What You Will Do to Change Lives
The Social Work Clinician III independently provides clinical and casework services to members served and natural supports. This involves person centered recovery planning and collaborating with other services and agencies. May act in the role of a clinical supervisor of unlicensed staff.

Expected starting wage range is $39.59 - $48.90. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

The Medical SocialWorker (MSW) contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.

JOB REQUIREMENTS
Master's of Social Work
Minimum of one year's experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
Demonstrates good verbal and written communication, and organization skills.
Possesses and maintains current CPR Certification.

Under the direction of the Facility Administrator (FA) or higher licensed medical professional, the Licensed Clinical SocialWorker (LCSW) or Licensed Marriage and Family Therapist (LMFT) will be responsible for providing diagnostic evaluations and therapy counseling using applied psychotherapy of a non-medical nature to persons served in compliance with all applicable LCSW/LMFT standards, state and federal rules and regulations.
JOB RELATIONSHIPS
REPORTS TO: Mental Health Director

DUTIES/RESPONSIBILITIES
Triages and conducts intakes, evaluations and/or assessments on patients admitted to outpatient housing unit and/or referred by Security, Healthcare staff, and other Community sources and makes appropriate recommendations to the healthcare unit and security staff regarding the direction or required level of care
Refer all 5150 eligible persons served to the County Mental Health and/or Emergency Room for evaluation
Recommends special housing or special watch and/or observations to security based on clinical findings
Identifies and conducts other essential treatment services such as, psychoeducational groups, art therapy, pre-release counseling and other transitional groups, when appropriate or when applicable
Recommends and provides treatment plans, such as other non-pharmacological treatments or services such as group or individual therapy, when appropriate, for persons served on his/her caseload and in all areas of the correctional facility with regular updates when needed
When applicable, initiates involuntary hospitalization for those persons served who need to be transferred to outside Mental Health facilities and generates appropriate paperwork relevant to the hospitalization
Assesses for Suicide Ideation and provides suicide intervention and management of suicidal and high-risk persons served
Conducts daily rounds and evaluations to assess the treatment needs of persons served that request or desire mental health services within the correctional facility setting
Hold daily sick call, evaluate patients, obtain medication history, verify current medication consult with medical provider/psychiatrist to continue psychotropic medications, and discuss conditions falling outside protocol perimeters
Recommends medication evaluations to the MD when necessary and coordinates closely with the Psychiatrist to ensure that persons served requiring evaluation or re-evaluation for medication are seen in a timely manner and the Prescriber is aware of any information pertinent to the patient's clinical condition
Upon receiving the necessary training and/or instruction, performs other related duties as required or assigned.

* Champions the vision and drives services delivery at affordable housing properties serving seniors by ensuring programs meet the needs and interests of residents• Manage all facets of programming and services by working collaboratively with community stakeholders• Implement on-site programs to meet the needs of residents, including:Adult Education and Job Readiness programs, computer literacy programs and services Health & wellness programs and to promote physical, mental health and general well being• Develop and implement community-wide events focused on developing safe and strong communities. • Develop and implement services based on community needs and resident feedback• Conduct one-on-one case management services to support residents of the community• Leverage, nurture and cultivate key community partnerships to forge new and/or stronger partnerships to maximize available programs and services for residents• Input data daily to track case management progress, demographic information, and program outcomes• Adopt a "customer service first" attitude that ensures residents, partners, and the community at large receives the highest quality of service in a caring and compassionate • Oversee the volunteer program, responsible for recruiting, engaging and managing volunteers• Perform all other duties as requested.
Qualifications
Required Experience: • Familiarity with the social services resources • Prior knowledge of affordable housing and area social service resources
Required Education/Training:
* Five (5) years of work experience in social services. A degree in related field a plus.
Required Skills and Abilities:
* Excellent verbal, written, and inter-personal skills• Knowledge of Microsoft Office, Google Apps and other software programs• Ability to safely lift 25 pounds
