POSITION OVERVIEW:
The Social Worker is responsible for supplying necessary clinical intervention within the realm of the program. Provide cultural competent and sensitive services. As part of a therapeutic team within the Agency, work closely with the divisions' medical, psychiatric and psychology staff, to ensure the appropriate treatment interventions for the client.
KEY ESSENTIAL FUNCTIONS:
Client Engagement
• Carries a caseload of residents, and provides services to those residents within the scope of program regulations and agency mission.
• Completes client assessments to include, intake assessments, overall client assessments, risk assessments
• Evaluates and assesses clients' functioning, and supports clients in meeting individual goals.
• Practices person-centered and collaborative service planning with clients, also ensuring service plans are completed and signed by residents according to regulatory guidelines and standards.
• Interprets and explains agency policies and procedures to residents.
• Develop safety plans
• Provide individual and family counseling.
• Makes referrals and coordinates services for clients, including but not limited to, medical, mental health, substance abuse prevention and treatment
• Advocates on behalf of residents with other services providers.

The social worker works under the direction of the principal to assist and support the implementation of student support initiatives. They aim to provide a school environment conducive to improving student achievement.
Responsibilities include, but are not limited to:
Supervise the consistent and fair administration and tracking of student data and create positive school culture;
Contribute toward a safe environment for learning, including ensuring that student conduct is maintained and discipline is enforced fairly and consistently, and updating and implementing the required annual school safety report;
Coach teachers on classroom and behavior management techniques;
Identify and intervene with high-need students;
Assess and differentiates between students with mental health, emotional and behavioral needs and assist in service interventions;
Provide clinical individual and/or group counseling;
Provide comprehensive case management and advocacy;
Support school administration in conducting risk assessment, crisis intervention and mediation;
Participate in Student Assistance Team to mMonitor and evaluate effectiveness of student intervention and action plans;
Foster positive partnerships between the school, families and child serving organizations;
Identify new opportunities to partner with youth service agencies, such as mental health providers, foster care, social services, juvenile justice, and drug prevention and treatment programs;
Collaborate with administrators, teachers and support staff to:
Implement and monitor individual student interventions and action plans
Develop school-wide behavioral management tools
Support attendance improvement initiatives
Advocate for child safety and wellbeing
Perform statistical, narrative and record-keeping duties, including data entry and maintaining case logs
At the direction of school administration, ensure compliance with mandated reporting, confidentiality, social work code of ethics and other school-wide policies and regulation
Strategic management and development of the school, including creating organizational goals and objectives, overseeing annual review, and-in collaboration with Capital Preparatory Schools-envisioning;
ongoing strategies to maximize the effectiveness of the school model;
Implement and follow policies and procedures for student dress code, discipline, and other policies; related to maintaining a positive school culture of learning and achievement;
Serve as a liaison to parents and facilitate parent education and involvement, including reinforcing the need for parents to support the school's policies and practices for student discipline, dress code, homework, and events;
Undertake administrative and instructional duties as the principal may assign.
Skills and Qualifications:
Masters in Social Work; Certification in Social Work
2-3 years of experience in a school setting preferred
Experience and demonstrated ability in working with children in a teaching, counseling or other applicable capacity (examples include, but are not limited to involvement with community youth organizations, supervisory roles in national youth organizations, and mentorship programs);
Demonstrated successful teaching experience;
Exhibited leadership in working with professional staff, students, and the community;
Agreement with the academic goals and philosophy of the charter school;
Commitment to accountability and student discipline;
Demonstrated success in encouraging parental involvement.
Demonstrated excellence in verbal and written communication.
Compensation:
This is a full-year, full-time, exempt position with a competitive salary range of $79,200 to $94,656, commensurate with experience. Capital Preparatory Schools also offers a comprehensive benefits package.

Qualifications:
MSW and LCSW required.
Clinical experience in a supervised setting necessary; prefer experience in team and/or interdisciplinary approach.
Post graduate experience preferred.
Experience treating clients and families struggling with Substance Use Disorder, Alcohol Use Disorder also preferred.
Additional candidates with experience working with the developmentally disabled population also encouraged to apply.
Schedule: Full-time, Flexible hours: 8am-4pm, 8:30am-4:30pm or 9am-5pm.

Job Duties May include:
Perform in depth client assessments including medical, psychosocial, environmental, and family assessment
Complete Fall Risk Assessment, Mini Mental Status Examination and Depression Screening for each client
Work with client and family to develop plan of care to ensure client safety and all other needs are addressed
Work with client, family, and professional staff to implement plan of care
Collaborate with hospital, nursing home and AL/memory care staff, physicians and other professionals in geriatric service provider world
Perform regular home visits to monitor client status, home environment or senior community
Give ongoing emotional support and counseling to the client and their family as needed
Provide problem solving support to clients and fellow care managers as needed
Provide crisis intervention and display critical thinking/problem solving skills
Give back-up support to the GCM team as needed
Carefully track and document time that is spent on each client to facilitate accurate billing

Social worker with 5+ years of experience working in Geriatrics
Active/valid NJ LSW or LCSW license
Ability to work with minimal supervision
Valid Driver's License and dependable personal vehicle
