Essential Functions
Assessment & Screening: Evaluates patients for transition planning, mental health, substance use, and goals of care.
Care Coordination: Develops and monitors care plans, addressing social determinants of health and community resources.
Behavioral Health Support: Uses motivational interviewing and therapeutic techniques to promote mental health care including women's services, behavioral change, trauma informed care, and substance use disorders.
Therapeutic Intervention: Provides brief individual, group, and family therapy, plus psychosocial assessments.
Diagnosis & Referrals: Identifies mental, emotional, and behavioral disorders and connect patients to services.

Minimum Qualifications
Master of Social Work (MSW) from an accredited institution (degree verification required).
Current state licensure, as applicable, is obtained prior to or upon completion of required supervision hours. (CSW/LCSW)
Basic computer proficiency, including familiarity with word processing and spreadsheet software.
Strong written and verbal communication skills.
Demonstrated ability to apply critical thinking skills.
Caregivers whose duties require them to conduct home or community visits must maintain current BLS certification, have a current driver's license, current auto insurance, an acceptable driving record and reliable transportation.
Preferred Qualifications
Case Management Certification.
Experience in clinical care management, social work, or working with third-party payers.
Demonstrated understanding of care management principles and practices.

Position Overview: The Intensive Outpatient Clinic (IOC) at University of Utah Health is seeking a Licensed Clinical SocialWorker (L.C.S.W.) or Clinical Mental Health Counselor (C.M.H.C.) to provide professional counseling services to a population of individuals with complex medical and psychosocial needs. Providing comprehensive and integrated care, the IOC operates as an interdisciplinary team of medical and behavioral health providers aiming to improve clinical outcomes and decrease the overall cost of care for the population we serve. In 2023, Governor Cox's One Utah Health Collaborative recognized the IOC has as leader in novel comprehensive care delivery and sustainable financial innovation. The IOC was also featured in Meet Me Where I Am - a short film that premiered in the 2022 Sundance Film Festival - that tells the story of one IOC patient and how this clinic provides unique integrated care.
This position provides direct therapy and works collaboratively with the medical and behavioral health teams to deliver comprehensive care oriented around treating the bio-psycho-social needs of the IOC patient population. Therapy activities include individual and group counseling. At minimum, the ideal candidate will have a working knowledge of trauma and substance use disorders. Case management experience is preferred. The IOC exclusively serves a Medicaid population and operates under a capitated model with the University of Utah Health Plans. No billing is required.
This position reports to the IOC Medical Director and will support the IOC patient population and the clinic team located at the University of Utah Population Health Center near Rose Park.

OVERVIEW OF THE ROLE

International Health and Medical Services is looking for an experienced licensed independent clinical socialworker who serves as a subject matter expert (SME) in mental health and substance use disorder(s). The Behavioral Health Provider (BHP) possesses a high level of skill in the assessment, diagnoses, and treatment of patients with mental health concerns, and/or serious mental illness or serious mental disorders (SMI/SMD). The BHP conducts mental health evaluations to enhance the behavioral health treatment plan development and services for detainees with SMI/SMD to include referral to acute and/or inpatient mental health care as clinically determined.
The BHP provides direct mental services, including on-site counseling, individual and group psychotherapy within diverse therapeutic modalities, substance abuse education, and behavioral analysis for detainees identified with mental health and substance use problems. This role conducts specialized programming within a Modified Therapeutic Treatment Community model. The BHP also consults with medical, contract staff, and other healthcare professionals to conduct and provide education and training on continuity of care, treatment, monitoring and management of patients with co-occurring diagnoses.
Provides direction and oversight to multi-disciplinary team in the implementation and management of an integrated mental health treatment program.
Provides direct care to patients within discipline Scope of Practice and performs functions in accordance with current accepted practice, licensure, certification, credentialing and or granted privileges.
Provides treatment of persons throughout the life span as appropriate to discipline and facility setting.

Required Education and Experience:

The job of Nutrition Services Worker is done for the purpose/s of providing support to the food service activities at assigned location with specific responsibilities for preparing and serving food items to students and/or school personnel; and maintaining food service facilities in a safe and sanitary condition.
Essential Functions:
Arranges food and beverage items at the request and direction of supervisors for the purpose of serving students and staff in an efficient manner.
Attends unit meetings, in-service training, workshops, etc. (e.g. Food Handler's Permit training, safety meetings, etc.) for the purpose of gathering information required to perform job functions.
Cleans utensils, equipment, and storage, food preparation and serving areas (e.g. oven, refrigerator, etc.) for the purpose of maintaining required sanitary conditions.
Cooks food for the purpose of meeting mandated nutritional and projected meal requirements.
Estimates food preparation amounts and adjusts recipes as needed for the purpose of meeting projected meal requirements and minimizing waste.
Inspects food items and/or supplies (e.g. flour, serving trays, etc.) for the purpose of verifying quantity, quality and specifications of orders and/or complying with mandated health standards.
Inventories food, condiments, supplies and equipment at specified intervals for the purpose of ensuring availability of items required for meeting projected menu requirements.
Maintains equipment, storage, food preparation and serving areas in a sanitary condition for the purpose of complying with current health standards.
Monitors kitchen and cafeteria areas for the purpose of ensuring a safe and sanitary working environment.
Performs functions of other nutritional services positions as requested by supervisor (e.g. account technician, lead cook, etc.) for the purpose of ensuring adequate staff coverage within site nutritional services operations.
Prepares food and beverage items (e.g. breakfast and lunch, etc.) for the purpose of meeting mandated nutritional and projected meal requirements.
Reports equipment malfunctions for the purpose of notifying supervisor of need for repair and/or replacement.
Serves one or more items of food for the purpose of meeting mandated nutritional requirements and/or requests of students and school personnel.
Performs other related duties as assigned for the purpose of ensuring the efficient and effective functioning of the work unit.
Requirements:
age 16+
High school diploma or equivalent.
Job related experience is desired.
Must be able to lift up to 50 lbs. repeatedly throughout a work day. Must be able to push/pull up to 150 lbs. repeatedly throughout a work day.
