Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients. This is an on call (as needed) position that does not include benefits.Essential Duties: Provides milieu management, guidance, safety, stability, and direction to clients in early recovery from a trauma-informed lens. Maintains acceptable attendance and punctuality and is consistently at work on time. Interacts with client population to provide guidance, safety, stability, and direction including de-escalating potentially dangerous situations. Performs accurate and timely documentation at all times, including but not limited to admissions, shift notes, Per Diem notes, discharges, inventories and screening of clients' personal belongings, individual and group sessions, Medication Occurrences (MOR), and Adverse Incident Reports (AIR). Must be able to administer blood pressure assessments, utilizing blood pressure cuffs, when Nurse is not available. Provides for the health, safety, and welfare of the residents with disabilities in their homes and in the community. * Attends to resident health and safety needs according to all assessment and care plans, including medication administration, personal hygiene, and home and community safety. * Performs cleaning, cooking, and other site maintenance as assigned. * Provides transportation as required using company vehicles. * Ensures the ongoing facilitiation of person-centered principals in the home. * Deals effectively with crisis situations. * Supports individuals new to permanent supportive housing navigate systems and ensure long-term success. 3. Implements, monitors, and reports on residents' plans and protocols as stated in their Person Centered Support Plan. * Completes all documentation and tracking timely and accurately. * Communicates effectively with residents, guardians and family members, and all other members of residents' treatment team. 4. Monitors and reports on data to ensure compliance with all applicable guidelines and regulations, including Imagine! Policies and Procedures. * Completes all required training in the appropriate timeframe. * Reports all behavioral incidents, medical emergencies, and/or significant changes in medical condition to Residential Site Supervisor, and completes all related paperwork in a timely manner. * Completes and documents home visits in a timely manner for residential sites * Maintains respectful and professional relationships with residents, staff, guardians and family members, and all other members of treatment teams. * Confronts conflict productively, as described in the residential training. * Utilizes effective time management skills. Other Duties/Responsibilities * Strictly complies with household budget protocols and spending limits, including guidelines for resident personal needs funds. * Maintains the residential site, vehicles, and resident personal belongings in good repair, being mindful of safety and cleanliness. Job Qualifications Knowledge, Skill, and Ability: * General knowledge of the developmental disabilities system in Colorado. * Demonstrated skill in working with adults with developmental disabilities, preferably in a residential setting. * Effective written and verbal communication skills with individuals and groups at all professional levels. * Ability to productively confront conflict. * Demonstrated organizational and time management skills. * Ability to work effectively with various technology including word processing software, internet-based databases, assistive technology, etc. * Possession of a valid driver's license and ability to meet Imagine! driving requirements. Develop sophisticated and highly individualized Treatment, Rehabilitation, and Recovery Plans (ITRRPs) that reflect the complexity and self-pay status of the client, focusing on root causes and sustainable lifestyle changes. Individual and Group Therapy Excellence: Serve as the Primary Counselor for an assigned caseload, delivering high-frequency, evidence-based individual counseling (e.g., CBT, Motivational Interviewing, trauma-informed approaches).
