Responsibilities
The Care Manager II is responsible for managing and coordinating care, services, and social determinants of health for Members with acute, chronic, medically complex and/or behavioral health conditions and other health needs. CPS social workers must be able to identify immediate threats made to a child and provide arrangements that comply with state and federal laws and agency procedures. These duties may include, but are not limited to:
Receive referrals of clients with a variety of problems which require social work assessment and/or intervention and make initial contact. The Care Manager (LCSW) assists members appropriate for care management and care coordination services in achieving their optimal level of health through self-management.
