Description

This is accomplished according to an established plan of care under the direction of the Director of Operations and Radiology Nurse Coordinator in charge of Nursing who is a licensed Registered Nurse. Primary Duties and Responsibilities
Provides pre-procedure telephone instruction and education to patients relating to their procedure.
Performs pre-procedure assessment appropriate documentation of assessment pre-procedure preparation of the patient which includes intravenous access review of allergies and laboratory results application of monitoring equipment and site preparation as indicated.
Monitoring is performed intra-procedure according to Radiology/Nursing policy to assess vital signs pallor responsiveness level of comfort emotional status; Neuro status and circulation are assessed according to Radiology policy and as indicated by the procedure; provides patient care utilizing the nursing process; information related to changes in patient's condition is relayed accurately and efficiently to the physician. Appropriate care and treatment are implemented; acts as a patient advocate; follows universal precautions.
Administers and titrating medications appropriately and in the correct increment dosages knowing the pharmacological effects and side effects of the medications being administered. Assesses and documents the patient's response to medications administered. Institutes appropriate treatment to medication induced changes in the patient's condition. Administers emergency drugs and utilizes emergency equipment knowledgeably and appropriately.
Post-procedure care is arranged prior to procedure date unless the case is an add-on. Performs and documents an appropriate and complete post-procedure assessment. Gives a complete and concise report to the department receiving the patient post procedure. Discharge instructions and patient/family education are provided according to Radiology policy.
Performs daily equipment drug and quality assurance checks as required by Radiology policy. Documentation is performed accordingly. Reports equipment failures and assures repairs are performed; Arranges for replacement of expired or used supplies.
Participates in the development implementation and evaluation of Performance Improvement and Research activities.
Participates in quality improvement initiatives. Recognizes cost reduction opportunities.
Supports unit and organization's goals.

SPECIALIZED SERVICES PROGRAM - CCS
The role of the Youth CCS Mental Health Professional (MHP)/Supervisor is focused on providing direct supervision and clinical support to the Youth CCS Care Coordinators. The MHP/Supervisor also provides clinical guidance to high-risk youth and young adults enrolled in Youth CCS. The MHP/Supervisor responsibilities are two equally essential roles. The supervisor responsibilities include direct supervision of the Youth CCS staff to ensure comprehensive services are being provided to meet the individualized needs of each person served by Youth CCS. The clinical work includes providing direct clinical consultation to Care Coordinators and supporting high-risk enrolled youth who may need additional clinical care.
Essential Duties and Responsibilities
Provide supervision to the Youth CCS team to ensure services are being coordinated to meet the identified needs of the individuals being served by the Youth CCS program. This includes being a champion of the Youth CCS process while providing leadership, and role modeling a strength-based environment.
Interview, hire, evaluate, manage, and train assigned Youth CCS staff in accordance with Milwaukee County Youth Behavioral Health programs and WCS Policies and Procedures.
Monitor and approve all documentation to ensure accuracy and timely completion (i.e. consent forms, provider notes, provider referrals, court letters Community Treatment Plans, Crisis Plans, Incident Reports, etc.)
Provide weekly, directive clinical consultation and coaching with each Care Coordinator for a minimum of one hour. Provide ongoing coaching and support regarding areas of strength of Care Coordinators, areas of need, and potential strategies for improvement through the consistent use of shadowing and supervision.
Facilitate clinical consultation for Youth CCS staff in accordance with DHS 36 and monitor staff's participation in/documentation of required consultation time.
Oversee CCS discharges in collaboration with Care Coordinators and Milwaukee County Youth CCS Administration.
Ensure accountability for timely annual functional screen evaluations which determine ongoing eligibility for youth enrolled in Youth CCS.
Provide Care Coordinators with ongoing assistance in day-to-day service delivery, as well as crisis situations.
Establish and maintain strong communication and collaboration with all system partners (Milwaukee County Behavioral Health Services, Division of Milwaukee Child Protective Services, Children & Youth Family Services, Milwaukee County Crisis Services, etc.)
Assist with facilitation of weekly agency Care Coordinator team meetings.
Responsible for accurate documentation and maintaining case files as required by agency policy, government regulations, and applicable local, state, and federal codes. This includes provider notes, Community Treatment Plans, Crisis Plans, and SAFE-Ts in AvatarNX.
Promote a person-centered, culturally intelligent, and recovery focused philosophy of serving people with mental health and co-occurring needs.
Ensure the maintenance of a 24-hour on-call system to assure access to Youth CCS Care Coordinator.
Provide direct clinical assessment and support to individuals who may be in crisis, in collaboration with the Care Coordinator. Services may be provided in the person's home or in the community.
Attend safety planning meetings, team meetings, and/or other face to face visits for high-risk youth, as requested by Care Coordinators, to provide clinical assessment and support.
Ensure fidelity to treatment protocols and evidence-based practices.
Enter timely clinical documentation directly into AvatarNX.
Keep Milwaukee County Youth CCS Administrators informed of emergency or critical incidents in collaboration with Program Manager and Assistant Program Director.
Provide coverage for Care Coordination staff when needed.
Engage in quality assurance and quality improvement activities as identified by WCS and Milwaukee County Behavioral Health Services.
Attend all required trainings and in-services set forth by both WCS and Milwaukee County.

Job Summary and Qualifications
The Registered Nurse coordinates and delivers high quality, patient-centered care per the nature and specific requirements of the department, and consistent with the scope and standards of practice for the relevant state and specialty. What you will do in this role:
Assesses patient condition during admission and during each shift as scheduled and/or as needed, identifying and reporting any changes in patient status.
Performs procedures, monitoring, or other functions as ordered by the medical provider(s). Documents the administration of care in the patient medical record in a timely and thorough manner.
Performs exceptional care by responding promptly to patient requests. Strives to anticipate patient needs and resolve them proactively.
Manages the quality of care by collaborating with a multidisciplinary team to coordinate, evaluate, customize, and deliver a holistic patient treatment plan that promotes and supports recovery.
Manages the continuation of care by serving as the interface between healthcare providers and community support systems who coordinate ongoing care needs for specialized patient groups.

Basic Cardiac Life Support must be obtained within 30 days of employment start date

Mental Health Counselor Job Responsibilities:
Engages in activities integral to the continued development and success at our sites of care across Inova Health System.
Provides assessment, planning, treatment, intervention, and evaluation to patients with psychiatric disorders.
