Essential Duties and Responsibilities:
Provide non-medical, short-term, solution-focused, walk-around counseling directly to service and family members. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
Deliver training, and health and wellness presentations, participate in health fairs and other bases/installation activities as requested/ directed.
Enter counselor activity data on a daily basis through smartphone, web, or fax to assure that reporting is accurate from assigned installation while maintaining client/service member confidentiality.
Create a presence on the installation in which the service and family members feel comfortable approaching the counselor and recognize the program to be confidential.
Establish and maintain working relationships with community resources and provide appropriate linkages for service members and their families.
Develop an excellent working relationship with the installation POC.
Provide MFLC response to critical incidents, surge and on-demand events, and special requests as directed by the POC and approved by the OSD program manager. Counselor will flex their schedule to respond to urgent requests as needed.
Manage duty to warn and restrict reporting situations according to DoD protocol and staff the cases with Regional Supervisor/Regional Director.
Communicate with Regional Supervisors and participate in regular individual and group supervision, sharing information regarding trends and issues on the installations to which they are assigned.
Participate in regular in-services/training, Quality Improvement committees, or other contract activities as assigned/appropriate.
Education and/or Work Experience Requirements:
U.S. citizen, and speak fluent English
Successfully pass government suitability requirements
Master's degree from an accredited graduate program in a mental health-related field such as social work, psychology, marriage/family therapy, or counseling; a valid unrestricted counseling license or certification, REQUIRED
Must be clinically licensed or certified and properly credentialed
Minimum 1 - 5 years post-licensure clinical experience
Provisionally licensed (Tier 1), or licensed (independent licensure Tiers 2, 3, and 4)
Understanding of the military lifestyle, sensitivity, and empathy for service members and their families
Working knowledge of military, state, federal, and local laws and resources
Work effectively with individuals and families from diverse racial, ethnic, and socioeconomic backgrounds
Must be current on all immunizations recommended for adults by the Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control and Prevention or meet any Military Service or installation-specific immunization requirements.
Ability to intervene in crisis situations, using sound professional judgment, ethical practice, and common sense
Work independently to develop, implement, and evaluate safety and intervention plans to meet individual and family needs
Agree to operate within established guidelines of the military services family support and quality of life programs
Work cooperatively with military and civilian medical, social service, law enforcement, and legal personnel on behalf of service members and their families.
Demonstrate sound professional judgment and the highest ethical standards in executing their responsibilities

Responsibilities:
Initiates face to face contact with assigned families within appropriate time frame.
Leads treatment team meeting and develops a treatment plan for each family within the required time frame.
Conducts a minimum of 2 hours in-home, face to face contact per week with each family to assess family functioning, provide service intervention, and gather information. Adjusts treatment plan as needed to meet the client's specific needs.
Utilizes historic Home Builders Training methodology for service delivery.
Provides face to face telephone contact with schools, therapists, and other providers to monitor child and family progress.
Provide support for children and families by accompanying them to appointments; coordinating transportation for family visits/activities.
Assists the family with locating and utilizing community resources, services, and activities.
Supervises family visitations, provides education, parenting support, and intervention as needed.
Attends ISPs, IEPs, court hearings, and other appointments along with family members to assure coordination of services.
Provides weekly consultation to DHR and immediate responses when health or safety issues pose a threat to children.
Provides progress reports to the referring DHR worker on a monthly basis and prior to any Family Court Hearing.

This is responsible, professional work as a direct services provider and an information and referral agent for targeted populations that are considered chronically mentally ill, being released from incarceration, and need assistance transitioning into the community.
DESCRIPTION OF DUTIES
Identify eligible individuals.
Visit referred persons and determine needs using the SUN Assessment by conversing with the individual.
Complete a SUN Assessment Form on each client utilizing information gained from the client or a family member.
Determine resources that would meet the needs identified in the SUN Assessment by contacting provider sources- public and private, both in and out of the catchment area.
Prepare a plan for the provision of community mental health services to the individual involved and review such plan not less that once every three months.
Assist in obtaining and coordinating social and maintenance services for the individual, including services relating to daily living activities, transportation services, habilitation and rehabilitation services, prevocational and vocational services, and housing services.
Assist the individual in obtaining income support services, including housing assistance, food stamps, and supplemental security income benefits.
Refer the individual for such other services as may be appropriate.
Transport clients to services, as well as conduct outreach visits.
Productivity requirement- 70% of time dedicated todirect services.
Participate in training activities as required by the Carastar Health to increase and enhance professional and therapeutic skills.
Provide mental health consultation to other agencies and providers assisting Carastar's consumers.
Complete reports required for client records as well as all forms and paperwork required by the Carastar.
Work closely with the clinical treatment team and the referral source to ensure that all needs are met, and that case management progress information is communicated.
Attend mental health court and serve as liaison between court and Carastar.
Provide testimony in court related to the case management services provided to the individual.
Screen individuals in the jail as needed to ensure a smooth transition upon release.
Perform other related duties as assigned by supervisor.
REQUIREMENTS

This job requires the ability to identify issues related to professional and facility provider claims data including determining appropriateness of code submission, analysis of the claim rejection and the proper action to complete the retrospective claim review with the goal of proper and timely payment to provider and member satisfaction. The incumbent is responsible for the implementation of effective Payment Integrity strategies on a pre-payment and retrospective claims review basis. It is also used to identify issues which can be used for education of network providers, identification and resolution of quality issues and inappropriate claim submission. The incumbent is expected to utilize specialized skills and knowledge to achieve successful and measurable outcomes. Will monitor and analyze the delivery of health care services in accordance with claims submitted, and analyzes qualitative and quantitative data in developing strategies to improve provider performance and member satisfaction. Will be expected to identify potential discrepancies in provider billing practices and intervene for resolution and education or if necessary involve Special Investigation Unit or the Utilization Management area.

* Manage duty to warn and restrict reporting situations according to DoD protocol and staff the cases with Regional Supervisor/Regional Director.
* Communicate with Regional Supervisors and participate in regular individual and group supervision, sharing information regarding trends and issues on the installations to which they are assigned.
* Participate in regular in-services/training, Quality Improvement committees, or other contract activities as assigned/appropriate.
