Full-time Description PrimeCare Medical is currently looking for a Licensed Social Worker (LMSW/LICSW) or Licensed Clinical Mental Health Counselor (LCMHC) to work as a Behavioral Health Professional in the medical department at the Carroll County Department of Corrections in Ossipee, NH. This a Full-Time, 40-hour position, working on-site Monday - Friday. Job Responsibilities: Conduct clinical assessments based on referrals and self-referrals. Assessments include documentation of findings, including at a minimum a mental status examination and case conceptualization, diagnosis, and treatment plan. Makes referrals to psychiatry when appropriate. Risk assessments are to be conducted utilizing structured interviews and risk assessment tools. Inmates at risk for suicide and / or engaging in self-harm are managed utilizing existing protocols according to current policy and procedures. Serve as the PrimeCare Medical (PCM) mental health liaison for mental health services, outside the facility. Facilitates admission and placement of inmates/patients to forensic hospitals when necessary. Monitors all mental health services provided. Monitors clinical services to ensure that standards of care are in accordance with NCCHC, and accepted psychiatric/ psychological standards. Is a member of the Quality Assurance Committee. Develops and conducts in-service education programs according to annual in-service calendar. Collects data and prepares monthly reports, which reflect performance. Emergency Department Crisis Consultations • Provide psychosocial assessment including diagnoses, interventions, and treatment planning • Meeting/speaking with appropriate collaterals • To coordinator with patient's primary care provider/specialist to for transitional care management • Provides education to both patient and family on treatment that are available • Documentation of all interventions in the patient's medical record. • Recommendation of plan: outpatient, voluntary hospitalization, involuntary admission and review with physician/APRN/PA • Execution of discharge plan including making appropriate referrals and follow up inquiries • Insurance pre-authorization when required and completion of transfer forms 4. MHP Social Work Liaison • Assists in building and maintaining good relationships in the community. • Identify and refers to health related agencies to ensure continuity of care after discharge. • Adheres to safety practices, reports accidents/hazards and maintains equipment as outlined in hospital policies. Supervising, assigning and carrying out NHH Transportation Services for patients to appointments. Initiating or overseeing the initiating of guardianship and/or involuntary commitment proceedings consistent with RSA 135 and 464-A, while ensuring congruency with the Social Work Code of Ethics. Adhering to all applicable laws and policies including The Joint Commission on Accreditation of Healthcare Organizations (JCAHO), Health Care for All (HCFA), NHH policies and the Health Engagement Model (HEM). Monitoring other legal issues such as the status of probation or parole involvement, pending court hearings for criminal or civil actions, facilitating appropriate involvement of the patient in these proceedings and giving direct testimony at court hearings as appropriate. Providing support, modeling and assistance to other hospital staff to reinforce courteous interactions and clinically appropriate interventions with patients. Providing clinical analysis and recommendations at diagnostic and treatment review conferences as necessary. Assisting in covering social service needs throughout NHH as they arise. Qualifications Basic Requirements/Certifications: Minimum of one year of clinical experience in direct patient care as a licensed Clinical Social Worker. Experience in detention or correctional healthcare, residential healthcare, a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided direct care. Experience with pediatric and adult patients. Education Required: Current license in New Hampshire and have no record of prior or pending disciplinary action by the licensure body or agency. Must possess at least a master's degree in social work (MSW). Program must be accredited by an entity recognized by the U.S. Department of Education. Licensure must be maintained current, and any required education, trainings, continued education to maintain licensure must be completed within the required timeline for completion by the licensing/ certifying authority. Candidate must not have been convicted of, pled guilty to, pled nolo contender to, or received probation before judgement for any crime other than a minor traffic violation, the record of which has not been expunged. Basic Life Support (BLS) Certification must be from one of the following approved organizations: American Heart Association (AHA) BLS for health care providers American Red Cross CPR/BLS for the professional rescuer American Safety and Health Institute (ASHI) CPR and AED for healthcare providers and professional responders Course components must include Adult, Pediatric, and Infant CPR, Foreign Body Airway Obstruction, and Automatic Defibrillator (AED) proficiency. Physical Requirements: Work is normally performed in a typical interior/office work environment. Work involves sitting and standing for prolonged periods of time. May require bending, stooping and lifting up to 15 lbs. Other Special Qualifications: Must be able to read, write and speak English to effectively communicate. US Citizen or Permanent Resident Card is required. Reliably commute or planning to relocate to Concord, New Hampshire. May be required to travel short notice, or to serve in various medical units within the region/location. Additional Information Compensation | Min: $48.00 Max: $52.00 Pay range is based on several factors and may vary in addition to a full range of medical, financial, and/or other benefits. Final salary and offer will be determined by the applicant's background, experience, skills, internal equity, and alignment with geographical market data.
