Program Summary In the Adult Partial Hospitalization Treatment and Intensive Outpatient Treatment programs, we focus on creating a therapeutic environment where people can learn strategies for coping and improve their functioning. Each client's treatment plan is centered on group therapy which allows for sharing of experiences while gaining support and validation. When people need intensive, comprehensive treatment for psychiatric and chemical dependency issues but do not need inpatient or residential hospital care, Natchaug offers intensive, structured clinical treatment in programs that may include up to six hours of treatment a day, five days a week. These intensive treatment programs can help prevent the need for hospitalization. They can also help people make the transition to home after inpatient or residential treatment. Natchaug brings this clinical care closer to people's homes by offering programs throughout Eastern Connecticut. Job Summary Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in the Adult outpatient department. Clinical services include individual, group and family therapy. Key responsibilities of Behavioral Health Clinician: Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate. Provides direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end -of-life care, referral and discharge planning. Refers patient to appropriate levels of care and available resources that promote the individualized care plan. Maintains timely patient-centered documentation that reflect the patient and client systems' pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies. Schedule: Monday - Friday; 8:00 AM to 4:30 PM.
