ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
2. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.
3. Assesses for, and educates interdisciplinary group, on any special needs related to the culture of the patient and family. Includes communication, role of family, space, and any special traditions or taboos.
4. Educates patients and families on, and assists in, preparation of advanced directives.
5. Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness.
6. Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.

14. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.

1. Participates in on-call duties as defined by the on-call policy.
2. Assists and supports the RN Case Manager to ensure that arrangements for equipment and other necessary items and services are available.

The Medical SocialWorker provides medically necessary intervention to patients who have social or emotional problems that are, or are expected to be, impediment(s) to the effective treatment of the medical condition and/or rate of recovery. Additionally, the Medical SocialWorker provides these services in accordance with physician's orders.
Essential Duties and Responsibilities:
Provides medically necessary social work intervention in accordance with physician's orders
Assesses patient's psychosocial needs; plans for, evaluates, and modifies interventions to meet the established patient goals
Collaborates with other disciplines to plan and evaluate team goals for each patient
Facilitates patient referrals to community agencies when appropriate
Provides financial counseling when necessary to achieve the treatment goals established by the multidisciplinary team
Assists patient/family/caregivers in planning for present and future psychosocial needs related to the patient's current status and prognosis, including terminal illness
Adheres to company policies and procedures while providing services and preparing/submitting required paperwork
Observes and reports patient progress and/or changes in patient status to physician and appropriate multidisciplinary team members
Functions as a resource person for other members of the health care team, providing inservice education and consultation in understanding the social and emotional factors related to health problems.
Maintains knowledge of the field including Medicare, Medicaid, and other third-party reimbursement sources.
Participates in planning and implementing quality improvement activities as necessary
Provides positive experiences in assisting patients, families, physicians, other departmental employees, referral resources, service agencies, and others.

Minimum Requirements:
Baccalaureate degree in Social Work (BSW) or Master's degree from a school of social work accredited by the Council on Social Work Education per state/client requirements required
Minimum of one (1) year of social work experience in a health care environment in the last two years preferred
Complies with all relevant professional standards of practice

Carroll Hospital's Adolescent Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) offer structured, short-term treatment for adolescents who need more support than traditional outpatient therapy but do not require 24-hour inpatient care. Through daily group therapy, psychiatric support, and collaboration with schools and families, our programs help teens stabilize, build coping skills, and successfully transition back to home and academic life.
As an Adolescent PHP/IOP SocialWorker, you'll be part of a compassionate, multidisciplinary team dedicated to empowering youth and families to achieve emotional wellness. You'll lead therapy groups, conduct psychosocial assessments, and coordinate care that fosters growth, resilience, and long-term success.
Our program is LGBTQ2S-affirming, gender-affirming, trauma-informed, and trauma-responsive, providing inclusive and culturally competent care for all adolescents and families. What You Will Do

Engage parents and caregivers through communication and education, reinforcing treatment goals and supporting continuity of care at home.
Incorporate DBT-informed and other evidence-based approaches to promote emotional regulation and interpersonal effectiveness.
Monitor progress and adjust therapeutic interventions in collaboration with the multidisciplinary team.
Document assessments, group sessions, and progress notes accurately and promptly in accordance with hospital, departmental, and legal standards.
Maintain confidentiality and uphold ethical and professional standards consistent with NASW, ACA, and AAMFT codes of ethics.
Participate in regular supervision, treatment team meetings, and clinical reviews, contributing to quality improvement and program development.
Coordinate discharge and aftercare planning, ensuring smooth transitions to community and school-based supports.

Qualifications
Minimum Requirements

Associate/Provisional: LMSW, LGPC, LGMFT candidates will be considered based on experience
Preferred Experience
1+ year of clinical experience in a hospital, residential, IOP, or PHP setting.
2+ years' experience facilitating group therapy for adolescents.
Experience with systems-involved adolescents and families (e.g., child welfare, juvenile justice, or special education).
Familiarity with DBT and other evidence-based and experiential modalities relevant to youth and family systems.
Strong skills in clinical documentation, assessment, and treatment planning.
Knowledge of community mental health and educational resources for adolescents.
