You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies. Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care. You will participate in all required continuing education and staff meetings. You will collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community. Safety & Quality. You will help with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes. You will also participate in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with federal, state, and local laws and regulations. What we're looking for Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE). Current licensure (in good standing) in applicable state is required unless employed in the state of AZ, PA or Guam. Must meet any practice requirement(s) for the applicable state. Demonstrated working knowledge of the English language and ability to communicate verbally and in writing. Basic computer skills, including Microsoft Office (Word, Excel, Outlook). Proficiency in all USRC clinical applications required within 90 days of hire. Preferred Previous experience in providing social services to dialysis patients preferred. May require travel to locations outside of a military base installation in a variety of physical environments. Due to the nature of working on military installations or related worksites, counselors may need to comply with various site-specific requirements to work at designated locations. The PRN Social Worker provides psychosocial evaluation and counseling patients who reside in a Skilled Nursing Facility or rehabilitation center regarding dialysis related issues; works in collaboration with the multidisciplinary treatment team to incorporate psychosocial needs/concerns in planning care; documents on patients in dialysis chart in keeping with facility policies to meet Medicare and Quality Assessment Performance Improvement (QAPI) requirements. This position requires facilitation as needed between the dialysis unit and the Social Services Department of the SNF. Function as a member of the Interdisciplinary team. Reports directly to the Manager of Social Services. Essential Functions Conducts psycho-social assessments on each patient and determines social work treatment plan compatible with a total medical treatment plan per CMS guidelines (initial, 90-day, annual and as needed for unstable patients). Track CIPA/POC for SNFs, coordinate POC meetings with RN, RD, MD and patient. Complete KDQOL and other tools as required Provide monthly documentation in every patients chart on caseload. Provide education to new patients regarding their Rights and Responsibilities as well as the Grievance Procedure as well as documentation that this was completed. Complete monthly tracking of patients needed for QAPI. Maintains professional working relationship with other staff of Concerto as well as in the skilled nursing facility. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person. Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR. Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers. Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care. Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment). Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload. Be comfortable utilizing Excel and Word and completing assessments and productivity trackers daily. Time-management skills are a must and the BHCM needs to be able to manage their day effectively through calendars and the EMR system. This role is telephonic, however in person visits are done and the BHCM has to be comfortable completing them both ways. Qualifications: The Social Worker Care Manager will have a minimum of MSW Preferred Licensure or provisionally licensed Behavioral Health experience and knowledge Must be licensed driver with an insured automobile in good working order Ability to communicate effectively, both verbally and in writing.
