieve expected outcomes.
Manages further diagnostic tests through ordering, interpretation, performance, and supervision.
May perform advanced procedures consistent with privileges and competency validation, including minor suturing, complex wound management, line and tube placements and removals

Maintains ongoing communication and collaboration with interdisciplinary health care team.

Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and/or clinic note where applicable. Key Responsibilities:
Aids Psychologist in carrying out various assessments, behavioral observations, gather collateral, and historical information with efforts to create a comprehensive clinical profile to make treatment recommendations. Contribute to the multidisciplinary treatment team regarding youths diagnose, risks and care planning.
Keep clear written records, draft reports, letters and summaries of assessments and observations of treatments.

Requirements:
Education -Must be met programmatically, i.e., as a matriculated in-residence student in a formal graduate training program organized to provide graduate education and training in psychology, and whose stated purpose and design is to educate and train Psychologists, Senior Psychological Examiners, Psychological Examiners, and/or Certified Psychological Assistants work, psychology, rehabilitation, special education, or in a related human services field, is required. A candidate that has a masters degree in an appropriate field, the experience requirement is preferred, but not required.
Experience -Working with children with serious emotional disturbances or substance abuse problems. Experience with group work techniques is a plus.
Skills -Participates in both basic and applied research endeavors and contributes to the basic fund of psychological knowledge; Must be able to manage for results, work collaboratively, serve as a change agent, possess strong organizational and communication skills, and make quality, timely decisions. Responsibilities include, but are not limited to, the following:
* Provide therapeutic counseling, advocacy and case management for justice involved individuals within the Department of Corrections Division of Addiction Services
* Assist in the coordination of addiction services with justice involved individuals located in Correctional Facilities or Probation and Parole.
* Responsible for screening, assessment and treatment of clients with a validated substance use disorder. This includes developing a comprehensive treatment plan, participating in interdisciplinary teams, facilitating treatment groups while maintaining clear communication with other correctional staff not directly involved with the offender's use disorder treatment.
* Serve as a resource person for staff, service providers, courts, and referral agencies.
* Maintain confidential case records.
* Be in compliance with all substance use disorder treatment program licensure standards.
* Comply with the Substance Use Disorder Counselor Code of Ethics and all correctional policies and procedures.
* Provide administrative support for substance use disorder treatment operations.
The ideal candidate should possess the ability to work in a close setting and within all State and Federally mandated deadlines. They should possess strong verbal and written communication skills and have displayed the ability to complete work related activities in a professional manner. The ideal candidate should be able to multitask with a proven history of sound decision making in high stress situations. This candidate should display the ability to think critically and analyze data to forecast positive outcomes. This candidate should possess the ability to adapt to the ever-changing needs of the Department and Division.
Minimum Requirements
EDUCATION: A master's degree in social work, sociology, psychology, marriage and family therapy or a related field.
