GENERAL DESCRIPTION
Performs complex (journey-level) social services work for the Texas Physician Health Program (TXPHP). Supports the rehabilitation of program participants and monitors program participants adherence to agreements, policies, and rules. Qualifications: PRINCIPAL JOB DUTIES AND RESPONSIBILITIES
90% Provides guidance and support to facilitate participants ongoing recovery and successful completion of monitoring agreements. Collaborates with evaluators, treatment providers, and outside monitors to coordinate treatment and gather information about the health status and impairment status of program participants. Tracks and documents changes in health conditions and outcomes over time. Assists with management of participants in crisis, involving TXPHP medical director and participants care providers, where indicated. Timely responds to positive drug tests, monitoring reports, or other health crises. Provides support to participants during transitions through evaluation, treatment, return to work, monitoring, and retirement. Conducts calls and conferences with participants as needed to gather information and provide supportive follow-up. Maintains complete and accurate records of all interactions involving TXPHP participants. Prepares timely and accurate case reviews for TXPHP Governing Board.
10% Performs data entry, analysis, retrieval, reports, and searches. Performs tasks related to the completeness and accuracy of program data. Manages the ongoing implementation of the participant database program. Coordinates the updating of features, reports, and forms available in the database. EXPERIENCE AND EDUCATION

EXPERIENCE AND EDUCATION
· Master's degree in mental health field is required. Must be a Licensed Master Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW) with a current, active, and unrestricted Texas license in good standing.
· At least five years' experience with direct clinical services, with broad exposure to chemical dependency, psychiatric, and medical diagnoses.

These assessments are used to contribute to the resident care plan.
Contributing as an integral member of the interdisciplinary Resident Care Plan Conference. This also involves stating the social/emotional needs related to the resident's illness/disability, adjustment to placement, supportive network and response to treatment. The social worker makes specific recommendations to assist in the resident's overall care and genuine well-being.
Writing progress notes as least quarterly, earlier when there is a notable change. Each note should document progress made towards "social service" oriented goals, provide significant information about the individual and serve to communicate with the entire interdisciplinary team.
Initiating and/or participating in the written discharge plan which states the resident's specific need to be in the facility and if the resident is expected to be able to function in a more independent setting. The discharge plan should include consultation with otherdisciplines, the family and of course, the resident (whenever possible).

Attending Resident Council meetings as a liaison to listen to resident concerns and clarify issues and/or facilitate changes (when invited by the Resident Council governing body). Supports the rehabilitation of program participants and monitors program participants' adherence to agreements, policies, and rules.
