PURPOSE OF POSITION:
The School Social Worker is that professional charged with the responsibility to serve as liaison between the school and home and school and community agency. The School Social Worker reports to the Program Supervisor, of those special education programs, to which assigned.
KNOWLEDGE & SKILL REQUIREMENTS:
Education Required: Master's degree in social work
Certification Required: School Social Work Certification; current license as a Social Worker (LSW) or licensed Clinical Social Worker (LCSW) issued by the Pennsylvania Department of State
Experience Required: Field work or internship in social casework situation
Other Qualifications: Knowledge of school system, family system, social agency operation and ability to fit all together. Ability to work with families of children with disabilities in a knowledgeable fashion.

Job Responsibilities:
Conduct clinical assessments based on referrals and self-referrals. Assessments include documentation of findings, including at a minimum a mental status examination and case conceptualization, diagnosis, and treatment plan. Makes referrals to psychiatry when appropriate.
Risk assessments are to be conducted utilizing structured interviews and risk assessment tools. Inmates at risk for suicide and / or engaging in self-harm are managed utilizing existing protocols according to current policy and procedures.
Serve as the PrimeCare Medical (PCM) mental health liaison for mental health services, outside the facility.
Facilitates admission and placement of inmates/patients to forensic hospitals when necessary.
Monitors all mental health services provided.
Monitors clinical services to ensure that standards of care are in accordance with NCCHC, and accepted psychiatric/ psychological standards.
Is a member of the Quality Assurance Committee.
Develops and conducts in-service education programs according to annual in-service calendar.
Collects data and prepares monthly reports, which reflect performance.

JOB SUMMARY:
The social worker facilitates effective referral coordination and information exchange with the Interdisciplinary Team (IDT), prospective and current hospital-based general inpatient (GIP) patients, their caregivers, and hospital and community teams.
Provides explanations about the hospice benefit and organization services in a manner consistent with the patient and family level of understanding.
Records information necessary to assist the hospice physician in determining eligibility, plans and coordinates discharge needs for the prospective patient, and collaborates with community senior living community staff and hospice teams to facilitate a safe and effective discharge plan.
Utilizes SW interventions, education, counseling, and support services to address the identified psychosocial needs of patients and caregivers.
ESSENTIAL FUNCTIONS:
Collaborates with hospital providers and staff IDT to develop a plan of care, complete initial and ongoing comprehensive assessments, and plan/evaluate SW interventions.
Conducts admission assessments/information visits and follow-ups on hospitalized patients
Conducts high-risk admission assessments/follow-ups in hospitals where psychosocial issues are present, including but not limited to patients living alone, inadequate caregiving/decision-maker situations, and significant mental health/substance abuse concerns.
Documents in accurately and concisely at the time the service is provided.
Initiates, coordinates, and revises the plan of care on an ongoing basis with input from the IDT, patient, caregiver, hospital providers, and staff.
Participates in hospital discharge planning, when needed, collaborating with hospital case management on the plan.
Participates in IDT meetings to report on the plan and update the comprehensive assessment.
Provides education and support to patients and caregivers regarding diagnosis, care, treatments, medications, and end-of-life expectations in a method or manner that they understand.
Provides education and support to hospital staff.
Supports special needs of patients and families including the pinning of Veteran patients through the We Honor Veterans program.
Communicates effectively with organizational staff, physicians, patients, families, hospital staff, outside contractors, and community agencies.
Anticipates, assesses, and responds to pain and other symptoms of physical, emotional, and spiritual suffering.
Works in collaboration with IDT members, managing the psychosocial care of assigned caseload.
Identifies patient and caregiver stressors and strengths to direct interventions throughout the patient's illness, including initiating post-death bereavement support,
Demonstrates proficiency in navigating systems, knowledge of community resources, advocacy, individual/group counseling, and rapport building,

Position Details
The Functional Family Therapist will provide structured family therapy sessions to children and their families using the Functional Family Therapy (FFT) model. The therapist will maintain a caseload of 10-12 families and deliver services primarily in the family's home, scheduling sessions based on family availability. This role includes completing training in the FFT model and working closely with a direct supervisor and national consultant to ensure fidelity to the model and successful outcomes for each family.
Schedule
All sessions are scheduled based on the needs of the family. The average caseload is 8-12 families, each seen at least once per week. Working hours vary, as staff have autonomy to schedule sessions directly with families. Most staff typically work shifts such as 10 a.m.-6 p.m., 11 a.m.-7 p.m., or 12 p.m.-8 p.m., and some may choose to work occasional Saturdays to accommodate client needs. There is one mandatory in-office meeting every Monday at 10 a.m. Overall, the schedule offers flexibility similar to Family Based services.

Identifies obstacles to adherence and assists in patient/family/caregiver understanding of goals and interventions; address patient/family/caregiver questions and issues.
Assesses for potential risk for suicide and/or abuse or neglect.

Provides clinical therapeutic services to pediatric patients and families such as, but not limited to, individual counseling (loss, anticipatory grief and bereavement, coping, transition) to patient and/or family, crisis intervention, sibling support (in collaboration with Bereavement Counselors), longer-term individual and family counseling for pediatric palliative care patients if indicated.
Establishes treatment goals and revises counseling interventions as indicated. Evaluate patient/family response to psychosocial interventions; identify obstacles to illness management and assist in understanding goals of intervention.
Consults with primary care and other providers to provide information about the full range of clinical resources available to the child; identifies and makes referrals to other community resources and services to support the plan of care and needs of the child and family and assesses patient/family/caregiver ability to access them.
Evaluates patient/family/caregiver response to intervention(s) when referred to community agency and the satisfaction of the services(s) provided, in addition to providing supportive presence for those staff (private duty nursing staff, etc).
Collaborates with primary care and other providers to support and promote the normal growth and development of the child and his or her siblings.
Participates in meetings as a contributing member of the team in ongoing communication, coordination, and continuity of the patient and family care/service needs from the psychosocial perspective, this includes developing and revising patients POC.
Provides social service coverage after hours, as needed.
Participates in program, organizational, community in-service programs or presentations, as appropriate, to support palliative care and hospice education and promote awareness of the Sunflower pediatric program.

Position Details
Family Based Therapist (FBT) works as one half of a team which provides treatment services to families including therapy and counseling to child and family. FBT helps with skills training and education designed to enable families to care for their mental health children at home. FBT works flexible hours, including evenings and weekends, to meet the needs of the children and families on caseload. FBT travels to client homes, other agencies, and community resources for the purpose of arranging and providing community and home-based services for children, adolescents, and families.
