Essential Functions and Responsibilities:
Provides direct social work service and counseling of patients and their families. Floats to other areas when needed. Gathers appropriate data and formulates relevant assessment of patient and family's medical, social, and emotional situations, using effective interviewing techniques, consultation with healthcare team and chart review. Assesses and identifies the social determinants of health of patients. Provides social work intervention and support for the complex social and emotional need of patients and their families. Direct clinical services may include individual and family counseling, crisis intervention, facilitating education and linkage with resources to patients, families, and staff as indicated by the assessment and treatment plan. Identifies and understands the behavioral and emotional factors underlying a patient's illness. Assesses and supports care coordination that meets individualized patient need. Provides crisis intervention and assists families in understanding the implications and complexities of the medical situation and its impact on one's lifestyle. Assists with discharge planning, and post-acute placement on complex discharges. Accurately documents all assessments, plans, interventions, patient/family involvement, outside agency communications, interdisciplinary contacts and case closure according to social work and patient care area standards. Flexible when assigned on other hospital floors/teams as needed by management. Actively participates in department activities including regular staff meetings, committees and task forces. Provides effective orientation, work direction, and guidance to less experienced social workers. Performs other duties as assigned.

Job Summary:
The Medical Social Worker (MSW) contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement. The MSW respects and promotes the mission, values and vision of VNA Health.
Job Duties:
Patient Assessment and Intervention
* Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
* Carries out social evaluations and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and problems arising from terminal illness, related conditions and the dying process." [Section 418.64(d)].
* Conducts an initial bereavement assessment of the patient and family focusing on social, spiritual and cultural factors and incorporate it into the POC [Section 418.54 (c)(7)].
* Maintains clinical records on all patients referred to social work. Completes thorough, accurate, and timely documentation as defined by agency policy.
* Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
* Provides information to patients and families/caregivers and community agencies.

WHAT WE OFFER The County of Ventura offers an attractive compensation and benefits package that includes: * Merit Increases: New employees are eligible for an initial 5% merit increase within the pay range upon completion of at least 1,040 hours (approximately 6 months) assuming work meets satisfactory standards. Subsequent merit increases within the pay range will be upon completion of each additional 2,080 hours (approximately one year) from the initial merit increase.
* Educational Incentive: An educational incentive of 2.5% for completion of an associate degree, 3.5% for completion of a bachelor's degree OR 5% for completion of a graduate degree.
* Bilingual Incentive: Based on agency need, proficiency levels by exam are $.69 per hour (Level I), $1.00 per hour (Level II) or $1.32 per hour (Level III).
* Flexible Workweek/Telework Schedule: We support work/life balance by offering compressed work (e.g., 9/80 or 4/10 workweek) and remote hybrid telework schedules for some positions subject to organizational needs and satisfactory performance.

THE SUCCESSFUL CANDIDATES
The successful candidates not only possess the minimum requirements of the positions but also possess advanced-level knowledge and experience optimizing water and/or wastewater service systems; experience utilizing the Supervisor Control and Data Acquisition (SCADA) system; and operations & maintenance (O&M) experience with a variety of pumps, control valves, instrumentation, reclaimed water treatment and distribution systems, sewer collection system cleaning, and other specialized areas. Education:
Minimum: Master's in Social Work (MSW) degree from a Council on Social Work Education (CSWE) accredited School of Social Work.
Certifications, Licenses, Registrations:
Minimum: Current California Clinical Social Work License (LCSW) required upon hire. California Children Services (CCS) paneling is required within six months of hire.
Preferred: California Children Services (CCS) paneling is required within six months of hire. Certified Social Worker in Health Care (C-SWHC).
Years of Related Work Experience:
Minimum: 1-2 years of relevant clinical social work experience required.
Preferred: 3-4 years of related experience in a healthcare setting preferred.
