This is a 40-hour position of on-site care per week (i.e., Monday's - Friday's), during normal business hours.
Job Responsibilities:
In this role the clinician is responsible for the clinical monitoring, coordinating and implementing of the mental health program in accordance with NCCHC standards and will serve as the mental health liaison between the medical unit and facility staff. Primary job responsibilities include:
Conduct clinical assessments based on referrals and self-referrals. Assessments include documentation of findings, including at a minimum a mental status examination and case conceptualization, diagnosis, and treatment plan. Makes referrals to psychiatry when appropriate.
Risk assessments are to be conducted utilizing structured interviews and risk assessment tools. Inmates at risk for suicide and/or engaging in self-harm are managed utilizing existing protocols according to current policy and procedures.
Serve as the PrimeCare Medical (PCM) mental health liaison for mental health services, outside the facility.
Facilitates admission and placement of inmates/patients to forensic hospitals when necessary.
Monitors all mental health services provided.
Monitors clinical services to ensure that standards of care are in accordance with NCCHC and accepted psychiatric/ psychological standards.
Is a member of the Quality Assurance Committee.
Develops and conducts in-service education programs according to annual in-service calendar.
Collects data and prepares monthly reports, which reflect performance

Coverage area: Washington County
Schedule: Full-Time; Monday - Friday

Responsibilities
As a Home Health Social Worker, You will:
Guide and counsel individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for them
Assist clients in obtaining government funds, education, or treatment
Secure information, such as medical, psychological, and social factors contributing to the client's situation, and evaluate these and the client's capacities. Ask clients to explain problems, such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments, to determine the nature and degree of the problem
Help patients and families through individual or group conferences regarding plans for meeting needs and aids clients in mobilizing inner capacities and environmental resources to improve social functioning
Help the client to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and the client's part in creating them
Develop a service plan designed to restore the patient to optimum social and health adjustment within the patient's capacity
Qualifications
Master's Degree in Social Work
Five plus years of clinical social work experience
Valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license
Ability to make psycho-social assessments and develop and implement viable care plans
