Job Duties Performs Mental Health Assessments for patients to ascertain their diagnoses and establish treatment plans Provides individual and group counseling and psychotherapy Provides crisis intervention and/or counseling to diffuse conflicts among patients Completes medical forms and documentation related to patient care including any necessary to facilitate transfer, and if indicated, referrals to outside programs or psychiatric clinics Makes and/or tracks all referrals to other mental health agencies and providers Works collaboratively with specialists for consultation and referral purposes Works with others on the intake team to help determine whether or not a patient would be best helped by the facility or another agency.
