Duties and Responsibilities:
Essential functions of the position may include, but are not limited to, the following:
Engage in appropriate assessment activities that result in the identification of the special needs of individual students to include interviews, observations and/or home assessments.

Maintain appropriate reports and record keeping procedures to document the above.
Coordinate home and school interventions to manage and improve student attendance.

Your Specific Responsibilities:
This position provides basic social services and direct WIC Program breastfeeding education, assistance and/or support to WIC participants in various locations throughout Miami Dade County. This position requires specialized training to provide appropriate breastfeeding counseling for WIC participants. The incumbent provides services for the WIC Program in accordance with DHM 150 24, and the FL WISE User's Manual. Incumbent is responsible to follow and adhere to all rules, regulations, policies, and procedures of the Florida Department of Health (FL DOH) and the WIC and Nutrition Program. The incumbent in this position will have the authorized "need to know" access to confidential WIC client records, and the FL WISE data system. Promote breastfeeding as the normal, preferred method of infant feeding by providing basic research based breastfeeding information to pregnant and breastfeeding or post partum mothers through peer to peer counselor contacts. Provide support through personal interaction, information on aspects of normal breastfeeding, anticipatory guidance to reduce the occurrence of problems, and/or additional information or resources. The incumbent will model breastfeeding and parenting of infants and may bring their infants (under one year of age) to class demonstrations, clinic waiting rooms, and other WIC program events.
Counsel WIC pregnant and postpartum mothers by telephone and/or in Miami Dade WIC units to manage common maternal and infant breastfeeding issues. Properly screen and assess client's breastfeeding needs, obtain demographic information and breastfeeding/health data, and document in the client's electronic record. Give basic research based information to assist mother to prevent, handle and remedy common breastfeeding concerns. Provide appropriate referral to IBCLC and/or other health care professionals and follow up with accurate documentation according to established protocols as needed and/or required.
Directly encourage and support breastfeeding clients through telephone assistance via the Miami Dade WIC Breastfeeding Helpline. Maintain Miami Dade WIC Breastfeeding Helpline during official operating hours, nights, weekends and/or holidays. During core work hours incumbent must maintain online availability to answer live calls. Return client calls within the approved time frame as determined by the Breastfeeding Coordinator. Provide appropriate follow up as needed and/or required.
Teach breastfeeding classes and lead support groups in FL DOH Miami Dade WIC units, hospitals, outreach events, local community organizations and businesses, as assigned.
Participates in outreach activities including but not limited to health fairs, DOH events, community programs, contacting healthcare professionals, contacting potential WIC clients, attending outreach activities, scheduling and rescheduling client appointments, and other outreach activities as assigned.

Medicaid and other benefit eligibility assessment:
Conducts appropriate assessment of needs and financial benefit eligibility.
Assesses patients for Medicaid criteria and assists with application process as needed.
Assists patients to obtain community resources/services as appropriate, e.g. meals, medications, housing, daycare, DME, HHA. etc.
Resource coordination and intervention:
Serves as care coordinator, linking patients with internal and external resources, prioritizing patients on the PCP's HPP and Top 40 patient lists.
Facilitates connections to community resources as identified by the patient's SDoH Wellness Screening.
Works with patient, family, and case manager to facilitate applications for higher level of care.
Works to provide self-management support and ongoing phone contact with patients.
Maintains an accurate repository of social wellness tools for the care team's awareness and utilization.
Communication.
Maintains communication with other healthcare team members by attending appropriate meetings (i.e. weekly Super Huddles and Transitional Care Team meeting.).

Timely and accurate documentation:
Maintains timely, accurate, thorough and appropriate documentation/reports per company policies and procedures. Initial psychosocial assessments will be completed withing 48 hours. Additional duties and responsibilities may include:
Works closely with the transitional care team to secure the appropriate level of care post hospital/SNF discharge. Further interventions may be conducted in the center, by phone call or patient's home.

The Care Manager (GCM) will manage a small caseload of private pay clients, provide coordination of services, resources and ongoing monitoring and management to clients in their home. This can include clients who are residents of Independent Living, Assisted Living or Nursing Facilities. Additionally, the Care Manager will help drive business and work with referral sources. Develops strong relationships with Community and professional organizations that will generate client referrals.
Job Duties May include:
Maintain about 10-15 hours per week
Perform in depth client assessments including medical, psychosocial, environmental, financial, legal and family assessment
Complete Fall Risk Assessment, Mini Mental Status Examination and Depression Screening for each client
Recommend and coordinate all service needs for clients
Work with client and family to develop plan of care to ensure client safety and all other needs are addressed
Work with client, family, and professional staff to implement plan of care
Collaborate with hospital, nursing home and AL/memory care staff, physicians and other medical providers as well as attorneys, financial planners, meals on wheels, and other professionals in geriatric service provider world
Perform regular home visits to monitor client status, home environment or senior community
Provide ongoing emotional support and counseling to the client and their family as needed
Provide problem solving support to clients and fellow care managers as needed
Provide crisis intervention and display critical thinking/problem solving skills
Work with business development team to meet prospective clients, families and/or referral sources
Provide back-up support to the GCM team as needed
Carefully track and document time that is spent on each client to facilitate accurate billing
Attend educational programs and networking events

We are currently in need of a Medical Social Worker to provide medical social services to patients in their homes in accordance with physician orders and under the direction and supervision of the Director of Nursing or another appropriate supervisor.
Position Responsibilities:
Assist in the admission process of the patient to the Agency by performing an initial evaluation, assessing the patient's psychosocial status and evaluating the patient, family and home to identify socioeconomic and emotional factors that will affect the plan of treatment.
Assist in development and implementation of the interdisciplinary patient care plan as it pertains to medical social work.
Observe, record and report changes in the patient's emotional and social factors that affect the patient's illness and his/her need for care and his/her response to treatment.
Consult with the attending physician concerning alteration of the plan of treatment.
Maintain and submit written clinical records as deemed by the Agency, including the initial evaluation, the care plan and daily notes.
Evaluate the patients and family's response to, and effectiveness of, the medical social work intervention.
Confirm, on a weekly basis, the scheduling of visits with the Director of to facilitate coordination of other staff visits.
Participate in Agency activities and committees when appropriate to include staff development activities and in-service education.
Supervise the social work assistant per policy.
Participate in discharge planning.
Assume responsibility for self-development by continually striving to improve his/her Medical Social Worker practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and individual research and reading.
Skilled Medical Social Work Services:
Assess the patient's potential to cope with his/her social and health problems.
Act as consultant to the members of the health team, assists them in understanding the social, emotional and environmental factors related to the patient's health problems.
Help patients to utilize the resources of their families and the community.
Provide rehabilitative and supportive casework.
Assist patients and their families in coping with personal and environmental difficulties which might predispose them toward illness or interfere with obtaining maximum benefits from medical care.
Qualifications:
Master's Degree from a School of Social Work approved by the Council of Social Work Education.
Must have or be in the process of acquiring certification from the Academy of Certified Social Workers.
Two years' experience preferred, with at least one year of experience in a health care setting (hospital, clinic, rehabilitation center, etc.).
Must have a criminal background check clearance, national sex registry clearance and OIG clearance.
Must have current CPR certification.
Compensation: $60.00 - $80.00 per hour
