The Social Worker will play a key role in enhancing participants' emotional well-being, coordinating care, and supporting families while ensuring compliance with state regulations. Key Responsibilities Conduct psychosocial assessments and develop individualized service plans for adult day care participants. Provide counseling, emotional support, and crisis intervention as needed. Coordinate with healthcare providers, administrators, caregivers, and community resources to ensure continuity of care. Assist participants and families with social services referrals, benefits, and community programs. Maintain accurate and timely documentation and case records in compliance with state and facility requirements. Participate in interdisciplinary team meetings and care planning. Qualifications Current New Jersey certification/license as CSW, LSW, or School Social Worker (required).
