The FACT program is a statewide system that provides an unlocked, supervised living environment with 24-hour staffing. FACT's model of care is centralized services with a slow transition to community-based services. Services needed to support community reintegration include, but are not limited to the following:
Under the direction of the Fact Program Director, the Residential Counselor provides community-based services and monitoring for FACT clients to include judicially committed individuals and mentally disordered offenders.

Provides rehab and intensive home-based services that is both helpful to the child and family and consistent with the philosophy of the program.
Responsibilities may include but are not limited to in school assistance; in home support; mentoring; skills development; crisis intervention; trouble shooting/ problem solving; parental guidance, support and education when requested by a parent.
Provides case management and intensive case coordination, which may include facilitating communication and cooperation among partnering agencies, schools, and community partners.
Empowers child and families in meeting their goals.
May facilitate Child and Family Team meetings.
Ability to effectively communicate, collaborate and partner with community partners and resources.
Safely transports clients and other passengers to and from appointments and activities using own personal vehicle.
Provides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation.

Provides after hours, “on-call” coverage to program clients which may include in-person response/assistance/intervention.

Job Summary and Responsibilities A Graduate Medical Education (GME) counselor is a mental health and wellness professional who can help trainees with a variety of issues (listed below). Responsible for the wellness and academic success of physicians in training (residents and fellows). The Graduate Medical Education (GME) Counselor provides mental health, emotional wellness, and academic and professional development support services to the St. Joseph's Medical Center GME programs, residents, and fellows. Conducts individual consultation, assessments, and support for physicians in training who require support with well-being, professional or academic challenges, and those seeking to improve performance. Provides linkage and referrals to outside resources as required. Ensures compliance with the Accreditation Council for Graduate Medical Education (ACGME) policies and requirements. This position also requires education delivery to residents and faculty, where appropriate. Experience in higher education, specifically counseling and academic coaching, is required. This position does not support medical students. It supports physicians in training (residents and fellows).
