The Social Work Reentry Department, within the Division of Correctional Health Services (CHS), is seeking a Social Worker. The Department is comprised of 70+ healthcare professionals who are responsible for providing re-entry services to individuals detained in the NYC jail system. The Social Work Department supports patients' rights and criminal-legal advocacy efforts in collaboration with patients' criminal defense attorneys, Alternative to Incarceration (ATI) programs, and other post-release supervision providers. Monitor patient progress and adjust treatment plans as needed. Requirements: Active and unrestricted license to practice as a Licensed Mental Health Counselor in State applying for. Effective oral and written communication in English Basic proficiency with technology, including electronic health records (EHR). In addition to performing the duties of Assignment Levels I and II at a higher level, also performs the following: Essential Duties and Responsibilities: In collaboration with mental health leadership, coordinates and implements counseling programs for an assigned area of a health care setting. Monitors and guides mental health counselors assigned at Levels I and II in the establishment and provision of these programs. Provides supervision for mental health counselors assigned at Level I, practicing under a limited permit. Assigns appropriate supervisory responsibilities to mental health counselors assigned at Level II. Assesses individual performance competencies for mental health counselors assigned to Levels I and II, and arranges staff development opportunities based on assessment outcomes. . Facilitates the participation of consumers of mental health services within assigned area. Acts as a liaison between the health care setting and community agencies or programs.
