Prepare and serve high quality food by utilizing the menus and standardized recipes.
Adhere to scheduled mealtimes while using designated portioning tools according to departmental policies and procedures.

Follow food safety policies and procedures including but not limited to taking temperatures, completing Hazard Analysis and Critical Control Points (HACCP) logs, and proper cool down and storage of leftovers.

The Care Manager Social Worker is a key member of an integrated Care Team which includes a Nurse Care Manager and an Advanced Practice Provider.â¯The patients we serve often struggle with multiple serious diseases and behavioral health challenges. This position involves telephonic visits with some car travel to patients' homesâ¯
Basic Life Support (BLS) certification is required in this role. The company will support your certification completion through onboarding.
