Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
Monitor patient progress and adjust treatment plans as needed.
Requirements:
Active and unrestricted license to practice as a Licensed Clinical Social Worker in State applying for.
Effective oral and written communication in English
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.
Psych or Addictions experience is a plus.

General Responsibilities: This is a professional position responsible for the provision of a wide range of psychosocial services to terminally ill patients and their families, including completing the comprehensive psychosocial assessment and regularly assessing the patient and family psychosocial needs on an ongoing basis. The SW will meet with each new client seen by the providers in order to thoroughly screen and assess client's Social Determinants of Health and to connect clients to appropriate social service supports and resources*. The SW will provide essential information regarding community resources to clients, their family, and the PHMC Health Network team. 3. Provides support and case management of all assigned families by maintaining quarterly contacts throughout process, communicating information to the family, referring family to appropriate community support systems when necessary and documenting all correspondence in Holt's database.

In case of an adoption disruption or dissolution, maintain relationship with family to ensure transition plan is developed that meets the child's needs. Assist child and family with transition of child to new placement.
6. Contributes to public knowledge about Holt and supports Holt programs by participating in activities arranged by Holt including picnics or parent groups, representing Holt at adoption meetings, coalitions and organizations, and performing other outreach activities as appropriate.
7. Maintains professional knowledge by attending relevant seminars and course work, keeping informed about State regulations, legislation, adoption issues and child-related issues overseas; maintaining license, if required.

Description: The Medical Social Worker is responsible and accountable for the provision of social work services to the patients, their families and significant others to assist them in coping with the impact of illness/disability, and to achieve maximum benefit from health care and community services. The Social Worker shall ensure that each patient is provided with coordinated care and services to attain and maintain the highest level of physical, cognitive, emotional, spiritual and psycho social well-being in accordance with the individualized comprehensive assessment and interdisciplinary plan of care. In addition, is responsible for the social work interventions as needed, supportive counseling, education and discharge planning services. #V1
Qualifications:
- Master of Social Work from an accredited school of Social Work education
- Current New Jersey Licensure to Practice Social Work from the New Jersey Board of Social Work Examiners (LSW)
- Minimum of two years' experience in a healthcare setting with children living with chronic and/or complex mental health conditions.
- Computer Literacy
Experience with the following:
Biopsychosocial assessments
Collaborative Care Planning and Coordination
Advocacy
Community Resource Referral
Supportive Counseling/Education
Discharge Planning
