POSITION SUMMARY:
The Social Worker provides direct case management services to On Lok participants. Provision of support services to resource families assigned to the social worker.
Maintaining regular contact with the County Case Managers
Provide On-Call services on weekends and after office hours.

POSITION SUMMARY:
The Social Worker provides direct case management services to On Lok participants. Responsibilities:
As a Social Worker within the Assessment Team, you will work to prevent safeguarding concerns, investigate significant harm, and use legislation to protect service users. You will assess and meet the needs of individuals, families, and carers through therapeutic work, care planning, and management. Managing a caseload, you will make complex decisions, plan, evaluate, and review casework. You will handle complex situations with multi-agency input and significant risk factors. Ensuring appropriate care, you will innovate with resources and supervise service users. You will provide expert witness in court, work with various agencies, and influence case outcomes. You will mentor less experienced Social Workers, manage your workload, and stay updated with best practices. You will participate in training, identify learning needs, and uphold Social Work ethical principles. Maintaining accurate records, you will produce high-quality assessments and support council policies on equality and anti-oppressive practice.
As a Social Worker within the Assessment Team, you are required to:
Hold a Social Work England registration
Be eligible to work in the UK
Hold a full UK license
Social Work Qualification, degree or equivalent
At least 2 years' experience as a social worker within children's services
