Key Responsibilities
Provide in-home family support and case management to children, adolescents, and families
Assist with behavioral health interventions, parenting education, and crisis management

Qualifications
Caseworker I:
Bachelor's degree from an accredited university (preferred inSocial Work, Psychology, Counseling, Human Services, or related field)

Associate's degree and 4 years of experience providing services to families OR
High School Diploma/GED and 4 years of experience providing family or social services
Additional Requirements:
Strong interpersonal, communication, and problem-solving skills

Position Overview
The SocialWorker is an experienced professional and member of the patient care team who provides emotional and practical support for patients and their families in accordance with their unique Plan of Care. As part of the patient care team the SocialWorker meets with patients and their families, coordinates with the clinical team and connects them with community resources and services.
Essential Functions and Skills
Patient Care
* Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the Registered Nurse and other members of the interdisciplinary care team. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
* Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, suicide risk, neglect, or abuse, and plans intervention based on evaluation findings.
* Counsels' patients and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.
* Provides ongoing Bereavement support to individuals identified as moderate or high-risk.
* Assesses any special needs related to the culture of the patient and family while educating interdisciplinary care team. Includes communication, role of family, space, and any special traditions or cultural considerations.
* Works under the supervision of Master's Prepared SocialWorker (MSW).
* Maintains clinical records on all patients referred to social work.
* Educates patients and families and assists in preparation of advanced directives.
* Provides information and referral services for patients and families/caregivers regarding practical and environmental needs.
* Provides information to patients and families/caregivers regarding and serves as a liaison to community agencies.
* Maintains and develops contracts with public and private agencies as resources for patient and personnel.
* Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness.
* Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
* Assists Bereavement leadership in maintaining a program to meet the needs of families/caregivers for 13 months following the death of a patient to ensure compliance with all state, federal, and CHAP regulatory requirements.
* Ensures the initial bereavement assessment is completed or assigned to appropriate team member in a timely manner.
* Assists Chaplain in making condolence call to bereaved as needed.
* Assists with maintaining, organizing, and updating bereavement support group schedule.
* Assists family and patient in planning funeral arrangements, financial, legal, and health care decision responsibilities.
* Responsible for maintaining compliance with all HIPAA regulations and requirements. Treats all patient information as confidential and utilizes protected health information on a minimum necessary basis.

Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in-house Support Staff. We are seeking a Licensed Independent SocialWorker (LISW) or a Licensed Clinical SocialWorker (LCSW) to join our team! You will provide recovery-oriented rehabilitation services in group or individual settings. We care for individuals in long-term care settings, both skilled nursing and assisted living or personal care. We address a wide range of problems from anxiety and depression to severe and persistent mental illness to issues around death and dying. As part of a multidisciplinary team, our clinicians have the chance to make a big difference in the quality of life for these residents. We solve problems, deliver excellent clinical care, and provide a critical link in the care a vulnerable population. Essential Duties and Responsibilities:
Assess each resident with a valid consent to determine individual needs. Develop and follow a treatment plan to meet each resident's needs and ensure that all treatment provided is medically necessary.
Perform written or verbal consultation with clients' primary care physician, family, facility staff, and Eventus WholeHealth support staff as needed.
Document treatment plans, psychotherapy sessions, and treatment plan updates according to Eventus WholeHealth's procedures and policies and provide the facility with written documentation within 48 hours of service for inclusion in the patient's facility medical record.
Meet with Resident Care Coordinator or other staff as designated by the facility upon arrival to and departure from facility to review current facility care needs and communicate findings or concerns.
Establish and maintain open and positive communications with facility staff and administration. Provide verbal and/or written instruction or feedback regarding treatment plans and other pertinent caregiver information.
Be knowledgeable of and adhere to Eventus WholeHealth standards, policies, and procedures. Be aware of and adhere to all legal and regulatory agencies' rules and guidelines and professional ethical standards. Comply with all regulatory agencies governing health care delivery.
Conduct self in a professional manner at all times.
Maintain patient confidentiality at all times including appropriate use of cell phone, email, text messaging, patient records, and EMR. Agree to abide by and be knowledgable of HIPAA rules and regulations.
Consult with the clinical supervisor when facing an unfamiliar resident, family, or facility request or recommendation regarding the care of a resident.
Qualifications/Education: Psychologists-Doctoral degree from an accredited educational institution and a current license as a doctoral-level Psychologist and Health Services Provider issued by the state in which you will practice, required. Licensed Clinical SocialWorkers-Master's degree insocial work, and a current license to practice in the state in which you will practice, required. Knowledge:
Knowledge of assessment techniques, psychotherapy techniques, population-specific interventions, and delivering care following treatment plan.
Knowledge of how to use a variety of diagnostic tools and techniques, including the current edition of the DSM.
Knowledge of rules and regulations of bodies governing psychotherapy practice.
Knowledge of organization's policies and procedures.
Skills Required:
Skill in gathering and analyzing physiological, socioeconomic, behavioral, and emotional patient data.
Skill in accurately evaluating patient problems and providing appropriate advice, intervention or referral.
Skill in written and verbal communication.
Skill in exercising a high degree of initiative, judgment and discretion.
