RESPONSIBILTIES:
Participates on the IDT's initial assessments, care planning and on-going re-assessments of participant care.
Attends IDT meetings; actively participates in team meetings by sharing pertinent information, providing follow up to assigned tasks and helps to develop participant's plan of care.
Completes all assigned assessments - (initial, semi-annual, annual, service request and/or significant event).
Assesses the psychosocial needs of the participant and provides supportive counseling, working collaboratively with behavioral health providers.
Facilitates hospital, rehabilitation and nursing home (NH) admissions and discharges as determined by the Interdisciplinary Team. Ensures that PASRR documentation is completed for NH admission.
Assists in the conversion process of the participant from community to long-term care. Works collaboratively with Medicaid Specialist, skilled nursing facility, and participant's caregiver to complete conversion.
Arranges and facilitates family meetings, as needed.
Refers participants and families to appropriate community services and acts as liaison and/or advocate with community organizations for participants.
Maintains professional, accurate and timely social service documentation in the participants' medical records.
Conducts participant council meetings as assigned.
Works collaboratively with Director of Social Work and Behavioral Health provider to ensure guardianship is up to date. Educates participant regarding health care proxy (HCP). Assists participant in completion of HCP form.
Works collaboratively with fiscal department to maintain participant insurance benefits and completes required documentation of fiscal information in the medical record.
Reviews plan of care with participants, guardian, and/or activated health care proxy as assigned.
Complete authorizations for home care and other approved services timely and accurately.
Completes home and/or skilled nursing facility visits to assess participant as indicated.
Works collaboratively with Palliative care team; Assists with end of life planning as indicated.
Provides timely communication to appropriate staff regarding the following: (disenrollment, conversion to long term care, transfer of sites, participant and/or caregiver demographic changes).
Reports allegations of abuse to appropriate state agency; provides support and resources to participant as he/she will accept; completes required documentation.
Ability to pass a fit test. Position requires mask where seal is critical. Incumbent is required to not have facial hair that interferes with a tight seal of the respirator.
Performs other duties as required.

Qualifications:
Current Social Work licensure in the Commonwealth of Massachusetts at the Masters level (L.I.C.S.W. or L.C.S.W.) required
Minimum of 1 experience in Social Work providing traditional Clinical or Case Management services with a geriatric population
Current C. P. R. Certification or ability to become certified
Treat all participants in a welcoming and professional manner.
Strong verbal, written and listening skills with ability to multi-task in a fast pace environment

Part Time/Hybrid Position
The licensed clinical socialworker will provide supportive care services for the Adolescent and Young Adult (AYA) Cancer Program. Responsibilities include psychosocial assessment, counseling, group facilitation, and resource navigation for AYA patients and their families.
Following the initial AYA needs assessment, the socialworker will meet with patients and/or families to evaluate psychosocial needs and collaborate with the AYA program manager and nurse practitioner to coordinate care as well as with disease group socialworkers. The Supportive Services Specialist is primarily responsible for providing trauma-informed, person-centered coordination of care and services for clients served by Hebrew SeniorLife's (HSL) Center for the Prevention of Elder Abuse and Neglect (CPEAN). The Supportive Services Specialist will also be responsible for maintaining relationships with CPEAN's community partners and referral sources across Greater Boston and Eastern Massachusetts, as well as the sites and departments across the Hebrew SeniorLife organization supporting CPEAN's efforts. The role is hands-on, with both micro-level and macro-level work.
CPEAN provides safe shelter and services for seniors suffering from abuse, neglect or financial exploitation, and works to prevent elder abuse by increasing awareness and providing education. CPEAN operates the first and only emergency shelter program in Massachusetts exclusively for seniors, and is funded largely by philanthropy.
The Supportive Services Specialist is a full-time position. Fully on-site opportunity with primary office location in Roslindale, and expected routine travel to other HSL sites and non-HSL locations across the Greater Boston area.
Position Responsibilities:
Provide specialized case management and supportive services coordination for all CPEAN clients, in collaboration with referring agencies or other community partners as appropriate
Manage CPEAN's elder abuse prevention phone line and email address, and timely follow up on any referrals or inquiries
Perform intake, initial and ongoing assessment, and discharge planning utilizing specific person-centered, trauma-informed tools
Create Supportive Services Plans (SSPs) for all CPEAN clients, and track client progress and outcomes using aforementioned tools; monitor types and frequency of services utilized
Work collaboratively with Case Manager and Housing Placement Specialist to provide supportive services directly to CPEAN clients as needed and dependent on qualifications and bandwidth; examples include - accompanying clients to appointments, assisting clients with completing benefit applications (paper and electronic), providing trauma-informed counseling, etc.
Conduct outreach to referral sources (including but not limited to protective service agencies, senior centers, hospitals, long-term care facilities, rehab facilities, domestic violence shelters, homeless shelters, faith communities, etc.) to foster collaborative relationships, capture appropriate referrals, continuously improve referral/screening/ admission procedures, etc.
Work collaboratively with Case Manager and Housing Placement Specialist to assess and address client housing needs
Assist Program Director as needed with facilitating multidisciplinary teams and elder justice task force meetings in and around Boston, as well one-on-one meetings between CPEAN and select Adult Protective Services (APS) agencies
Create and deliver (solo and/or with CPEAN colleagues and/or community partners) educational presentations and trainings on elder abuse prevention and related topics
Assist Program Director in creating, reviewing and enhancing CPEAN standardized documentation, policies and protocols around intake, care/service coordination, discharge, and post-discharge follow-up
Participate in CPEAN donor events and funding solicitations as needed
Represent CPEAN in committee and coalition meetings as appropriate, report on challenges and explore resources
Perform other related duties as assigned or as responsibilities dictate.
Qualifications:
Minimum 3-5 years of experience, including both direct care and case management and/or care coordination; experience working with survivors of trauma strongly
Must have a Masters degree in social work. Must be currently licensed to practice in the Commonwealth of Massachusetts. LICSW preferred; LCSW considered if on path to LICSW and depending on relevant experience.
Must have knowledge of safety planning, trauma-informed services, and person-centered care/services, or willingness to learn through focused training opportunities.
Experience creating and delivering virtual and in-person educational presentations to wide varieties of audiences a plus.
Familiarity with the state Adult Protective Services system a plus.
Experience working in a health care or congregate care setting a plus.

CPEAN provides safe shelter and services for seniors suffering from abuse, neglect or financial exploitation, and works to prevent elder abuse by increasing awareness and providing education. CPEAN operates the first and only emergency shelter program in Massachusetts exclusively for seniors, and is funded largely by philanthropy.
The Supportive Services Specialist is a full-time position. Fully on-site opportunity with primary office location in Roslindale, and expected routine travel to other HSL sites and non-HSL locations across the Greater Boston area.
Position Responsibilities:
* Provide specialized case management and supportive services coordination for all CPEAN clients, in collaboration with referring agencies or other community partners as appropriate
* Manage CPEAN's elder abuse prevention phone line and email address, and timely follow up on any referrals or inquiries

Qualifications
Social Work:
Master's Degree in Social Work; Current licensure as a Licensed Certified SocialWorker (LCSW)

Mental Health:
Master's Degree in Counseling Psychology, Clinical Psychology, Developmental Psychology or Educational Psychology

Two (2) years of pediatric experience preferred.
Effective verbal and written communication skills.

Role:
The SocialWorker plans, coordinates and manages social work services for frail older adults and caregivers of Elder Service Plan (ESP) within the community as part of Integrated Disciplinary Team (IDT). Duties include but are not limited to conducting initial, semi, and annual assessments; participating in the Intake process for potential participants; providing supportive counseling to ESP participants and their family members; actively participates in Interdisciplinary Plan of Care meetings and other team meetings.
Responsibilities:
* Participates in the intake screening process in potential participants residence or home as needed, conducts a bio-psychosocial assessment.
* Conducts timely initial, annual, and semiannual psychosocial assessments in client's residences in order to develop participants plan of care to include assessment tools of PHQ-9, MOCA, and CDR.
* Actively participates in required meetings to include but not limited to Morning/Weekly Team, Plan of Care, and Family Meetings.
* Responsible for attending monthly service plan meetings for participants in assisted living and nursing home facilities.
* Provides individual and family supportive counseling at ESP and client's residences and conducts participant and caregiver support groups.
* Manages clients during psychiatric hospitalizations.
* Assists/advocates for interdisciplinary team, clients and families with community resources.
* Works with financial staff to assist with bi-annual financial redeterminations.
* Must be able to travel between Harbor sites and participant's residences as needed in a timely manner.
Requirements:
* MSW from an accredited school of social work
* Current MA LCSW or LICSW License Required, current BLS Certification required
* MoCA certification required or obtained within 3 months of hire
* 2 years of professional experience of which 1 year was in providing direct behavioral health therapy to an adult population
* 1 year of experience working with the frail elder population (paid or volunteer experience), 2 or more years of geriatric experience preferred
