Job Summary: The Licensed Clinical Social Worker (LCSW)/ Licensed Marriage and Family Therapist (LMFT) is an integral part of the clinical care team providing behavioral health services to the patients and families served by PCHC. Coordinates patient care services for patients across the continuum of care through collaboration with the patient. Participates in our patient-centered medical home, collaborating with medical providers in developing a standard of care. The LCSW/LMFT provides baseline mental health assessments, counseling, crisis intervention and referrals for psychiatric evaluation and other community resources when needed. Essential Duties: * Provides strengths-based psychosocial assessments and diagnosis of patients with mental health and substance abuse concerns. * Develops appropriate plans of care, crisis intervention and referrals for individuals, couples, and families with various psychosocial needs/concerns. * Provides brief psychotherapy using evidenced-based treatment modalities to individuals, couples, and families. Maintains patient confidentiality as it pertains to HIPAA and Protected Health Information (PHI). * Participates in the Comprehensive Perinatal Services Program including assessing for risks during pregnancy and works with case manager to provide appropriate linkages. * Available for warm hand-offs during the clinic day to assist Providers and other staff. * Interfaces with other care team members, and maintains open communication, collaboration and care coordination with Providers. * Promotes the principles of a Patient Centered Medical Home. * Participates in quality improvement activities. * Documents client progress notes clearly, concisely and in a timely manner in patient's Electronic Health Record * Assist with provider and staff education and training to promote a strong culture of integrated behavioral health care in a primary care setting. * Provides on-site Supervision to MSW, MFT and/or BSW interns. * Provides services at all ParkTree locations, as assigned. * Complies with all mandatory trainings. Job Qualifications and Skills: * Master's in Social Work or Master's degree in Marriage and Family Therapy from an accredited program required. * Licensure as a Licensed Clinical Social Worker or Licensed Marriage and Family Therapist by the California Board of Behavioral Sciences, required. * 2-4 years' experience, preferred. * Counseling, listening skills. Treats all patients and staff with dignity and respect. * Productivity goal of 7-8 patients per eight-hour day (90% direct patient care/10% administrative time). * Bilingual English/Spanish required. Essential Duties: · Acquires, maintains, teaches and demonstrates knowledge of Prevention of Sexual Abuse (PSA) and Sexual Harassment standards at care provider facilities, evidence-based practices, and trauma-informed care and maintains a professional environment that is consistent with Title 22, licensing regulations, and established agency policies. · Participates in interdisciplinary treatment planning. · Provides support, training, to the Resource Family, working in collaboration with various members of the treatment team · Conduct Pre-Service and on-going trainings to Resource Families as indicated. · Develops a comprehensive psychosocial assessment. · Conducts assessments and screens as required. · Provides crisis intervention consultation to all members of the team. · Assists in maintaining client. · Provides timely and accurate documentation as required by payment source. · Transports clients to appointments and activities as required. Requirements Minimum Qualifications: MA, MS, MSW, PsyD or PhD in a related field of human services from an accredited school. Proficiency in threshold language is required of positions primarily assigned to mono-lingual clients. (Some contracts may allow waiver of advanced degree, with a minimum BA/BA and two or more years of directly relevant experience.) Additional Requirements: Must be license-eligible or a registered intern and must have a minimum of one semester of relevant practicum, working with adolescents or families. · Must possess and maintain a valid, unrestricted California driver's license and current vehicle insurance. · Must be able to complete and maintain required certifications and training including, but not limited to, First Aid/CPR, PRIDE, and as applicable to program.
